1.

KEY OBJECTIVES To identify and reduce the risk in all of our work activities that may result in personal injury, illness, property or environmental damage. To provide employees/contractors with information and instruction to increase their understanding of workplace hazards, to initiate and implement safe work practices and manage emergencies and comply with legislation and acceptable industry standard practices. Involve employees/contractors and clients in occupational health and safety matters and consult with them on ways to recognize, evaluate and control workplace hazards. To ensure that employees/contractors on project sites comply with specific site occupational health and safety requirements and the Policrete Concrete Grinding and Polishing occupational health and safety procedures To actively promote safe working practices for employees and contractors, and the ongoing development of occupational health and safety policies and procedures. To maintain records of documentation relating to occupational health and safety.

2. RESPONSIBILITIES 2.1 POLICRETE CONCRETE GRINDING AND POLISHING MANAGEMENT The general responsibilities of the Policrete Concrete Grinding and Polishing Project Manager, Construction Manager and Site Manager are to; Ensure that the Policrete Concrete Grinding and Polishing OH&S Policy and Procedures and the Site Safety Plan are effectively implemented in their areas of control. Take all practical measures to ensure that the workplace under their direct control is safe and without risk to health, and that the behavior of all persons in the workplace is safe and without risk to health Detect any unsafe or unhealthy conditions of behavior. Report the matter promptly to senior management, together with any recommendations for remedial action if they do not have the necessary authority to rectify a problem. Monitor the implementation of health and safety management systems, and report to Senior Management on performance. Advise others of their responsibilities, identifying unsafe working conditions or practices, carry out inspections and make corrections to defects as part of a total hazard management package. Develop an operational safety plan in consultation with the trade foremen and other groups as required. Ensure operations are adequately planned including the completion of a risk assessment. Ensure that all documentation associated with the management of the Site Safety Plan is completed.

Ensure that all employees are adequately inducted and trained to perform the tasks that they will be doing or to operate the plant/site equipment in use. Ensure that the plant/equipment is maintained in accordance with safety procedures and instructions of the manufacturer and those records are available for review. Ensure effective emergency response procedures, instructions and plans are in place for each section of the work. Be involved in the investigation of any hazards/incidents and the preparation of reports. Be responsible for the selection, monitoring and control of contractors and subcontractors to ensure adherence with health and safety standards. Ensure that all necessary applications are lodged with the appropriate authorities as part of the project planning process.

2.2 EMPLOYEE RESPONSIBILITIES The general responsibilities of an employee of Policrete Concrete Grinding and Polishing is to: Take responsible care for the health and safety of other people at work who may be affected by employees acts or omissions. Cooperate with management, to enable compliance with any requirements of the OH&S Act and associated regulations. Not intentionally or recklessly interfere or misuse anything in the interests of OH&S Not intentionally hinder aid to injured workers without reason. Not refuse a reasonable request to give aid to an injured person. Not refuse a reasonable request to do any act or thing to avoid OH&S risks. Not deliberately create a risk, or appearance of a risk, with the intention of causing a disruption at that workplace. Take reasonable steps to prevent risks by notifying the site manager, site safety representative of OH& S matters. Participate in on the job training. Perform works by following standard work procedures. Carry out hazardous work with the appropriate permit and in accordance with that permit.

2.3 CONTRACTOR RESPONSIBILITIES All contractors/subcontractors must provide a certificate of currency for Workcover and public liability insurance cover to Policrete Concrete Grinding and Polishing before any work may begin. The OH&S responsibilities for Policrete Concrete Grinding and Polishing contractors/subcontractors include; Provide a safety management plan for the site. To ensure that the health and safety of members of the public is not affected adversely by their work activities. Undertake a risk assessment on the work to be performed and provide a written Safe Work Method Statement for the site. Work with due care and consideration to safeguard their own health and safety and the health and safety of others, and to report any potential hazards, mishaps, incidents or injuries that may occur during the course of work. Comply with all safe working practices and procedures and observe all instructions issued by Policrete Concrete Grinding and Polishing management to protect their health and safety and the health and safety of others. Working towards achieving set OH&S targets and target completion times.

