SOURCES OF INFORMATION AND CIRCUMSTANCES OF THE REFERRAL

The family was admitted on April 12, 2011. The couples approached the center to place their children for long term placement due to financial instability, their environment situation was not safe because of riot and childrens might or could be hit by a vehicles. On the year 2009, the family becomes street dweller. Maam Mila, Agency Supervisor at Hospicio de San Jose, Pasay City saw them everyday at the front of National Teacher College at Quiapo. She referred them at Hospicio De San Jose, Ayala Bridge, Manila to seek help. The couple approaches the center to seek assistance to help their children continue their schooling and be place in temporary shelter and be provided their basic needs until they reached the age to live independently. III. BACKGROUND INFORMATION A. FAMILY BACKGROUND Hannibal was a native of Kalibu Aklan the eldest child among the seven (7) children of Mr. Aristotle Motos and Mrs. Lolita Idao. He was an illegitimate child. He has a curly hair and brown complexion. He decided to stay in Manila because all he thought is, he had good life and good work so that he help his family. He works as a vendor (union, garlic, tomato, fruits and etc.). He earned amounting of 100-150 per day irregularly. In his work he met Remedios the youngest child among the four children of Mr. Enrique Padua and her mother Mrs. Nenita Clarite from Ilaya Villa Padua, Gumaca, Quezon. She has a curly hair also, thin and have fair complexion. She works as nanny for almost one year. On the year 1999, she lived with Hannibal at the age of 31 while Hannibal was 40 years old. Hannibal and Remedios are not legally married. They rented a room amounting of 500/ month at Upper Bikutan, Taguig Metro Manila. Remedios and Hannibal were blessed of four (4) children namely: Harold Lloyd, Cherre Mae, Don King and Jhon Mark. Remedios give birth to her first child at the age of 32 at Upper Bikutan, Taguig Metro Manila. When she is 37 years old, she gives birth to Cherre Mae at Rizal Medical Center, Pasig City. After 1 year, she gives birth to Don King at Angono, Rizal and Remedios already 41 years old when she gives birth to Jhon Mark at West Rembu, Makati. The family lived in this place for almost two years from year 2007-2009. On the year 2008, Remedios sickness started. She stated that since they use catol for killing mosquito at night time but slowly she was hard to breath and suffered from cough. She was admitted at Jose Reyes Hospital and was diagnosed for Tuberculosis by Dr. Martian. The couples were able to ask financial assistance from Malacanang Action Center. They received financial assistance amounting of 1,500 pesos to her medication for six months. The Dr. advises her that the things will she used must separate but it was not followed because of their condition in life. On May 7, 2009 there was a demolition at West Rembu, Makati. The family affect to the said demolition because it is not their own property. Hannibal brought his family in Rizals Park because he saw that there are many peoples who lived here. They started to live on street, in Bay walk, PICC and Lawton Fly over.

When Ondoy came on Manila on the month of December year 2011, this place will affect so much. The family stayed in Lawton fly over. Hannibal stated that, just like it is now the end of their live but they survive. When the flood stops, his family started to beg to earn money to buy for food. On 19th day of March 2011, Don King suffered for fracture on his left leg because when he ask permission from his father that he only watch television at Tanduay Legarda St., Quiapo but he went to Casa del Carmen to play. When Don King stepped on the seesaw to reach the barracks but then a little child come and accidentally moved the seesaw hes been out balance and fell down. His left leg hang on the seesaw spring. He was admitted at Philippine Orthopedic Center at Maria Clara Corner Banawe Street, Quezon City. Dr. Cornelius advised to cast immobilization for 4-6 months and regular OPD follow up. On April 20, 2011, Don King undergo for X-Ray examination so that they know the condition of his left leg. The result was, his leg was already healed and need to drink milk three times but it was not done because the couple has no enough money to sustain it. The capital of Hannibal worth 2000 pesos for his work was been used for the medication of Don King. Hannibal stated that when it was happened in their family, they only eat one meal for one day. Hannibal stated that even they encountered a problem they still have strong relationship and happy family despite their condition or situation. He and his wife seek first the condition of their children because if they have food, children first then if there are left over the couple shared it. In terms of rearing and disciplining their children they were teach them a right manner. Seldom had they beaten their children for them to learn into their mistakes and for them to give lesson on it but most of the time they were given advices not to commit the mistakes for their own good. Hannibal is always having conversation with people because of his work. He is not ashamed of it because through this he supports the daily needs of his family. He is the one who talked to the people when they need assistance. Remedios also is not ashamed to be with people because she works as cook at Holy Face of Jesus Church. She earns 50 pesos every Sunday and Saturday. When they lived in street, the couple avoids their children to be with other children in the street because they dont wanted that they adapt and influence their behavior. The family was Roman Catholic, they went to church every Sunday. As being religious family they gather strength from Almighty God, for they believes that whatever the situation and conditions confronted God is always help them.

