DRUG STUDY

DRUG NAME Co-amoxiclav (Augmentin) DOSAGE 400mg/5ml 1 teaspoon 10mg Three times a day CLASSIFICATION Broad-spectrum Antibiotic ACTION - An antibiotic that combines amoxicillin and clavulanic acid. It destroys bacteria by disrupting their ability to form cell walls. - Clavulanic acid blocks the chemical defence, known as betalactamase, that some bacteria have against penicillins. l Co-amoxiclav is active against bacterial infections that have become resistant to amoxicillin. INDICATIONS - Known or suspected amoxicillinresistant infections including respiratory tract, skin and soft tissue, genitourinary, and ear, nose and throat infections. - Effective against strains of Escherichia coli, Proteus mirabilis, Haemophilus influenzae, Streptococcus faecalis, Streptococcus pneumoniae and some betalactamaseproducing organisms. SIDE EFFECTS - Hepatitis. - Dermatitis. - Dizziness. - Headache. - Convulsions (especially in high doses or in renal impairment). - Superficial teeth staining when using the suspension. - Phlebitis at injection site. - History of co-amoxiclavassociated or penicillinassociated jaundice or hepatic dysfunction. Cautions: - History of allergy. - Renal impairment. - Erythematous rashes common in glandular fever. - Cytomegalovirus infection. - Acute or chronic lymphocytic leukaemia. - Hepatic impairment. - Pregnancy. - Cholestatic jaundice. CONTRAINDICATIONS - Penicillin hypersensitivity. NURSING RESPONSIBILITIES - Assess bowel pattern before and during treatment as pseudomembranous colitis may occur. - Report haematuria or oliguria as high doses can be nephrotoxic. - Assess respiratory status. - Observe for anaphylaxis. - Ensure that the patient has adequate fluid intake during any diarrhoea attack. Patient teaching: - If the patient develops a rash, wheezing, itching, fever or swelling in the joints, this could indicate an allergy and should be reported. - Patients must ensure they take the full course of the medicine. - The medicine must

be taken in equal doses around the clock to maintain level in the blood. - Report diarrhea, cramping and blood in stools as pseudomembranous colitis may occur.

DRUG NAME Serrapeptase

DOSAGE 1 tablet 10mg Three times a day

CLASSIFICATION Anti-inflammatory

ACTION - The mechanisms of Serrapeptase consist fundamentally of a reduction of the production of the inflammatory reaction within tissue and an inhibition of the release of inflammatory mediators (histamine etc). - It induces the breakdown of fibrinose tissue and reduces the viscosity of exudates. The enzyme quite literally digests the inflammatory tissue. - In the case of bacterial infection, it has also been demonstrated that simultaneous use of Serrapeptase

INDICATIONS - Pain Reduction - Swelling - Fibrocystic breast disease - Sinusitis - Chronic Bronchitis

SIDE EFFECTS The fact is that there have been a few side-effects as reported anecdotally, though side effects are rare. These reports have included: - a worsening instead of lessening of pain or edema - a sudden onset of pain and tight muscles in the legs, knees and ankles - the sudden appearance of varicose veins; digestive upsets - increase in blood pressure - Lung congestion problems and even a case or two of pneumonitis.

CONTRAINDICATIONS - Pregnancy and breastfeeding: Not enough is known about the use of serrapeptase during pregnancy and breastfeeding. Stay on the safe side and avoid use. - Bleeding disorders: Serrapeptase might interfere with blood clotting, so some researchers worry that it might make bleeding disorders worse. - Surgery: Serrapeptase might interfere with blood clotting. There is a concern that it might increase bleeding during and after surgery. Stop using serrapeptase at least 2 weeks before a scheduled surgery.

NURSING RESPONSIBILITIES - If patient have bleeding disorder, consult with physician before using serrapeptase. - Watch for signs and symptoms of bleeding. Patient teaching: - Instruct the patient to take serrapeptase according to the physicians order. - Teach patient signs and symptoms of bleeding. Tell the patient too notify health care provider immediately. - Advice patient top avoid alcohol and aspirin.

DRUG NAME Methisoprinol (Isoprinosine)

DOSAGE 2 teaspoon Three times a day

CLASSIFICATION Immunomodulator Antiviral

ACTION - The action of Isoprinosine can be summarized as follows: - Normalizes the cell-mediated immunity by stimulating the differentiation of T-lymphocytes into T-cytotoxic cells and Thelper cells and increasing lymphokine production - Increases NK cell (natural killer cell) function - Increases the humoral immune response by stimulating the differentiation of B- lymphocytes into plasma cells and by enhancing antibody production - Increases the number of IgG

INDICATIONS - It is used in the treatment of acute and chronic viral infections. - Shortens the course of illness & reduces the severity of symptoms of resp infections, flu, measles, chickenpox, hepatitis A, German measles, herpes simplex, herpes zoster, epidemic parotiditis, infectious mononucleosis, viral encephalitis, subacute sclerosing panencephalitis (SSPE). Adjunct to cancer & TB chemotherapy

SIDE EFFECTS - dizziness - stomach pain - digestion problem - itching - lactic acidosis

CONTRAINDICATIONS - Hypersensitivity to any OTC drugs and Methisoprinol and its derivatives. - Pregnancy and breast feeding. - Presence of cardiac and liver disorder. - Patients with gout or has predisposition to gout.

NURSING RESPONSIBILITIES - Use cautiously in patients at risk for liver disease. - Monitor renal and liver function tests in these patients - Watch for signs and symptoms of lactic acidosis. - Instruct patient to take drug exactly as prescribed.
