SPECIFIC RESPONSIBILITIES SAFETY ENGINEER (SE) Safety Engineer will survey and inspect all company work activities, workshops, premises, plant, equipment and storage areas to ensure compliance with base regulations of accident prevention and all applicable standards AF EM 385 -1-1, US ARMY CORPS OF ENGINEER, SAFETY AND HEALTH REQUIREMENTS MANUAL.

It is Company_name's policy not to restrain, interfere, discriminate or reprise by virtue of employees participating in reporting of unsafe conditions. B.1 EMPLOYEES

All employees have the right to report unsafe conditions. Each employee is responsible for complying with occupational safety and health standards, rules and regulations which are applicable to his own action or conduct, and for reporting suspected unsafe or unhealthy working conditions. a. He should correct the condition himself if it is within his area of control. b. Report it orally to his immediate supervisor if he can not correct the problem. c. If the supervisor has failed the produce results make a written report to safety engineer, providing one copy to his supervisor. B.2 SUPERVISORS

Supervisors are responsible for identifying and correcting unsafe working conditions. a. He is responsible for correcting (or attempt to correct)unsafe conditions that are reported by his employee. b. If he can not correct the problem he should report it to the Safety Engineer. c. Make written report to company chief safety officer if safety engineer fails to correct the problem, (providing one copy to safety engineer.) B.3 SAFETY ENGINEER

It is the safety engineers responsibility to create a safe environment for employees on the construction site, for any identified problem. a. Safety engineer shall make his routine daily inspection to identify any safety violation or unsafe condition. b. Safety engineer is responsible for correcting any reported unsafe condition with the help of his supervisors and employees. (Hard had, safety shoes etc.) c. If he receives any written report from his supervisors or employees he

should report it to the chief safety officer or the company. C. ACCIDENT REPORTING AND RECORDS

* All accidents which occur incident to the site operations will be investigated and reported. * Safety Engineer will be responsible as the prime contractor for recording and reporting all accident exposure and experience incident to the work. This will include exposure and experience of contractors and its subcontractors. The records will include exposure work hours and occupational injuries and illness. * All diagnosed occupational injuries and illness that result in a fatality or a lost work day shall be investigated and reported to the Safety Engineer. * Safety Engineer will keep a record of employee exposure to toxic materials and harmful physical agents and will report any excessive exposures and the hazard control measures that have been taken. * Accident Investigation Procedure: Accident investigation (after rendering First Aid, and after ensuring the injured victim has been correctly cared for): a. Isolate the accident area b. If possible, establish from the victim what happened. c. Obtain eye witness statements of fact (not conjecture) d. Examine area of accident, look for existing conditions. For example, if the accident is inside, look at the condition of the floor, lighting, and slippery areas, obstructions, any mechanical equipment faults, defective tool cables, any exits blocked, defective stairs or any sign of unusual conditions. If the accident is outside, look for the climate conditions, rain, bright sunlight, visibility, as well as items stated above. e. Examine cause of accident, i.e. machine, equipment, tools, ladders stairs, inadequate lighting, substances, scaffolds, wiring, etc. If obviously faulty, broken, bent, damaged, do not interfere or move until accident investigation is complete. f. Ascertain from the victim or witnesses the actions of the injured person immediately prior to the accident. g. Was any second person involved, any if so, how why and when.

h. Accident time to be carefully noted. i. Was protective equipment or clothing required? Was it available? Was it being used? Was the victim using it correctly? j. Who first raised the alarm? k. How long was it before help arrived?. l. Medical reports from doctor/hospital as soon as injury is diagnosed. * Emergency telephone numbers and instructions for summoning ambulance, doctor, fire and police will be posted at the work site. D. 1. 2. TEMPORARY FACILITY PLAN Warning signs will be posted for non-potable water. A toilet facility will be constructed, which is composed of eastern type toilets. It will be so constructed as to be sound and protected against weather and falling objects Adequate ventilation shall be provided in the toilets. Windows and vents will be screened. Toilets shall be kept clean at all times: once a week they will be thoroughly cleaned and sanitised. In the toilets and near the work site washing facilities shall be provided with soap and a means of drying and a metal covering receptacle for waste. Sewer line of the toiled facility shall be connected to the base sewer system. An adequate number of waste receptacles shall be provided in the food service area. They shall be maintained in a sanitary condition. No food or beverage shall be consumed or stored in a area exposed to toxic effects.

