He shall coordinate with the Company at the Site as Contractor Site Representative. He shall develop and implement a site execution plan necessary for the successful completion of construction works He shall monitor the allocation of resources to the construction and approve the assignment of Key Personnel. He is also directly responsible for developing and arranging the construction team to the Site necessary to manage and supervise the construction works and to manage the Subcontractors. requirements. (10) He will supervise the Contractors Quality Team, Implements inspections, tests and examinations in accordance with Project Quality Plan. He shall ensure a comprehensive HSSE induction and training program is developed for all personnel assigned to Contractor work sites(s) prior to any mobilizations. He shall review and audit the works of all disciplines to ensure that HSSE requirements are addressed.

(10) He shall monitor work performance, empower work supervisors leaders and staff, and suspend work when sub-standard safety or environmental performance occurs until remedial actions are agreed, resourced, communicated, implemented and vigilantly monitored. (11) He will be responsible for the monitoring and reporting of all client required HSSE metrics, including all regulatory compliance requirements as directed in Exhibit D. (12) He shall specify the key job responsibilities relating to HSSE for each member of the project management team 5.1.4 Project Control Manager (Site) (1) The Project Control Manager shall provide the Site Manager with the tools necessary to exercise effective project management and control of their respective functions. He and his project control planners & engineers shall provide the information necessary for the Project Team to control and manage the resources necessary to meet the project program including the development and reporting of schedules, budgets, contract status, invoicing status and the various elements. However, the Project Control Managers role is not only the reporting on time (schedule), resources (manpower) and budget (costs), but also the supporting of the Site Management by providing comprehensive analysis of where the project is heading in terms of schedule (planning) and costs (trend reporting). This shall include day-to-day status reporting on all phases of the Construction execution such as construction, Pre-commissioning and Commissioning assistances. He shall be responsible for preparing the project schedule and progress measuring system with support of Construction Superintendant. He shall develop progress reporting procedures to allow progress tracking and productivity assessment.

Commissioning /Start up Manager (1) The Commissioning/ Start up Manager shall be responsible for Pre-commissioning & Commissioning and Initial Operation of the facilities of the work to achieve the Final Acceptance Certificate. He shall prepare the Pre-commissioning and Commissioning plan and procedures which shall be approved by the Company. He shall plan, organize and coordinate the required flow of information between all

parties involved in Pre-commissioning, Commissioning and operation. (4) (5) (6) He shall maintain close contact with the Company to safeguard all operations for Pre-commissioning, Commissioning and operation. He shall recruit and direct the team and arrange further required personnel inclusive of training on the job and maintaining required discipline. He shall analyze problems, resolve issues after due consideration of process safety, check alternatives and arrange measures to eliminate unforeseen difficulties. He shall advise the engineering team of the necessary input from an operational point of view during the design stage and during the HAZOP studies so that the required input and precautions for Pre-commissioning, commissioning and operation can be implanted.

Administration Manager (1) (2) The administration manager is responsible for all kinds of activities in relation to the construction administration. Under direction of Construction Manager, he shall assist with field Engineer and Construction superintendent in performing field accommodation, site transportation activities, and general services. He shall Plan & Direct office and camp administration. He shall be responsible for all personnel matters. He shall provide immigration services for field assigned employees. Be responsible for payroll functions. He shall arrange for local recruitment and personnel administrations. Be responsible for enhancing companys image toward locally hired employees. He shall collect and analyze information on local rules, regulations, and guidelines.

Responsible for the planning of resources required to complete the project and ensures that site personnel are adequately experienced and properly trained to carryout designated duties and responsibilities. Responsible for day to day management of the project, both on and off site, through delegation of duties in accordance with the Organization Chart and for the effective implementation of the Project Management System and agreed Contract Quality Plan. Responsible for liaison with head office for the purpose of resource procurement and support services. Performs contract reviews, as required, to understand and incorporate the clients requirements, monitor progress to achieve target dates and assess whether there is a need for additional resources.

