Application to establish and operate a medicinal marijuana alternative treatment center in the State of New Jersey.

Applicant: New Jersey Relief Initiative, Inc. Attestation of veracity of supplied material:

Criterion 1: Submission of Required Information Regarding Applicant & Facility Measure 1: The applicant shall provide the proposed legal name and the following documents applicable to the applicants legal status. Supporting documents are included as Appendix A. Certificate and Articles of Incorporation and By-Laws for Corporation Organizing documents for associations Evidence of nonprofit status Certificate of good standing issued by the New Jersey Secretary of State Measure 2: The applicant shall provide the proposed physical address of the ATC. Supporting documents are included as Appendix B. Letter of Intent for the 35 State Street property Only one location for ATC. Measure 3: The applicant shall provide evidence of compliance with the local zoning laws for address of proposed location for an ATC. Supporting Documents are in process and can be submitted as needed. NJRI is currently drafting a Zoning Certificate/ Occupancy Certificate Application for the New Jersey Meadowlands Commission for filing next week. Measure 4: The applicant shall provide evidence that all of the physical addresses and proposed locations provided in response to Measure 2 are not located within a drug-free school zone. The applicant shall provide the distance to the closes schools from the ATC. Supporting documents are included as Appendix D. Measure 5: The applicant shall provide a legible map or maps of the ATC service areas by Zip Code to be served by the ATC. Supporting documents are included as Appendix E

Measure 6: The applicant shall provide the role, qualifications, name, address and date of birth for each staff member and the role, name percentage interest, address and date of birth of each principal, officer, board member or partner of the ATC. As this is a non-profit there is no percentage of interest. Joseph James Coffey, President and Member of Board of Trustees Qualifications: Mr. Coffey is a licensed attorney with a strong background in financing and launching start-up organizations, with a specialty in managing businesses in strictly regulated industries. In 1996, Mr. Coffey began raising debt and equity financing, placing executive leadership, and providing other management and legal consulting services to the his firms clients. Ocala Poker and Jai Alai is one of the most recent companies Mr. Coffey has financed and managed. He assumed control of the business in 2006 and worked for a year and a half to finance and rebuild the struggling Jai Alai fronton. In December of 2007, Ocala Jai Alai received approval from Marion County to operate a Card Room and in May of 2008 it opened its doors with 28 tables and welcomed players from all over North Central Florida. Mr. Coffey's recent experience in successfully launching and sustaining a highly state regulated gaming business will be vital to financing, staffing and building NJ Relief Initiative's operation. Kevin Patrick Daly, Member Board of Trustees Qualifications: Mr. Daly has been involved in the financial markets for the past 20 years. For the past 11 years Mr. Daly has been involved with starting, financing and managing early stage start ups in numerous industries. In particular, he has worked with the management teams of these various companies in building a string corporate culture and in developing excellent and clear lines of communication. This has been the key to success in many of these ventures. Mr. Daly has been involved with Denver Canna Club from the very beginning and has helped build this company into one of the leading companies in the Medical Marijuana business in the state of Colorado. He received a BA in communications from the University of Colorado in 1987. Oleg Rutman, Member Board of Trustees Qualifications: As the founder of Denver Canna Club, Mr. Rutman created the largest Patient Collective in Colorado. Mr. Rutman has created a successful business plan for the 15 dispensaries associated with DCC. Through the leadership and vision of Mr. Rutman, DCC is the first to have its own compliance department, operation manuals, security department and one of the largest growing capacities in the state. He has assembled a talented team of growers that designed and patented a state of the art growing method. Mr. Rutman is originally from Kiev, Ukraine. He is a graduate of the University of Colorado, Denver.

Eric Scot Villency, Secretary Qualifications: Mr. Villency was President of Maurice Villency Furniture for over 12 years a high-end furniture design, manufacture, warehouse and sales business with warehouses and showrooms in NY, NJ and Washington DC. Mr. Villency brings insight and expertise in large warehouse management and logistics. In addition, he will provide critical management of the point of sales operation of the ATC, customer service and Human Resources as he has deep experience in managing a large work force both in sales and warehouse operations. Timothy P. Pellegrini, Treasurer Qualifications: Mr. Pelligrini is a seasoned business executive with over 20 years of experience with concentrations in accounting and investment banking. As a 13 year investment banker Tim interacted with various constituencies (investors, clients, lawyers, consultants, accountants) to successfully execute leveraged finance and mergers and acquisitions (M&A) transactions. Tim was also an accountant with Lehman Brothers and both an auditor and tax consultant for Price Waterhouse in Hackensack, NJ where he became a Certified Public Accountant (CPA). Tim received a Master's of Business Administration (MBA) from Columbia Business School and a Bachelor of Science (BS) from Boston College. Mr. Pellegrini's deep experience in accounting and auditing will be vital to the operations complex data and inventory management. In addition, Tim's expertise in business management will contribute to the overall execution of the business model and help greatly with its growth. Joseph John Coffey, Law Enforcement Consultant Qualifications: NYPD Detective Coffey (Ret.), a 30-year veteran of law enforcement, is a worldrenowned expert in Terrorism, Serial Killers, Narcotics Trafficking and Organized Crime. He is also a qualified expert in the State of New Jersey on Organized Crime its members and associates for exclusionary hearings in Atlantic City. Mr. Coffey with Mr. Taylor (see below) with be in charge of law enforcement relations with the state and local authorities to insure coordination and communication with these entities. He will also be overseeing security of the operation and oversee strict compliance with the rules and regulations of the law.