The accountabilities for contractors/subcontractors include; 2.4 VISITORS RESPONSIBILITES The responsibilities of visitors include; Obey any reasonable instruction given in relation to OH&S. Comply with the Policrete Concrete Grinding and Polishing OH&S policies and procedures ensuring that they are not, by their consumption of alcohol or drugs, in such a state to endanger themselves or others. Ensure that people are not exposed to health and safety risks arising from their conduct while they are on site. Familiarize themselves the Policrete Concrete Grinding and Polishing emergency procedures. Comply with all safe work practices and procedures Ensure that all workers complete induction training prior to commencing work. Report all incidents, accidents, and injuries prior to completion of the working day. Participate in safety meetings as requested. Comply with all external safety and health instructions issued by the Work Cover Authority. Provide all relevant information regarding hazardous substances or atmospheric monitoring or medical surveillance

Where necessary, personal protective equipment must be worn. Not perform any actions, which have potential to cause harm to others and the environment.

4. GUIDE TO THE USE OF DOCUMENTS AND FORMS IN THE SITE SAFETY PLAN J.0.01 IMPLEMENTING THE SITE SAFETY PLAN An introduction to the obligations of all Policrete Concrete Grinding and Polishing employees and other stakeholders with respect to site safety within the general requirements of the Policrete Concrete Grinding and Polishing OH&S Management System. ln particular stakeholders should understand their personal responsibilities when entering a Policrete Concrete Grinding and Polishing managed site and who are the members of the site safety team and their specific responsibilities. J.0.02 CONTRACTOR HEALTH AND SAFETY GUIDELINES The guidelines outline the minimum health and safety standards for contractors undertaking work on projects managed by Policrete Concrete Grinding and Polishing. The guidelines should be followed as a minimum requirement, but do not replace practices and procedures of an equal or more stringent nature that may be followed by contractors. Contractors are required to read and understand the requirements and ensure that the relevant requirements are included in their Safe Work Method Statement (SWMS). CONTRACTOR AGREEMENT This form must be completed and signed by the Contractor to verify that the person responsible for the project within the Contractors organization has read and understood the Health and Safety guidelines, Completed by: SM/Contractor File: SITE J.0.03 CONSTRUCTION SITE ESTABLISHMENT CHECKLIST This checklist is to be completed by the Policrete Concrete Grinding and Polishing Site Manager or their representative during the site establishment stage of a project, this is to ensure all specific requirements of the OH&S Management system have been addressed and implemented. Its recommended that the checklist be updated as the site is progressively established. ln particular re-visiting the "comment / corrective action needed" section of the checklist to ensure the corrective action has been addressed and completed, to the satisfaction of the Policrete Concrete Grinding and Polishing Site Management team, is an important requirement. Completed by: PM/CM/SM File: SITE J.0.04 OH&S INSPECTION CHECKLIST The Policrete Concrete Grinding and Polishing Site Manager or their representative is required to undertake regular inspections of the site to ensure all Policrete Concrete Grinding and Polishing employees and contractors are complying with the requirements of the Site Safety Plan and that all safety equipment and personal protective equipment is being properly utilized, is in good order and is being controlled by an authorized person and that all work areas are clear: clean and free of hazardous materials and substances. J.0.04i OH&S AUDIT This form is to be completed by the Policrete Concrete Grinding and Polishing OH&S Officer or an external party to ensure the management system is in order and up to date. J.0.04i contains an inspection checklist. Should an inspection indicate that there is a breach of Policrete Concrete Grinding and Polishing Policy & Procedures a Site Instruction should be issued with action items and deadlines. To be completed on a monthly basis. Completed by: OH&S REP File: OFFICE J.0.05 SUBCONTRACTOR COMPLIANCE FORM This form is to be completed by Policrete Concrete Grinding and Polishing site management for every contractor / sub-contractor starting work on the site, to ensure that they have complied with all of the requirements of the Policrete Concrete Grinding and Polishing conditions of contract, the relevant EBA and the specific requirements related to Site Safety. The Contractors representative is required to sign the form in acknowledgement that they understand the terms and conditions within the Site Safety Plan and that they will abide by them and the requirements of the Victorian Workplace Health and Safety Act. Completed by: PM/CA/SM File: OFFICE/SITE J.0.06 EMPLOYEE / CONTRACTOR SITE REGISTRATION FORM It is a specific requirement that all individual Policrete Concrete Grinding and Polishing employees, contractors and their employees complete this Site Registration Form at the time they undertake their site induction. lt is important that this vital information be kept on site during construction. The information contained herein may be critical, particularly if an incident or accident occurs. Completed by: SM File: SITE J.0.07 SITE INDUCTION CHECKLIST Every Policrete Concrete Grinding and Polishing employee, contractor and their employees must be inducted by the Policrete Concrete Grinding and Polishing representative. Each of the relevant topics in the checklist need to be addressed in sufficient detail so that the person being inducted has a clear understanding of the specific site safety requirements for the project. Every individual induction form must be completed and signed by the Policrete Concrete Grinding and Polishing representative.

