Sample Board of Director Survey

Let me begin by thanking you for serving on our board of directors. Your role is extremely important in assisting us with meeting our mission. Attached is the survey we mentioned at our last board meeting. It is designed to help us identify weaknesses associated with our board of directors. The survey should take no more than 15 or 20 minutes to complete and the results will be presented to our board of directors. However, please note that your confidentiality will be protected. We ask that you not put your name on the survey and not identify yourself in any way. We also ask that you be as straightforward as possible in your responses. Please provide feedback in the comments section, especially input that can help us increase the functional roles and capacities of our board. When you have completed the survey, please place it in the enclosed self-addressed envelope, attach a stamp, and mail it. We ask that you not provide a return address to protect your confidentiality.
