Standardized Testing Registration Form

Please complete the information below and email this form back to hollycraw@gmail.com In order for the test results to be sent back to you directly, and not to me first, you will need to contact BJUP on your own and order the test. You will be asked to set up an account and pay with a credit card over the phone. You will also complete a Homeschool Verfication Form over the phone or online. The test will be sent to me as the test administrator, and I will let you know when I have received it.. BJUP will ask for a testing date that is about 2-3 weeks away. In my experience, they will send the tests as soon as the Homeschool Verfication Form has been processed in their office, and I receive the test much sooner than the 2-3 weeks. (During the summer, I have gotten them in 3-5 days.) It is imperative that you are able to complete the ordering by the date specified below in order for us to get the materials on time for the given test date. If the shipment will definitely not arrive on time, it can be sent express delivery, which will be a $30 charge to your account. Student information: The test cost for each student is $44 no matter who orders them. The administrator fee will be based on the total number of students according to the chart below, and you will be sent an invoice for that amount when the registration has closed.