Completed by: SM/Cont/Empl File: SITE J.0.08 SITE SAFETY RULES These are general Site Safety Rules which apply to every project managed by Policrete Concrete Grinding and Polishing. They are to be given to every Policrete Concrete Grinding and Polishing employee, contractor and their employees at the induction. The individual concerned must read and initial beside each item to confirm they have read and understood the site safety rules. Individuals with language difficulties will need to be assisted through this process. Completed by: SM/CONT File: SITE J.0.09 HAZARD REPORT FORM This form is to be completed by Policrete Concrete Grinding and Polishing employees, contractors and their employees when a workplace hazard is identified. The form must be received by the Policrete Concrete Grinding and Polishing representative and signed. The Policrete Concrete Grinding and Polishing representative will then activate and document the remedial action taken to ensure workplace health and safety is restored to an acceptable Standard. This register must be filed for a period of 5 years after the project is completed. Completed by: SM/OH&S Rep File: SITE J.O.12 RECORD OF TOOLBOX TALKS Toolbox talks should be conducted at regular intervals. A record of every meeting must be taken and include a list of all attendees, the item / subject discussed and what actions were agreed to be implemented, It is the responsibility of the Policrete Concrete Grinding and Polishing representative to record the information, distribute it as necessary and ensure that the information is filed as a complete record of the meeting.

