A inibição destas enzimas atenua de modo significativo a degradação
proteolítica da matriz extracelular da cartilagem que ocorre na oa.
Antibióticos, tais como as tetraciclinas e suas formas semi-sintéticas
(doxiciclina e minociclina), podem inibir MP (colagenases e gelatinases), reduzindo a gravidade da oa em modelos animais. Atualmente há alguns protocolos investigando estas ações na OA de homens. Outros compostos como hidroxamatos, tióis e carboxilalquilas, ao atuarem interagindo com o sítio ativo da molécula de MP têm sido estudados em ensaios clínicos conduzidos em pacientes com artrite reumatóide e, também, naqueles com OA.
4. Cirurgia
A cirurgia deve entrar na planificação terapêutica quando a abordagem clínica
conservadora mostrar-se ineficaz.
Cirurgia profilática, como osteotomias para corrigir alterações posturais
biomecênicas, não é bem recebida, em nosso meio, pelos pacientes, talvez por falta de adequado convencimento por parte do médico que a propõe. A cirurgia corretiva de deformidades fixas ou de perda da função articular tem indicações precisas. Hoje estão muito em voga as artroplastias totais, mas, voltamos a insistir, ainda há lugar para as osteotomias que visam corrigir os desalinhamentos articulares e a concentração anormal de cargas.
A remoção artroscópica de corpos soltos e de fragmentos articulares dos
joelhos é um procedimento lógico, tendo em vista sua participação na perpetuação do processo. Muitas vezes, a lavagem articular fechada ou a artrolavagem percutânea (com material comum de toracocentese) podem oferecer resultados idênticos.