Ronald R. Taylor, Law Enforcement Consultant Qualifications: During the past 40 years, Mr. Taylor has conducted several thousand narcotics investigations involving individuals, corporations and large-scale criminal organizations. These investigations involved widespread complex violations by subjects with very complex interrelationships and diversified interests that were difficult to establish. As a Federal Agent, these investigations required extensive planning and coordination with other Federal, State, and local law enforcement agencies, relating to investigative matters. Mr. Taylor has testified on several thousand occasions in numerous Federal and State courts relating to numerous investigative matters, resulting in approximately 1000 convictions and the seizure and forfeiture of hundreds of millions of dollars in currency and assets derived from illegal activities. Mr. Taylor will be in charge of managing the sensitive relationship with Federal Law Enforcement authorities as it relates to the issue of NJ Relief Initiative's medical marijuana cultivation and sales organization. In addition, he will join Mr. Coffey in overseeing the strict compliance with NJ law. Megan A. Sanders, DCC Operations and Compliance Officer, NJRI Consultant Qualifications: Over the past 6 years, Ms. Sanders as acted as Director of Compliance and Process Management for a private investment firm in Boulder, CO. This position has required a detail study of financial regulation and laws pertaining to a private investment vehicle. Her experience in a highly regulated industry has transferred perfectly to her position as Operations and Compliance Officer for Denver Canna Club. Ms. Sanders has a working knowledge of dayto-day operations from the compliance perspective. This knowledge allows for constant assessment and improvement of our processes to ensure full legal and health code compliance. Ms. Sanders has also served as the director of Boulder Youth Football since its inception in 2009. BYF is a Colorado non-profit serving 300 plus youth football players ages 8 13. She has been a critical component in funding and budget management for the league. She is also solely responsible for managing the league on a daily basis. The league operates year roundFall tackle season, Winter off season training and Spring flag league. BYF is committed to providing a football experience for any youth, regardless of ability to play. Ms. Sanders has been successful in securing funding to support 30% of the league that applies for scholarships. She has created and managed an asset determination and fee scale for any family requesting financial assistance. Measure 7: Disqualifying Drug Offenses- N/A Measure 8: N/A

Criterion 2: Submission of Required ATC Operational Information. Measure 1: Draft of operations manual and training plan addressing compliance with Subchapter 9 of N.J.A.C. 8:64 addressing ATC General Administration Requirements for Organization and Recordkeeping. Supporting documents are included in Appendix G. Measure 2: The applicant shall provide a description of how the ATC will operate on a longterm basis as a not-for-profit entity and a business plan that includes, at a minimum, the following: I The applicant shall provide a detailed description about the amount and source of the equity and debt commitment for the proposed ATC. a. Immediate and long-term financial feasibility of the proposed financing plan. NJ Relief Initiative will initially raise $500,000 from a loan from its operating contractor Denver Canna Club to launch the business to prep pre-construction and build out of the first grow room of the ATC (i.e., Architect, Engineering, Permits Utility Connection, Insurance, construct 3000 sq foot grow room, etc.) For long term, growth to meet the expected eventual demand, NJRI will raise $9,000,000. The feasibility of this is high due to the following factors: High Margin Product (80%); Relatively high return (20-27% interest on loan); Exclusive License; Low Execution Risk. In the extensive experience of our three principals, these factors are extremely attractive to both their investors and investors in general. However, it is very difficult to have a hypothetical conversation with a potential investor and it is next to impossible to collect firm commitments from investors without the license in hand.

b. Relative availability of funds for capital and operating needs. New Jersey Relief Initiative, Inc. has employed Middlebury Group, LLC, a New Jersey based Investment Banking and Advisory Firm. Middlebury has worked on numerous banking transactions over the years and conducts its business through its wholly owned subsidiary, Middlebury Securities. Middlebury has agreed in principle to effectively advise and manage a capital raise of $9 mm for the sole purposes of working capital to facilitate the build out of NJRI warehouse grow facility as well as provide additional working capital to commence the start up operations. Middlebury has already identified immediate available bridge financing which would be available once our license is granted to commence the start of New Jerseys build out and operations while the additional capital raise is taking place.