E. REPORTING SAFETY INSPECTION Safety Engineer and supervisors will be responsible for routine daily inspections. Inspections shall be done prior to start of new phase of work and shall continue during the work. In case of any safety violations the work shall

stop until the corrective safety actions are taken. The following inspections forms are used: 1. Inspection checklist for construction activity. (A sample form is attached) 2. Inspection checklist for construction equipment. (A sample form is attached) F. JOB HAZARD ANALYSIS

A- SCAFFOLDS 1- Capacities. a)- Scaffolds and their components shall be capable of supporting without failure at least 4 times the maximum anticipated load. b)-Scaffold system components which are subjected to a bending moment (such as outrigger beams with suspended scaffold and counterweights ) shall be capable of providing resisting moment of at least four times the tipping moment. 2- Design. a)-The dimension of the members and materials used in the construction of various working platforms or scaffolds shall conform to the sizes shown in the tables and appendices in this manual. b)- Factory-fabricated scaffolds and components shall be designed and fabricated in accordance with the applicable ANSI standard. When there is a conflict between the ANSI standard and this manual concerning the design or fabrication of factory-fabricated scaffolds, the ANSI standard shall prevail. 3- Scaffolds shall be plumb and level. 4- Scaffolds (other than suspended scaffolds) shall bear on base plates upon sills or other adequate foundation. 5- Working levels of work platforms shall be fully planked or decked. 6- Planking. a)- All planking of platforms shall be either overlapped (minimum 30 cm. (12 in) or secured from movement. b)- Scaffold planks shall extend over their end supports not less than 15cm (6 in) (unless the planking is manufactured with restraining hooks or equivalent means) nor more than 30 cm. (12 in)

c)- Planking on scaffolds shall extend from the toe board to not more than 35cm (14 in) from the face of the building or structure unless standard guardrails are installed or personal fall protection systems are used; the maximum distance for outrigger scaffolds shall be 7.5 cm.(3 in) d)- Planking shall be supported or braced to prevent excessive spring or deflection and secured and supported to prevent loosening, tipping, or displacement. e)- When a scaffold materially changes its direction, the platform planks shall be laid to prevent tipping. (1)-The planks that meet the corner bearer at an angle shall be laid first, and extend over the diagonally placed bearer far enough to have a good safe bearing but not far enough to involve any danger from tipping, and (2)- The planking running in the opposite direction at an angle shall be laid so as to extend over and rest on the first layer of planking. 7- Work platforms shall be securely fastened to the scaffold. 8- Access. a)- An access ladder or equivalent safe access shall be provided. b)- Where a built-in ladder is part of a scaffold system, it shall conform to the requirements for ladders. c)- Climbing of braces shall be prohibited. 9- When the scaffold height exceeds four times the minimum scaffold base dimension (and including the width added by outriggers, if used ), the scaffold shall be secured to the wall or structure. a)- the first vertical and horizontal tie shall be placed at this point; b)- vertical ties shall be repeated at intervals not greater than 9 m (30 ft.) 10- The used of brackets on scaffolds shall be prohibited unless the tipping effect is controlled. 11-Use of the following types of scaffolding are permitted if they are designed constructed in accordance with ANSI A10.8, Scaffolds: a)-outrigger scaffolds, b)-needle beam scaffolds, c)-interior hung scaffolds,

d)-bricklayers square scaffolds, e)-float/ship scaffolds, f)-boatswains scaffolds, g)-window jack scaffolds, and h)-carpenters bracket scaffolds. B- LADDERS 1- The construction, installation, and use of ladders shall conform to the following, as applicable. a)- Safety codes for portable ladders, ANSI A14,1; b)- Portable metal ladders, ANSI A14.2; c)- Fixed ladders, ANSI A14.3; and d)- Job-Made ladders, ANSI A14.4. 2- Length. a)- All portable ladders shall be of sufficient length and shall be placed so that workers will not stretch or assume a hazardous position. b)- Portable ladders used as temporary access shall extend at least 0.9m (3 ft) past the landing. (1)-When a 0.9 m (3ft) extension is not possible, a grasping device (such as a grab rail) shall be provided to assist employees in mounting and dismounting the ladder. (2)-in no case shall the length of the ladder be such that ladder deflection under a load would, by itself, cause the ladder to slip from its support. c)-The length of portable step ladders shall not exceed 6m (20 ft). d)-When splicing is required to obtain a given length of side rail, the resulting side rail must be at least equivalent in strength to a one-piece side rail made of the same material. 3- Width. a)- The minimum clear distance between the sides of individual rung /step ladders shall be 40 cm. (16 in) b)- The minimum clear distance between side rails for all portable ladders shall be 29 cm (11.5 in). 4- Spacing of rungs, cleats, and steps.