6.
6.1 6.1.1

SAFETY
Lifting General: Mobile cranes which subcontractor has are used in this project for erection, construction, alteration and maintenance. They are also used to load and off-load general cargo on to vehicles and any other means of transport. Therefore, considering the high frequency of their use, the risks are higher; incidents involving lifting by crane may harm people and lead to loss of assets and reputation. It is, therefore, vital that people operating the crane and their assistance, people working in the vicinity of the crane, people working on the loads themselves are qualified, are alert and constantly vigilant to the inhering hazards.

Operator: The Site Supervisor shall ensure that only trained, qualified and specifically designated persons shall operate a mobile crane. They must have a valid license issued by ROP and they must have had intensive training to familiarize themselves with their machines, the safe load, interpretation of the crane chart, the colour code and the risks involved when operating in a workplace. The operator shall not start a crane without going round his machine to see if any persons, animals or objects are in the vicinity and may be affected by the operations. He would have established a clear communication signage between him and his Banksman on the ground. The operator must attend refresher courses at the designated times. The crane operator shall not engage in any practice that will divert his attention while operating the equipment. The operator shall respond to signals only from a trained signal person. The operator shall ensure that the crane is secured at all times whether idle or working. A critical time is when the operator is moving from one cabin to the other.

6.1.3

Empowerment: When in doubt as to the safety of rigging and hoisting activity, the operator has the authority and responsibility to stop to handle loads. In actual fact the operator is the direct person responsible for his own and other peoples safety, the integrity of his machine, the goods and the assets.

6.1.4

The Crane: The PMV Engineer shall ensure that the crane complies with the specifications laid down in this plan. It should be regularly inspected and load tested, with special attention given to locking devices, brakes and hand brakes. The safe load should be indicated on the jib of the crane. Cables, slings, shackles, pullies shall all be genuine parts quality controlled and tested, and copies of certificates shall be present at site at all times. The crane chart should be available in the cabin at all times. Registration should be valid at all times when crane is in operations.

6.1.5

Operations: (1) The Site Supervisor shall insure that barricades, warning signs or other methods shall be used to prevent entry into a lift area or turn radius of the crane.

At no time shall persons work under a suspended load, and suspended loads shall never be left unattained without proper safe guards. The Crane Operator shall always have an assistant on the ground that is a trained signal person. For critical lifts a critical lift plan shall be prepared by the Site Supervisor in consultation with the crane operator. Objects that may roll, such as pipes, shall be loaded and off-loaded on special pipe support with stoppers to avoid the pipe fall and hurt people. The operator shall not move the crane when loads are suspended to it. No persons other than the operator are allowed to get on the crane when in motion. Mechanics are not allowed to operate or move the crane by themselves. The operator should always be present with them when they want to attend maintenance or repair on the machine.

6.2 6.2.1

Transportation of People, Material and Assets Drivers All drivers (Heavy or Light) shall undergo specific related defensive driving at an approved local HSE Institute prior to operating any vehicle. The drivers shall also possess a ROP approved driving license. Prior to each journey > 20KM, a journey plan shall be filled by a certified Journey Manager and a vehicle spot check shall be done by the driver to ensure the minimum Health and safety requirements to perform the journey. While commuting or on a journey, the drivers shall never deviate away from the Governmental Black Tops roads and never take short cuts or attend to personal matters unless prior consent has been given and Journey Manager informed. Drivers shall always abide by the approved company speed limits (120 Black top & 80 Graded road) and will be subjected to penalty in case found negligent.

Load Material (1) Personal Load: All personal load shall be loaded and placed in a designated loading space (Light vehicles may include rear wire-mesh grid separating luggage from the rest of the individuals seated in the seating cabin as for busses they will include a specific external cabin where ones luggage can be stored) and shall never be placed in between or beside the passenger and specially never in walk ways such as Busses. Heavy, Assets and Building Material Load: All transported material shall be loaded and secured in the safest way possible ensuring the safety of the Driver, passing pedestrians, vehicle and cargo. The loading of material shall be done by a