II.

The applicant shall provide a copy of its proposed policy regarding charity care/servicing indigent patients. Our proposed policy is to have a sliding scale policy similar to the New Jersey Charity Care Program structure. Below is an example of that policy.

We will create an Assets Criteria to help determine eligibility to the program. One of the prime objectives of our non-profit is to provide medicine for people in need.

III. Policy related to disposal of returned or unusable marijuana. Denver Canna Club is committed to conserving natural resources and being an industry leader in our environmental policy. One key area of our policy is developing new methods of disposal of waste and unused product. We make every attempt at full recycling of our product. Production of Biodiesel from Medical Marijuana Waste Material Biodiesel is a renewable alternative fuel produced from a wide range of vegetable oils and animal fats. Pure biodiesel or biodiesel blended with petroleum diesel can be used to fuel diesel vehicles, providing energy security and emissions and safety benefits. US Department of Energy website "We think biodiesel from hemp is an easy biodiesel to make," UConn Professor Richard Parnas Bio Researchers at UConn have found that the fiber crop Cannabis sativa, known as industrial hemp, has properties that make it viable and even attractive as a raw material, or feedstock, for producing biodiesel sustainable diesel fuel made from renewable plant sources. Basically methyl esters, or biodiesel, as it is commonly called, can be made from any oil or fat, including hemp seed oil. The reaction requires only oil, an alcohol (usually methanol) and a catalyst (usually sodium hydroxide [NaOH, or drain cleaner]). The reaction produces only biodiesel and a smaller amount of glycerol or glycerin. Producing oil from the plant material The oil can be extracted from the plant material using a screw press like the one show below. This kind of oil screw press is composed of feeder, gearbox, pressing cage, screw shaft and machine stand etc. It has better results to press soybean and cotton seeds. It utilizes the screw shaft that is set-up with different styles of screws and rotates when machine is running to push the material into chamber. Then the oil is separated out through the extrusion of pressing worm and pressing ring.

Refining the oil into Biodiesel


HOW IT'S MADE Biodiesel is actually very simple to make. It is made by chemically altering the molecular structure of any organic oil through the use of a chemical catalyst and an alcohol. To do this, oil is simply heated to a designated temperature (to help with the chemical reaction) and then a mixture of catalyst and an alcohol are added to the oil. The oil, catalyst, and alcohol mixture are then mixed for a period of time and then allowed to settle. If successful, the chemical reaction between the oil, alcohol, and the catalyst will have broken down the oil into several layers. The top layer will be biodiesel, chemically called an Ester, the next layer may contain soap, and the bottom layer will be glycerin. Once the layering has occurred, the glycerin and soap are drained off. The biodiesel is then washed with a mist-wash, a bubble-wash, or both. The washing is done to remove any additional soap, alcohol, or other impurities in the biodiesel. After it's been washed, it is then dried to remove any water. Commonly it is then filtered through fuel filters and is then ready to be used. Refining System The B-60 Home Biodiesel system is the easiest Biodiesel processor to use. The system will be more efficient if we add 3 cooling tanks. Additional chemical needed include: Sodium hydroxide Methanol (this will act as a catalyst) Byproducts The byproduct from the processing will include: Glycerin Dry plant matter This glycerin can be filtered to remove any plant particles or impurities, and used as an industrial degreaser in its raw form or made into bar soap. Bar soap made from glycerin byproduct is excellent for use in the shop because of its degreasing abilities, but can also be used as a household soap for everyday use. Both glycerin and Dry plant material can be combined composted and used as a fertilizer for the next generation of plants or sold as fertilizer. The THC (short for tetrahydrocannabinol) is stored in the plants oil, once the oil is extracted from the plane so is the THC. By co-opting with other growers we can help them turn their plant material into more Biodiesel. Why Produce Biodiesel? The Biodiesel we produce will be used as fuel for our generators. The generators will provide needed back up electricity for our operation. Not only is air pollution reduced but we can now use waste products to produce energy.