It is important that the Policrete Concrete Grinding and Polishing site management team understand the requirements for dealing with and reporting accidents and incidents that may occur on site. In this regard it is important that they read and understand Sections J8, J9 and J10 of the Policrete Concrete Grinding and Polishing OH&S Management System. Completed by: SM File: OFFICE/SITE J.0.15 WORKCOVER NOTIFICATION This notification form must be completed as part of the process of advising the Workcover Authority when an incident or accident has occurred. The Policrete Concrete Grinding and Polishing Project Manager / Site Manager should immediately contact the Workcover Authority and report the Incident / accident by telephone after becoming aware of the incident. The Authority will normally advise when notification is required to be formalized. ln some cases they will require the Authorities standard notification form to be completed and submitted. Our requirement is that the Policrete Concrete Grinding and Polishing form J.0.15 should be completed and kept on file for every incident / accident where an injury is incurred. The contractor is required to submit a Safe Work Method Statement (SWMS) for their specific works and shall provide safe work procedures to demonstrate safe work practices are in place. These guidelines have been developed from the Policrete Concrete Grinding and Polishing Occupational Health and Safety Management System. All work shall be conducted in accordance with the Victorian Occupational Health and Safety Act 2004, regulations made under the Act, approved Codes of Practice, and appropriate Australian Standards. 1. RED CARD / CID INDUCTION All contractors and their employees must have completed the Construction Industry Training or Red Card OH&S course. All persons on the site must be in possession of their red card at all times while on site. In the event that the red card is not in a persons possession, they will be asked to leave the site until such time as they can produce evidence of completion of red card training or proof of enrolment in an approved course. 2. ACCIDENT / INCIDENT REPORT Contractors are responsible for the reporting of accidents and incidents in accordance with Victorian legislative requirements. Accidents and incidents shall be reported using a system complying with Australian Standard AS1885.1. Contractors will be required to report any accident or incident to Policrete Concrete Grinding and Polishing management and complete a company accident and incident report form on the same day as the accident / incident. Every accident or incident will be investigated in line with the Policrete Concrete Grinding and Polishing procedures and contractors will be required to undertake an investigation of their own employee accidents/incidents, Where the investigation indicates that corrective measures are required to reduce the risk of the incident occurring again, the contractor involved will be required to implement control measures and report this back to Policrete Concrete Grinding and Polishing. Significant incidents such as death or where a person is required to be treated at a doctor or hospital must be reported to Policrete Concrete Grinding and Polishing immediately on 13 000 547 843. 3. FIRSTAID Prior to the commencement of work on any site, contractors shall make provisions for the prompt medical attention in case of injury. The telephone numbers of the hospital, physician and emergency services shall be known by every contractor and displayed on site. The contractor shall ensure that a First Aid trained person is on site at all times in accordance with the Code of Practice for First Aid in the Workplace. It will be expected that contractors will have access to a mobile phone where a site phone is not accessible. Policrete Concrete Grinding and Polishing will maintain a first aid kit that will be located in the workplace and in a company vehicle that will be used to travel between work sites. 4. SMOKING No smoking zones are to be adhered to on or around the site. 5. ALCOHOL AND DRGUS Alcohol and non-prescription drugs are not permitted to be consumed on site. Persons found to be under the influence of substances mentioned above will be requested in the interest of health and safety to leave the site.

6. AMENITIES Amenities provided on project sites are expected to be kept in a clean and orderly condition with debris, waste materials and surplus equipment removed and cleared as work proceeds. This includes eating, cleaning and toilet areas. Housekeeping of these areas is important in reducing associated fire hazards. 7. HOUSEKEEPING Contractors shall maintain a high standard of housekeeping on the site with respect to the nature of work undertaken. It is the responsibility of the contractor to maintain their materials, tools and other equipment in an orderly manner on site and in designated storage areas. All debris and waste on site resulting from contractor activities shall be removed by the contractor responsible. It will be expected that every contractor will clean up all waste materials and tools prior to leaving the work site every day. lt is particularly important that equipment and materials are not stored in designated walkways. All waste generated by contractors on site must be disposed of in accordance with Victorian environmental legislative requirements. 9. PERSONAL PROTECTIVE EQUIPMENT (PPE) Personal protective equipment appropriate for the hazards associated with the work being conducted by the contractor shall be provided by the contractor. The personal protective equipment and clothing shall comply with relevant Australian Standards. All personal protective equipment required for the project shall be outlined in the contractor project risk assessment document, The contractor is responsible for ensuring that personal protective equipment and clothing provided to contract employees is used where required. Contractors shall be responsible for appropriate storage and maintenance of their own personal protective equipment. 10. PUBLIC SAFETY Visitors and members of the public are prohibited from accessing the work site unless accompanied by a responsible person from the contractor organization that is knowledgeable of the hazards and risks on site and aware of emergency procedures. Contractors shall ensure that the following aspects of public safety are considered: Prevention of unauthorized access to the site External loading and unloading arrangements Work performed on footpaths (where required) internal pedestrian access

Separation of work zones from completed operational areas and equipment.

