Checklist for operators
This checklist is designed to be completed in conjunction with the conditions of your Flammable and Combustible Liquids Licence, Dangerous Goods Safety Management Act 2001 (DGSM Act), Dangerous Goods Safety Management Regulation 2001 (DGSM Reg) and Australian Standard 1940-2004 (The storage and handling of flammable and combustible liquids) (AS1940). It does not limit your legal responsibilities and obligations under the Dangerous Goods Safety Management Act 2001 or any other relevant legislation. Outstanding licence conditions It is recommended that: you make a copy of your licence conditions and check that each of the conditions are being complied with attach the copy to this form and keep for your records. (s83 DGSM Reg) The quantities stored on-site match those stated on your licence and the emergency manifest box. (s84 DGSM Reg) The Flammable & Combustible Liquids Licence is valid. (s82 DGSM Reg) Structural audit - tanks and associated equipment The number, contents and location of the tanks are as per licence conditions. (s83 DGSM Reg) Fill Points check as per licence conditions: Location (AS1940: 5.3.2) Liquid tight/dry break connections (AS1940: 5.3.1) Liquid level indication. (AS1940: 5.3.3) Venting check as per licence conditions: Type and capacity (AS1940: 5.4.1 and 5.4.2) Outlet height and location (AS1940: 5.4.4) Approved vent cap. (AS1940: 5.4.5) Structural audit tank/pipe work identification Tank markings are as per licence conditions. (s83 DGSM Reg) (AS1940 5.2.2) Aboveground pipes are colour-coded and/or labelled. (AS1940 6.2.1) Structural audit bunding Tank bund is sufficiently impervious to retain spillage, has adequate structural integrity, is able to contain at least the capacity of the largest tank, and is designed to enable recovery of spills. (AS1940 5.8) Bunding is free of cracks or defects. (AS1940 9.2.10) Bunding is clear of combustible materials. (AS1940 9.2.10) Bund drainage flows to sump. (AS1940 5.8.6) Drainage open/shut valve is obvious. (AS1940 5.8.6) Drainage valve is closed and locked. Sign is present stating BUND DRAIN VALVE - TO BE KEPT CLOSED AND LOCKED. (AS1940 9.2.9) Site perimeter A HAZCHEM outer warning placard is displayed at each entrance to the facility. (s51 DGSM Reg) A manifest of dangerous goods and/or combustible liquids (that complies with Schedule 4 of the Dangerous Goods Safety Management Regulation 2001) is kept in a red weatherproof container inside, or as close as practicable to, the main entry to the facility. Note: required for large dangerous goods locations only. (S.60 DGSM Reg) Fences and barriers are maintained for control of public access. Security access to area must be controlled. (AS1940 3.8.1)

The following signs are visible: (AS19403.8.2) DANGER - NO SMOKING - NO NAKED FLAMES 2 For flammable liquids, a class label and subsidiary risk label (if any) - minimum of 250mm each. For combustible liquids, a COMBUSTIBLE LIQUID sign. Structural audit fire fighting Fire protection equipment as required by Australian Standard 19402004 (The storage and handling of flammable and combustible liquids) and licence conditions: (AS1940 11.9, 11.10, 11.11, 11.13) Check number, type and location of extinguishers and test every 6 months. Where applicable: - hose reels with foam are tested every 6 months - automatic foam sprinklers are tested annually - hydrant points are tested as per Australian Standard 18512005 (Maintenance of fire protection systems and equipment) Spill kits and equipment are provided for the containment and clean-up of spills or leaks on-site. (AS1940 3.2.1) Structural audit minor storage Site checked for unlicensed storage, eg paint, drums, maintenance stores, labs etc. (AS1940 section 2) Check for safety and environmental risks, eg spillage containment kept closed, ignition sources. Note: If additional storage exceeds minor storage, it must comply with Australian Standard 1940-2004 (The storage and handling of flammable and combustible liquids) and licence must be amended accordingly. Procedures Tanks and equipment: (AS1940 9.3) Procedures are developed for maintenance of tanks, bunds, pipe work, pumps and other equipment, including dispensers and tank support structures. Wastewater treatment (from bunds and dispensing areas where applicable): (AS1940 9.3) Procedures are developed for maintenance of bund/sump waste collection, and treatment systems and disposal. Receipts and records are kept. Safety management system A safety management system has been developed, documented and implemented (for dangerous goods locations only). (S53 DGSM Act) The operator has notified the Queensland Governments Department of Emergency Services of the site being a large dangerous goods location (a receipt from the departments CHEM Services Unit acknowledging entry in the large dangerous goods locations database is held on-site). (s49 DGSM Act) A register has been established, containing a list of all dangerous goods and combustible liquids stored or handled at the facility and including current material safety data sheets. (s41 DGSM Reg) Hazards associated with the storage and handling of dangerous goods or combustible liquids are identified and recorded. (s17 DGSM Reg) Risk assessments carried out for the site are dated and recorded; records are held on-site. (s18 DGSM Reg) Documented operating, maintenance and emergency procedures covering all aspects of the day-to-day operation of the site are held on-site and are regularly reviewed. (s19 DGSM Reg) Processes are established for the investigation of accidents at the facility; dated written records are kept. (s46 DGSM Reg) All fire extinguishing equipment has been maintained and checked in accordance with Australian Standard 1851 (Maintenance of fire protection systems and equipment). Note: inspection records must be kept. Documented emergency procedures for dealing with a hazardous materials emergency on the premises are held on-site (includes documented emergency response procedures for various scenarios). (s45 DGSM Reg) Employees are provided with information, education and training in the safe handling and storage of dangerous goods and combustible liquids. (s29 DGSM Reg) For a minimum of five years, records are retained of staff induction, education and training.
