MENTAL STATUS EXAMINATION ORIENTATION The client was able to respond when she was asked to sate her

full name. She is also oriented to date and place.When she was asked about the time, the client answered: Mga alas-diyez na siguro ng umaga ngayon. which shows that the client is also oriented to time. Interpretation The client is oriented to date, time, and place. Analysis Some people who have a history of mental illness can recognize other persons, aware of when and where they are presently are, and who are they. (Fundamentals of Nursing by Kozier et.al., volume1, 8th edition, page 642 ) ATTENTION The client has a short attention span in every activity that was being rendered to the group. She easily get bored and shows actions like crossing of arms or putting her hands on her face. When she is not in the mood on an activity, she will keep quiet and frown. However, if she is in the mood, she will do the activity. Interpretation The client has a short attention span and shows actions that reveals boredom. MEMORY

The client was able to remember her familys names, especially her parents. She can recall her provincial address, though not completely. She cannot recall the exact years of their stay in the province but she was sure that they already lived there since she was born. The client was able to reminisce some childhood experience. Interpretation The client can recall some people and events that happened in the past. Analysis

CALCULATION The client was able to count one-to-ten in a slow and fast manner without interruption and no help of other people. She can add and subtract numbers within five seconds intervals, however, she could not perform multiplication and division with ease. Interpretation The client can count numbers and do simple calculations like addition and subtraction. Analysis Patients with bipolar disorder may perform poorly at math due to difficulties

ABSTRACT During the remotivation therapy, when the student nurse showed a picture of an island and a sea with visible coral reefs, the clients perception on the picture was beautiful yet difficult to locate the place because she said she had not been in the place before. Through bibliotheraphy, when being asked about what is the possible moral lesson of the story Si Tunying Sinungaling, she answered Masama ang magsinungaling kasi hindi lang ang kaibigan ang mawawala, pati na rin ang pagtitiwala. When she was asked to paint anything, she drew a house with a tree on the side. When she was asked about her drawing, she said that it reminds her of their province where she and her parents lived peacefully. Interpretation The client was able to express what she wants or what she thinks. Analysis

A change in a bipolar client happens only during severe manic, mixed or