Criterion 3: Community Input- Describe the ATC planning process and involvement of community stakeholders in detail. Describe remaining steps if any to receive necessary approval for site location or operations. Measure 1: Input from the city or town where the applicants ATC would be located. NJ Relief Initiative's President Joseph Coffey met with the Mayor of Moonachie on February 8, 2011 to introduce the potential for and ATC in his borough. The Mayor listened and indicated that he would keep an open mind, but the decision to approve ultimately lies with the New Jersey Meadowlands Commission and the borough's council. The Mayor indicated that the Council would be meeting on February 17, 2011 and approval of NJRI's request would rest on that meeting. On February 10, 2011, our President met with principals of the New Jersey Meadowlands Commission and requested approval for the proposed use to grow and sell medical marijuana within the confines of the Meadowlands. During the meeting, we were informed that our use would most likely be approved, but the ultimate authority rests with the borough of Moonachie. We are currently preparing an application for approval of our use for submission to the New Jersey Meadowlands Commission's consideration. Measure 2: Input from the general public regarding the suitability of the applicant and the general standards for the location such as, distance from a school, daycare center or other childoriented location; distance from a commercial shopping district, pharmacy: etc During our meeting with the Mayor of Moonachie, we discussed the remoteness of our location relative to community activities and facilities like schools, commercial shopping districts ,etc. and the Mayor expressed that he thought such a location would be a positive element in our favor for any ultimate acceptance by the Council.

Criterion 4: Dispensary specific considerations. Measure 1: Through our patient management system, MMJ Menu, we will make any adjustments necessary to meet New Jersey HIPAA requirements with regards the handling of confidential health care information of our patients. New Jersey Relief Initiative, Inc. counseling and educational materials for registered patients and their primary caregivers regarding methods of administration and research studies on health effects of medicinal marijuana are prepared by Denver Canna Club, LLC. Denver Canna Club, LLC is a reputable and experienced Colorado management company specializing in the Medical Marijuana industry. The customers will be provided with a form to fill out at their first visit to ATC. This form will contain questions regarding their preferred way of receiving information. The patients will also be provided with: Informational brochures A current monthly education class schedule Seminars Webinars Access to the online library A comprehensive interactive website showing their purchasing history and preferences providing a blueprint for the health effects of different types of medicinal marijuana. Patient Hot Line to ask questions regarding their medicine Patients will have an opportunity to sign up for electronically distributed newsletter with industry updates, research studies, updates on state rules and regulations. The information will be obtained through established leading Medical Marijuana doctors. These Doctors have served on the Department of Revenue Advisory Board during the rules creation sessions, evaluated and recommended Medical Marijuana to numerous existing patients. The attorney firm currently exclusively representing Canna Club, LLC in Colorado, will handle legal questions. The attorney firm has a currently established working relationship with New Jersey Relief Initiative, Inc. In addition we will work through entities like Phoenix Tears foundation, Full Spectrum Lab (Medical Marijuana testing lab), Greenway University Americas first and only state approved and regulated MMJ education institute.

Measure 3: The applicant shall provide an acceptable safety and security plan, including staffing and site, and a detailed description of proposed security and safety measures which demonstrate compliance with the Rules Related to the Medicinal Marijuana Program.