11. SITE SECURITY Project sites shall be appropriately secured during working hours to ensure that unauthorized access is prevented. At the end of daily work the site shall be secured by means of physical barriers to prevent public access. Areas of restricted access shall be signposted accordingly. 12. ELECTRICAL SAFETY Consideration shall be given to the safety of project electrical equipment. Contractor electrical equipment shall be tested and tagged in accordance with Australian Standard AS3760 In service safety inspection and testing of electrical equipment. Electrical leads shall: Be connected to the nearest electrical point Not be over extended Be switched off at the point of power supply before removal Be removed when not in use Be supported clear of ground level by use of stands and not run across walkways Not be laying in or adjacent to water or electrical equipment

It will be expected that contractors will only operate power tools through an appropriate earth leakage device such as a residual current device. This may be within the main board or a portable unit. 13. FIRE PROTECTION Contractors shall conduct project work in a manner so as to prevent the proliferation of fires. This will involve the consideration of fire protection in the selection of tools, work methods and materials. Combustible materials will not be stored in large quantities within the work site to reduce the likelihood of fire occurring. 14. COMPRESSED AIR Use compressed air only for the jobs which it is intended. Always cut off all air and bleed lines to compressed air hoses, receivers etc. before disconnecting couplings. Check air hoses for leaks or bad connections before placing them under pressure. Where safety chains are provided they must be used. Always release air slowly into any system, ensure that control switches on air tools are in the off position. When the job is completed, turn off the main isolator and bleed the air from the system. The use of combustible oil, grease etc. must be avoided in the presence of high pressure air containers and hose connections. Blowing down of clothing or any part of the body with compressed air can cause serious injury.

15. EMERGENCY PROCEDURES Contractors shall, prior to the beginning of the project works familiarize themselves with emergency exits. They shall also ensure that access is available to a phone to notify emergency services if required. 20. WARNING SIGNS Warning signs are placed for your protection, Read them and take notice of their warning. The removal, shifting or destruction of any warning sign is forbidden without authorization from Site Management. Should any sign be removed or destroyed accidentally, notify Site Management immediately and ensure the hazard addressed by the sign is not left unprotected in the meantime. 21. CONCRETE CUTTING, CORE HOLE DRILLING AND EXCAVATING PERMIT A permit system for all concrete sawing, core hole drilling and excavation is in place on all Policrete Concrete Grinding and Polishing' construction and development sites. The permits are issued by the Site Manager/Foreman. The permit system focuses on services such as electrical, plumbing, stressing cables and general safety of falling debris and housekeeping. None of the above works can take place without first obtaining a permit from the Site Supervisor or Foreman. All conditions on the permit must be complied with at all times. 22. CRANE OR HOIST WORK Only certified persons or authorized persons in training under direct supervision from certified persons are permitted to carry out slinging operations and direct crane movements. Safety procedures shall be followed when climbing onto or down from a crane. Unsafe conditions must be reported to the Site Manager or Foreman immediately. Mobile crane work will not be permitted near powerlines. Slings or chains must not be carried on the jib of a mobile crane travelling without a load. Only one authorized person is permitted to relay signals to a mobile crane operator. Outriggers must be used when needed. When positioning a mobile crane to take a lift, it must be on firm level ground and be blocked at the outriggers. 23. UV PROTECTION Exposure to ultra-violet radiation is potentially harmful. When working outdoors the following precautions shall be taken: Factor 30+ (or greater) block out applied every four hours Sunglasses (be sure to use safety glasses where the task requires it) Wearing of a broad brim hat for head and neck protection ` Loose fitting clothing which provides adequate coverage over arms and legs

24. HOT WORK A hot work permit is required before any temporary work involving open flames or producing heat and/or sparks (oxy & acetylene work, welding, grinding etc.) is carried out. Hot work permits will be issued by the Site Manager. The following conditions must be adhered to before, during and after hot works: Employees must have been given instruction and training in the safe use of all equipment including firefighting equipment Sprinklers, hoses or extinguishers are in service, operable, and available Hot work equipment is in good repair and appropriate for the works to be undertaken. Ventilation is adequate All services have been located and made safe (electrical cables, conduit etc.) Area is cordoned off and designated with appropriate warning signs Spark/flash screens are used at all times for welding works lf the alarm needs to be disconnected, the Site Manager/Foreman is informed prior to disconnection