On portable ladders, spacing shall be not less than 25cm. 5- Fixed ladders shall comply with the requirements in Appendix J. 6- Single-rail ladders shall not be used; three-legged ladders may be used for specific tasks if approved by the designated authority. C- WORK PLATFORMS 1- Manufactured work platforms shall be erected, used, inspected, tested, maintained, and repaired in accordance with the manufacturers recommendations as outlined in the operating manual or in accordance with guidance from the scaffolding, shoring, and Forming institute. A.copy of the manufacturers recommendations (operating manual) or guidance from the Scaffolding, shoring, and forming institute shall be available at the work site. 2- Work platforms shall comply with appropriate access and fall protection requirements of section 21. a)- All requirements of section 21. A shall be applied to work platforms and means of access. b)- Standard railing and handrails for work platforms shall be in compliance with the requirements of sections 21 and personal fall protection devices and safety nets shall be in compliance with the requirements of section 21.C. c)- Ladders used as work platforms shall be in compliance with the requirements of section 21.D. 3- Prior to commencing any activity which requires work in elevated areas, all provisions for access and fall protection shall be delineated in the hazard analysis, accepted by the designated authority, for the activity. 4- The following hierarchy and prohibitions shall be followed in selecting appropriate work platforms. a)- Scaffolds, platforms, or temporary floors shall be provided for all work except that which can be done safely from the ground or similar footing. b)- Ladders may be used as work platforms only when use of small hand tools or handling of light material is involved. c)- Ladder jacks, lean -to and prop-scaffolds are prohibited. d)- Emergency descent devices shall not be used as working platforms.

5- Erection, moving, dismantling, or altering of work platforms shall be under the supervision of a person responsible for job safety. 6- Work platforms shall not be erected or used in the immediate vicinity of power lines or electrical conductors until such are insulated, de energized, or otherwise rendered safe against accidental contact. 7- The use of work platforms for the support of on outrigger boom, hoist, well pulley, or any other device or equipment used for hoisting materials will be permitted provided the platform supports, and the individual member to which each device is attached, are reinforced and braced to withstand the additional loads imposed. 8- Where persons are required to work or pass under a working platform, a screen- consisting of no.18 gauge US Standard wire 12.5 mm (0.5 in) mesh or the equivalent-shall be provided between the toe board and the guardrail and extending over the entire opening. G. SAFETY PLAN

G.1 OBJECTIVE The objective of this safety plan is to establish and implement safety and health standards and requirements for the prevention of accidents and hazards. All provisions of the EM 385-1-1, US ARMY CORPS OF ENGINEER, SAFETY AND HEALTH REQUIREMENTS MANUAL will be confirmed and These rules will be strictly followed during the construction phase. Safety shall be appointed to act as the Safety Engineer who will be responsible for the implementation of this plan and for the prevention of accidents and material hazards. His Curriculum Vitae is attached. He will report directly to the Project Manager Mr. Project_manager G.2 GENERAL Regularly scheduled safety meetings shall be held at least once a month for all supervisors on the project to review past activities, to plan ahead for new or changed operations and to establish safe working procedures for anticipated hazards. An outlined report of each meeting shall be submitted to the designated authority. Call for these meetings will be done by posting date, time and place of each meeting on the bulletin board at least one week in advance. Attendance will be taken during the meeting. Those who do not attend shall be warned by the safety engineer. 2. Each individual employee shall be responsible for complying with applicable safety requirements, wearing safety hats.

3. Each employee shall be provided initial indoctrination by safety engineer and such continued safety training to enable him to perform his work in a safe manner. 4. Regularly scheduled safety meetings shall be held by the project Manager and the safety Engineer once a week to which all of the personnel will attend. 5. All persons required to enter into confined or enclosed spaces immediately dangerous to life shall be instructed as to the hazards involved and precautions taken. 6. Where employees are exposed to cold or heat they shall be instructed about the avoidance of injury and first aid procedures to be followed in the event of injury. 7. An adequate supply of drinking water shall be supplied. 8. An adequate number of waste receptacles shall be provided in the food service area. 9. Prior to start of work, arrangements shall be made for medical personnel to be available for prompt attention to the injured and consultation on occupational health. Measures to be taken as a minimum shall be as follows: a. Communication and transportation to effectively care for injured provided. A motor vehicle shall always be available for the transportation of ill or injured personnel to the nearby medical facility. b. The social Security Hospital of Adana shall be utilised in case of illnesses or injuries. c. A first aid kit shall be provided with essential medicine, drugs, and dressing materials and each type of item there in shall be maintained sterile and shall be easily accessible to ailing workers. d. Warning signs shall be placed to provide G.3 adequate warning of hazards

MATERIAL HANDLING, STORAGE AND DISPOSAL

1. All materials in bags, containers, or stored in tiers shall be stacked, blocked and limited in height so that they are stable and secured against sliding or or collapse. 2. Unauthorised persons shall be prohibited from entering storage areas. All

persons shall be far away from the point of operation while materials are being loaded or unloaded from the trucks. 3. Excess material and material to be dump area shown by contracting officer. G.4 FIRE PREVENTION

1. Fires and open flame devices shall not be left unattended. Only temporary heating devices approved by the designated authority shall be used. 2. Unopened containers of paints, varnishes, thinners and other flammable paint materials shall be kept in a well ventilated location, free of excessive heat, smoke, sparks, flame or direct rays of the sun. 3. No smoking, open flames or other sources of ignition shall be permitted in areas where spray painting is done.