B. THE CLIENT Harold Lloyd is 12 years old, the eldest child among his three (3) siblings. He was born at Ilaya Villa Padua, Gumaca, Quezon on the 20th day of February 1999. His weight is 32 kg. He has fair complexion with a medium body built. His family called him Big Boy because he is a big baby when her mother gives birth to him. In the center, he is observed to be respectful because he show respect to the older person and dont argue with them as stated by Mrs. Josefina Litaban, responsible and obedient child because he do his tasks given to him at the center. He easily follows instruction because one or two words are enough to him and learns faster than the other children in the center because

during our tutorial he has a higher score and when he is in grade five, he is the top one among the twenty pupils. He has an average of 82%. He is aware of their situation because in his age, he noticed the hardships of their life. He is matured child because his father always advised him and explained the situation to understand it. He uses his time for reading books and his ambition in life is to become a lawyer so that he helps or fight the peoples right especially the poor one. He loves to play with his co-children and his favorite sport is basketball. He participated and cooperates to the activities of their school. He has a friend when he is in Onesimo, Bulilit Foundation. Namely: Edmund, Edmundo, Armando, Jerome, Lester and John-John. Harold knows how to pray the rosary the English version and Tagalog version. Not only the rosary but when he called to led a prayer during our tutorial, he prayed what is in his heart because he mentions that God helps us to understand our lesson and blessed our teacher and gives them more patients. Cherrie Mae is the second child and only daughter of Mr. Hannibal Idao and Mrs. Remedios Padua. He was born on March 22, 2003 at Rizal Medical Center, Pasig City. Her weight is 17.5 kg, she has curly hair and has fair complexion. Her family called her Mae-Mae because of the second word of his name. She is observed to be quiet bully because sometimes she teases the little children in the center until they cried. She is fast learner because she already knows how to write, read and add (basic) and can easily follow instruction as stated by Irene Ramos, her teacher in tutorial. Her parents scolded her if she commits mistakes with his co-children. When she step on college, she want to get a Doctor because she stated that she wants to take care and helps the sick person. He loves to play and watching television with her co-children in the center. His favorite song is the song in the movie of Mutya because she always sings it together with the children in the center. She has her own rosary because she is a Catholic. She uses it when they have prayer in Tahan Center during 11:30 in the morning and inside the institution during 4:00 in the afternoon. Cherrie Mae was diagnosed of Conjunctivitis both her two eye on April 13, 2011 by Dr. CecilleJean Villareal. Conjunctivitis is the inflammation of the conjunctiva, a mucous membrane that lines the inner surface of the eyelids and joins with the cornea of the eyeball. Conjunctivitis may be caused by infection, allergy, or injury and is marked by redness and swelling, accompanied by a feeling of roughness as the membrane passes over the exposed surface of the eyeball. The eye may be sensitive to light; in more severe cases a mucoid sticky fluid, or even pus, may be discharged, depending on the cause of the infection. Most cases of bacterial conjunctivitis are treated successfully with antibiotics and sulfonamides. Viral conjunctivitis usually lasts about two weeks; treatment may be necessary in cases that involve complications. Silver nitrate or antimicrobial ointments are applied to the eyes of newborn babies to prevent ophthalmia neonatorum.

