Nursing Priority Problem List 1. Ineffective breathing pattern related to abdominal fluid accumulation compressing the diaphragm.

Rationale: Inspiration and/ or expiration that does not provide adequate ventilation. 2. Ineffective renal tissue perfusion related to decreased hemoglobin concentration in the blood. Rationale: Decrease in oxygen resulting in the failure to nourish the tissues of the capillary level. 3. Fluid Volume Excess related to compromise regulatory mechanism (Inability of kidneys to maintain body fluid balance). Rationale: Increase isotonic fluid retention. 4. Noncompliance to therapeutic regimen related to lack of financial resources Rationale: Behavior of person and/ or caregiver that fails to coincide with a health- promoting or therapeutic plan, person s or caregiver s behavior is fully or partially adherent and nonadherent and may lead to clinically ineffective, partially ineffective outcomes. 5. Deficient knowledge related to lack of information about the disease process, treatment and selfcare. Rationale: Absence or deficiency of cognitive information related to specific topic. Lack of specific information necessary for clients/SO to make informed choices regarding condition/treatment/lifestyle changes. 6. Hopelessness related to long-term stress Rationale: Subjective state in which an individual sees limited or no alternative or personal choices available and is unable to mobilize energy on own behalf. 7. Disturbed body image related to abdominal distention. Rationale: Confusion or dissatisfaction in mental picture of one s physical self. 8. Risk for impaired skin integrity related to accumulation of toxins in the skin. Rationale: Altered epidermis or dermis. 9. Risk for loneliness related social isolation. Rationale: At risk for experiencing vague dysphoria. 10. Readiness for enhanced coping. Rationale: A pattern of cognitive and behavioral efforts to manage demands that is sufficient for wellbeing and can be strengthened.