Measure 5: The applicant shall submit a description of its Medical Advisory Board, including by-laws, setting forth the names and expertise of its members and describing how it will function within the organizational structure of the ATC, consistent with the Rules Related to the Medicinal Marijuana Program. The board will be comprised of three New Jersey licensed health care professionals, at least on of which is a physician; one patient registered with the ATC; and one business owner from the same region as the ATC. The By-Laws will be completed by the advisory board. They will be given the following outline as a guide. 1. Statement of NJRI Board Members a. Plan for term lengths b. Plan for election of new members 2. Purpose, Roles and Duties of Advisory Board a. Purpose of the Board is to support the patients supported by NJRI. b. Role of the Board is to serve as a representative link between the NJRI, the medical community and the patients. c. Duties of the Board are including but not limited to: Reviewing NJRI policies and performances for recommendations, Review and make suggestions for research grant awards, Promote communication among the medical community and alternative treatment centers, Participate in appropriate activities that enhance NJRI and its ability to serve patients, recommend changes to by-laws, but need approval of the NJRI Board of Directors to make those changes. 3. Measure 6: The applicant shall submit a plan to track and analyze data including but not limited to patient outcome, utilization and trends. Our experience in Colorado has allowed us to create professional partnerships with many industry leaders. One of those leaders is MMJ Menu. They have worked with us extensively to create, develop and implement data tracking and analyzing pieces to their software program. We are able to track every aspect of a plant from seed to patient. We are also able to track every aspect necessary for us to in compliance with the local Medical Marijuana laws. In Colorado, we are required to report all transactions of a patient. Measure 3: The applicant shall demonstrate an ability to provide a steady supply of medicinal marijuana to registered qualifying patients. I. Applicant shall provide a start-up time table. This is in Appendix H. II, III, IV: Knowledge of organic growing practices Denver Canna Club (DCC) is leading the Medical Marijuana growing community by implementing the highest standard of organic growing methods. While official standards have yet to be codified for growing certified organic Medical Marijuana, DCC is looking to the regulations of the National Organic Program set forth by the Organics Food Production Act to guide us in meeting a possible future 'organic' certification. DCC emphasizes the use of renewable resources for producing our product and refrain from using conventional pesticides and fertilizers for treating our plants. For example: Our soil method of growing uses a dry mixture of guano's, meals, and minerals combined with an organic liquid feeding program that utilizes fish and kelp. The main ingredients ensure a growing method that uses all organic materials. Our fungicides are organic as well. We use sulphur, copper, pyrethrum and neem oil all of which are natural and non-synthetic. By using soil inoculators and composting, we can reuse our potting soil ensuring our entire method is renewable, bio-degradable and eco-friendly. A commitment to the safety and well being of our patient requires a commitment practicing and continuing education in organic farming. This is the future for the Medical Marijuana industry. Procedures for routine scouting of insects and plant disease Growing a mono-crop indoors creates a set of challenges different from farming outdoors. Without the use of synthetic pesticides, eradication of pests, such as mites, is nearly impossible, so by implementing the Integrated Pest Management system we can greatly reduce pest infestations. The Integrated Pest Management system works in a three stage process: prevention, observation and intervention. Prevention is critical to an indoor growing environment. By maintaining a clean and sterile environment, pests/molds have little to feed on and can easily be located. Our grow facilities a strict schedule for regularly cleaning rooms, floors, nooks, crannies and work surfaces. Each facility has locker rooms and showers for employees to change into a clean uniform to help stop the spread of unwanted pests/molds through body and clothing. Along with these precautions, growing healthy plants ensures their own natural defense and immune systems are working properly to help ward off any unwanted pests/molds. Keeping rooms well ventilated, dry and clean is the best prevention for keeping molds from growing.

Observation of every plant and its grow space is key to detecting a threat before it has the ability to become a problem. Observation has two parts, inspection and identification. A daily visual inspection is as simple as looking at the plant either through the naked eye or a magnifying scope looking for signs of mold, physical damage, discoloration or pests. Once a problem has been observed the next step is to identify the problem so appropriate action can be taken. Intervention is eliminating the threat. During the different stages of the plants growth different methods can be used. During pre-flowering the plants can have sulphur applied directly through dusting or by burning the sulphur creating a fog effect that coats the plants and room to prevent and cure molds and pests.

Controlling insects without pesticides During flowering if conditions are kept right molds are rare and pests can be treated with predator mites, lady bugs and preying mantises. Proper sanitation practice, secured disposal area If a plant becomes diseased and must be discarded the proper procedure is to bag that plant and contain it in a sealed trash can until it can be discarded in a secured trash receptacle outside of the growing environment. Testing for mold / Quality Control: purity potency and consistency DCC requires a regulated schedule for our in house lab to takes samples to do microbiological screening, tests for pesticides and fungicide residue, foreign matter testing and cannabinoid testing to determine the level of quality for each batch. Our quality control experts take air samples and surface samples to determine the levels and types of molds that are in the grow facility. Each crop is grown with its own unique recipe to ensure consistent potency and purity. Each strain has specific needs. The key to our successful cultivation is understand the requirements of each strain and managing the growth cycle effectively. Preventing Seed production To prevent seed production and flowering male plants from spreading their pollen: DCC only clones proven female strains of Marijuana to ensure complete control of all plants in flowering area. We also build our growing environments to make sure no light leaks are ever allowed to disrupt our plants night cycle which might induce a female plant to hermaphrodite causing that plant to pollinate itself and possibly others. Having a quarantine room for all new plants that arrive is our way to allow some time to pass to determine the plants sex and determine if it should be allowed to go into production. Most important is training our staff to recognize male traits and to cull that plant before it has a chance to spread its pollen. Again cleanliness will help prevent spreading unwanted pollen from entering our growing rooms when each employee changes clothes and washes before entering the rooms.

Airflow method Our airflow method relies on temperature controlled rooms that stabilize our temperature between 72 and 76 degrees when lights are on and as low as 55 degrees when the lights are off. Depending on the climate we provide an air exchange rate of once every minute to once every five minutes. Dehumidifiers are used to keep moisture levels in check and prevent mold.
