I acknowledge that while I am working for [Company Name], I am expected to take proper care of
company equipment. I understand that upon termination, I am expected to return all property of
[Company Name] in proper working order. This agreement includes, but is not limited to, the

understand that continued failure to return equipment may be considered theft by the company
and may lead to criminal prosecution.
