1. Persons shall provide eye, hand and face protection equipment when machines or operations present potential injury from physical, etc. 2. All person working or visiting shall wear protective headgear

3.1. Tools, material, extension cords or debris shall not cause tripping or other hazards 3.2. Form and lumber shall be cleared from work areas 3.3. Work area shall be kept free of materials, supplies, and obstructions at all times. 3.4. Open rebar edges will be covered with wooden cap

4. Protruding metals in scrapboards, planks and other materials shall be remove and place trucks for removal

5. The wires will be cleaned or firmed and grounded. Wires and electrical tools to be checked before use

6. Worker will be working in the tank and portable lights will be used in the tanks

7. Safficient lighting will be provided for applying waterproof material in to the gray water tank.

8.Temporary lights shall be equipped with heavy duty electric cords with connection and insulation maintained in safe condition. Temporary lights shall be suspended by their electric cords and lights are designed for this means of suspension.

9. Confined space procedure will be followed EM 385-1-1 06.I

9.1.Permit required and do not enter etc. sign had been put around entrance on the tank.

9.2.Barricading had been put around entrance to secure employees from falling in to the tank.

9.3.Barriers had been put to prevent uncontrolled access to the tank. 9.3.a. Confined Space Awareness and Entry training will be giving all employees, authorized entrants and attendants. 9.3.b.Entrants will not enter confined space until acceptable entry conditions are verified. 9.3.c.Entry supervisor will sign the permit only after all precautions have been implemented and the atmospheric readings have been taken and found to be acceptable.
9.3.d. Entrants will sign entry documents before entering to the tank and leaving from tank.

9.3.e. Entry form will completed before entering by all entrants and team members to acknowledge their understanding of hazards involved with the confined space.

9.4.Atmospheric test will be done before employees will enter to the tank. 9.4.a.An Atmoshheric Oxygen concentration should be between 19.5 % - 23.5%. 19.5 % is the minimum acceptable oxygen level for work without an air supplied respirator. If Oxygen level is below than 19.5% air respirators will be used. 14.4.b.It will be ensured that interior confined space combustible gases and toxics are not present before entering. If there is any toxic gases it will be ensured these gases are below OSHA permissible exposure limit. If there is Flammability should be less than 10% LEL. If there is Carbon Monnoxide (CO) should be less than 25 ppm. If there is Hydrogen Sulfide should be less than 10ppm.

9.5.a.Two way voice (within sight) communication will be used with entrants. 9.5.b.Entrants will be observed 9.5.c. Monitoring will be used to observe and communicate with workers inside the space at entrance. 9.5.d.Entry record sheets will be used and kept to entry and exit time.

9.6. It will be ensured there will not be any kind of engulfing (i.e. grain, sand,) Employees will be working in the concrete made gray water tank.There will be no any kind of engulfing hazards.

9.7. Emergency procedures will be reviewed including rescue and evacuation before entering to confined space. 9.7.a.Emergency Rescue team will help entrants in emergency 9.7.b.In emergency will be contacted with hospital.