4. All necessary permission shall be taken from base fire department before any work is begun. 5. Construction schedule shall be given to CO and base fire department before any power outage. 6. Do not permit open flames or sparks close to the fuel tanks. 7. Do not use open flames to examine drums, tanks or cans or other containers of cutback asphalt. 8. Properly and fully vent all vehicles transporting fuel or ext. G.5 1. FIRE PROTECTION Depending on working places, required amount of fire extinguishers shall be provided on each site. Two fire extinguishers shall be present in the storage area and office area. Fire extinguishers shall be distinctly marked, readily accessible and maintained in a fully charged and operable condition. 2. 3. Vehicle, equipment, materials and supplies shall be placed such that they won't obstruct the fire fighting equipment. In the case of big fires, which cannot be extinguished with the portable extinguishers, help shall be obtained from the Fire Department of Incirlik Air Base. GENERAL SAFETY PRECAUTIONS FOR SCAFFOLDING Erecting and dismantling of scaffolding shall be done under the supervision of safety engineer. to

2. Proper precautions shall be taken to present workers form slipping on scaffolding during rainy and snowy periods. Workers will not be allowed work on wet surfaces. 3. Scaffolding shall be kept free of obstacles and debris. 4. Defective and broken parts in the parts of the scaffolding shall be repaired or replaced immediately. 5. The passageways used as a bridge between scaffolding shall have guard rails. G.7 1. WELDING AND CUTTING All welding equipment shall be inspected daily. Defective equipment shall be

removed from service, replaced, and re inspected before being placed in service again. Object to be welded, cut or heated shall be protected from moved to a safe distance or if it cannot be moved, all combustible materials shall be protected from heat, sparks and slag of welding. Temporary power lines, switch boxes, receptacle boxes and metal cabinets shall be marked to indicate maximum operating voltage. All circuits shall be protected against overload. Ground Fault Interrupter (GFI) shall be provided on each circuit. Patched, oil soaked or worn cables shall not be used. HAND AND POWER TOOLS All hand tools shall be in condition and used only for the purpose for which were intended and designed. Tools having defects that will reduce their Safety shall be removed from service. Throwing tools or materials from one person to another or dropping them to levels shall not be permitted. Power tools shall be inspected, tested prior to use.

G.10 WARNING MARKING AND LIGHTING FOR AIRFIELD CONSTRUCTION OPERATIONS 1. The Contractor shall report to the designated authority before initiating any airfield construction work and shall notify the designated authority of proposed changes of locations and operations. The designated authority shall keep the airport owner or operator informed so Notice to Airmen (NOTAMS) can be issued to reflect hazardous conditions. 2. Construction and storage areas affecting aircraft movement areas shall be outlined during the day by red flags and at night by low intensity battery-operated electric red lights spaced

every 200 feet or less. All objects, placed in safety precaution areas, which project above the approach-departure clearance surface or above the transitional surface must be provided with a red light at night. 3. Nothing shall be placed in the aircraft movement area without authority of the designated authority. 4. Work shall be carried on in such a manner as to leave the portion of aircraft movement area which is available to aircraft free of hazards, holes, material piles, or projecting shoulders. The Contractor shall keep all paved surfaces such as active runways, taxiways, and hardstands, completely clear at all times, and specifically, free from small objects which might damage aircraft propellers or jet engines. 5. Waste material shall be removed often enough to insure that is does not create a hazard. Debris deposited on any active portion of a runway, taxiway, or apron shall be removed concurrently with the work. 6. Effective control of vehicles required to enter or cross aircraft movement areas shall be established and maintained. 7. All vehicles operating in aircraft movement areas shall be identified by a flag on a staff attached to and flying above the vehicle. This flag will not be less than 3 feet square and will consist of a checkered pattern of international orange and white squares of not less than 1 foot on each side. 8. All idle construction equipment shall be removed to locations 750 feet or more from the center line of the runway or to an area identified by the designated authority. 9. Only those trenches for which material is on hand and ready for placing therein shall be opened.