Don King fondly called King because of the second word of her name. He is 7 years old and was born on April 9, 2004 at Angono, Rizal. His weight is 17 kg, has fair complexion and no teeth at the front portion. Among his siblings, he is the very bully because he always teases both the little and big children in the center that leads into conflict or argument. Maam Analyn the houseparent in the center stated that Don King and the one child in the center watching television. The child covered the T.V. until Don King got tempered. They tried to punch each other but the mother of the one child saw it, instead she away them for this case. She stands her son until both parents have misunderstanding. When he teases the children some are cried and they complained him to their houseparent. He is mischievous and observed to have a hard head because when his parents scolded him, he obeys for a while but he will do it again. King is brave because he is not ashamed to be with people. He has a friend to name: Yoyong, the one child in the center. Jhon Mark is the youngest son. He was born at West Rembu, Makati on the 1st day of August 2007. He is 4 years old and his weight is 14. 5kg. He has fair complexion, no teeth on the upper portion and has a good eyelashes. He observed to be a quite child but he is also a bully because if he is in mood he teases his little co-children. He is easy to get mad and moody especially if they dont give what he wants. His father stated that they observed him as a self-centered because if they have food and when his siblings have toys or school supplies he want also that he have. When they dont give it, he cried. He is observed to be a sensitive because when they scolded him, he left and cried. In the center, he participated in coloring book, he draws and writes but in a short time because he is easy irritated. There is a time that he want to be alone and preferred to played in there room. John Mark was seen and examined with a diagnosis of Upper Respiratory Tract Infection on April 13, 2011 by Dr. Villareal. Acute infectious disease of the upper respiratory tract is caused by more than 200 kinds of viruses. Children probably have so many because they spend time in close contact in schools and daycare centers, making it easy for the virus to spread. Infection with a cold virus affects the mucous membranes of the nose and throat, causing such symptoms as nasal congestion and discharge, sore throat, and coughing. These symptoms are typical also of respiratory infections caused by bacteria, and of allergic conditions such as hay fever and asthma; therefore, the common cold is difficult to diagnose with certainty. Normally a cold runs a mild course, without fever, and goes away on its own in about seven days. If the symptoms do not go away after two weeks, they may result from an allergy. The medical significance of a cold lies in the possible complications that may follow. Various diseases, such as bronchitis, pneumonia, and sinus or middle-ear infections, may arise from the cold. A cold accompanied by high fever, by a cough that brings up mucus, or by severe sinus pain may call for attention from a physician.

The great majority of colds are caused by viruses, and the rhinoviruses cause about half of all colds. Research indicates that there are several strains of each type of cold virus with varying degrees of virulence. Infection with one strain of cold virus provides only a brief immunity to reinfection by the same strain, and gives no immunity against the other viral strains. The common cold was once thought to result from exposure to cold weather, a belief probably traceable to the fact that people tend to crowd together indoors at this time, causing a more efficient transmission of virus from person to person. Chilling was also believed to cause a cold, but there is no scientific evidence for the belief that chills or exposure to cold play a role in causing colds. Research does suggest, however, that stress may weaken the bodys immune response and thereby make it easier to become infected. Cold viruses are transmitted from person to person through droplets of mucus in the air. The droplets containing cold viruses enter the air when an infected person sneezes. The viruses can also be spread by handfor example, by touching the hand of an infected person, or by touching a telephone, piece of paper, or other contaminated surface, and then touching the eyes or nose. No cure is known for the common cold and no preventive drug has yet been found. Various antihistamine drugs were once claimed to be effective in the prevention of colds if taken at the onset, but that view has been discredited. American chemist and Nobel laureate Linus Pauling proposed that large doses of vitamin C can prevent a cold or alleviate its symptoms, but subsequent studies have failed to substantiate his claim. There is some evidence, however, that vitamin C can shorten the duration of a cold. Physicians also advise people with colds to drink plenty of fluids. Bed rest may be recommended to avoid complications, even when the cold itself is not incapacitating. Antibiotics, which fight bacterial infections, should not be administered because nearly all colds are caused by viruses rather than by bacteria. Antibiotics may be prescribed if sinus or middle-ear infections develop; however, they are ineffective as a preventive measure.

IV.

DIAGNOSTIC ASSESSMENT

Hannibal is 51 years old and Remedios is 44 years old. Hannibal works as vendor whose income is 100-150 per day irregularly. Remedios was works as prex and earned 50 pesos every Saturday and Sunday but due of her sickness, she is jobless. The couple could hardly provide the needs of their four children because their income is not sufficient to continue the schooling of their children. The family becomes street dwellers for almost two (2) years. As Hannibal expressed, they could barely eat three times a day especially when he used his capital for the medication of their third child. The couple brought their four children to Hospicio de San Jose on April 12, 2011. During intake interview the former narrates the hardships they encountered. They approach the center to place

their children for long term placement due to the financial instability and seek assistance to help their children continue their schooling until they provided their basic needs and live independently.

VI.

EVALUATION AND RECOMMENDATION

Based on the above facts gathered it is evaluated that the client seeks assistance to help their children continue their schooling and be place in long term placement and be provided with their basic needs until they reached the age to live independently at the Hospicio de San Jose and refer their children for further management and intervention of the case through the basic programs and services of the office. In view of the foregoing facts, the undersigned recommends to place the four children in a long term placement and to assist for their schooling so that their parents can work and continue what they had before so that when the children go back to their parents they have new house and to meet the needs of the children.