Requirements within 5 meters of the work zone, ensure: Flammable liquids, dust, lint and oily deposits are removed Explosive atmosphere in area eliminated. Floors swept clean. Combustible floors wetted down, covered with damp sand or fire resistant sheets. Remove other combustibles or otherwise protect with fire resistive tarpaulins or metalshields All wall and floor openings covered

Work on walls or ceilings, ensure; Construction is noncombustible and without combustible covering or insulation. Materials on other side of the wall are not combustible All services have been located and made safe (electric cables, conduit etc.)

Work on enclosed equipment, ensure: Enclosed equipment is cleaned of all combustibles Containers are purged of flammable liquids/vapor Confined space hazards are addressed (Refer to AS 2865 -1995; Safe working in confined space)

Oxygen and acetylene brazing, cutting, ensure: Gas cylinders must be kept upright when in use, secured to prevent falling, protected from extreme heat and from being struck by moving equipment and falling objects

lf transported by crane, hoist or derrick, gas cylinders must be handled in a suitable cradle, net or skip box, NEVER by wire or fiber rope, web or chain sling, or by dragging. Regulator must be removed. Care must be taken to ensure that oxygen cylinders and fittings do not come into contact with oil or grease Oxygen cylinders must never be stored near highly combustible material, especially oil or grease or near fuel gas cylinders

16. PLANTAND EQUIPMENT SAFETY Contractors shall comply with all aspects of the Victorian Occupational Health and Safety (Plant) Regulations when working with plant. Every item of plant owned and operated by contractors will be required to be inspected daily prior to work to ensure that it is operating in a safe condition. It will be expected that items of plant owned and operated by contractors will be inspected and maintained according to a regular schedule and that records of this maintenance can be provided if required by Policrete Concrete Grinding and Polishing. Every employee required to operate plant on a Policrete Concrete Grinding and Polishing managed site shall be appropriately trained by the contractor to work with the plant. Working with compressed air shall be undertaken in accordance with the Policrete Concrete Grinding and Polishing Occupational Health and Safety Plant and Equipment Safety Procedure. 17. MANUAL HANDLING Contractors shall have due regard for handling equipment and loads in a way that is safe. It will be expected that contractors will have assessed the risk involved with undertaking manual handling and will have implemented appropriate control measures. 18. WORKING AT HIEGHTS All work at heights shall be performed in accordance with the Policrete Concrete Grinding and Polishing Occupational Health and Safety Procedure Manual. This specifies the requirement for risk assessments to be conducted before work commences on jobs equal to/greater than 2m in height. When working at height appropriate fall protection equipment shall be utilized such as guardrails and safety harnesses if there is a possibility that any person may fall or where a risk assessment has indicated that this equipment is required. When working on ladders the following guidelines shall apply. Always extend and lock ladder braces into position before work commences on them Never over reach when working from a ladder Always maintain one hand on the ladder for stability

Never use ladders on uneven surfaces where a ladder may be unstable Never work off the top rung of a ladder. Where work is to be conducted on a roof, the structural stability of the roof must be tested prior to loads being placed on them or any person walking on them. Fall protection equipment must be provided when work is to be conducted within 2 meters of the roof edge (safety harness, handrails). lt is also a requirement that appropriate footing is available for walking on the roof.

19. CHEMICAL SAFETY Contractors who are required to bring chemicals onto any Policrete Concrete Grinding and Polishing managed site must provide Policrete Concrete Grinding and Polishing with a copy of the material safety data sheet for the chemical. Personal protective equipment for using the chemical shall also be used where it has been determined that there is a need to where such items as gloves etc. A designated storage area will be provided at every Policrete Concrete Grinding and Polishing managed site to ensure that all chemicals are stored in a central area. Chemicals that cannot be stored together because of their physical properties must not be stored together. All containers must be clearly marked with their contents.