professional rigging and slinging individual (Store In-charge) and the HSE Manager on site to perform a final inspection and ensure the safety of the load prior to the trip. 6.2.4 Vehicles All vehicles must comply with the following: (1) Light Vehicles 1) Comply with the laws and standards of the Sultanate of Oman. 2) Be suitable for operation in ambient temperature of up to 55C 3) No modification may be made to any vehicle without endorsement from the vehicle manufacturer or the local agent. 4) Must not be fitted with a bull-bar even if provided by manufacturer. 5) Sun roofs are not allowed. 6) 1 spare tire as a minimum requirement. 7) Seatbelts for all passengers. 8) Rollover bars. 9) Luggage and cargo space (Wire-mesh grid). 10) Air conditioner and Radio. 11) Anti lock braking system & air bags. 12) Emergency equipment (Powder fire extinguisher & 1st aid kit) 13) Speed Limiters. 14) Mirrors (2) Heavy Goods Vehicles 1) Comply with the laws and standards of the Sultanate of Oman. 2) Be suitable for operation in ambient temperature of up to 55C 3) No modification may be made to any vehicle without endorsement from the vehicle manufacturer or the local agent. 4) 1 spare tire as a minimum requirement. 5) Seatbelts for all passengers. 6) Air condition and radio. 7) Should be fitted with a dual dual-circuit, dual-line service brake system with a secondary braking system. Daily break efficiency testing will be carried out by the operator using marker poles and the results shall be recorded on the daily checklist. No other person shall be allowed to operate the machines, not even the mechanical team who attend repairs or maintenance of the machine. Operators shall never start a machine without going around it first to see if any person, object, animal are not in the vicinity.

1.1.3

Banksman: The Site Supervisor shall ensure that sufficient banksman are displayed to assist the operators and warm them about blind spots ,approaching persons, machines or obstacles. The banksman shall always wear highly visible jackets, coveralls and shall keep himself far enough to be in a safe environment from the machine in operations. He should always be in eye contact with the operator.

1.1.4

PPE: Earthmoving equipment generate dust, smoke, noise and sometimes heavy vibrations. It is essential that all the crew protect their eyes, mouth, face, and ears from dust by using mainly goggles, dust masks and ear fenders. Those operators who run vibrating machines (such as rollers) should have frequent rests. Operators should wear their seat belts all the times when the machine is running. AC units shall be maintained as per the manufacturers recommendations.

1.1.5

Improper use of machines: These earthmoving machines are not designed to transport people from a place to another (grader, dozer, roller) or lift them up (Front end Loader) and no person other than the operator should access the machine when in motion for whatever reason. Each machine should be used only for the task it is designed for.

1.1.6

Night Operations: In case operations have to be carried out at night, special authorization should be granted by the PM and the following rules and steps shall be followed to ensure safe operations, minimize risks and prevent accidents from occurring as low as practically possible: (1) Prior to commencing the night works, an individual head count shall be performed and recorded by the HSE Advisor on site. Each individual shall sign after the head count and once they get back to camp ensuring that all individuals are returned back safely and prevent any man lost accidents. Tower lights shall be placed providing enough lighting around each equipment being operated on site.

Individuals performing the night works should rest for at least one hour prior to their commencing of their night works. Tool Box Talks should be given again by the supervisor to the team operating at night and recorded. Male Nurse should be informed of the activities that are taking place and to stay on guard in case any accident occurs. Individuals shall never commute on foot between their machines and the camp and a driver shall be designated for insuring transportation at all times. The HSE Advisor shall perform a site visit to ensure operations are carried out safely and as per this Procedure. One may have to lower your sails and start you engine - beware of loose sheets fouling the propeller. 7) If possible note your position - most navials have a MOB function - it may prove vital if contact is lost with the person in the water. REMEMBER the MOB function records where the person fell overboard - he/she will drift away with the tide. 8) If the individual is the only person remaining on board, he/she should not leave the deck as he/she may become disorientated and loose sight of the person in the water. 9) During the hours of darkness, a white parachute flare, which will pick up the retro reflective tape on clothing/lifejacket, can be used to illuminate area. 10) If you cannot see the person in the water, or have any doubt about your ability to recover him/her, send a mayday call on your VHF radio. 11) If all individuals are well prepared and have practiced the drill regularly, they will automatically know how to react.

Steps to be taken by the individual related to Man Overboard 1) Individuals should look for the lifebuoy which may be close by remaining calm, keep their legs close together and restrict their legs close together and restrict their movements to stop flushing cold water under their clothing. 2) Whatever the individuals situation one should conserve their body heat, the greatest threat to ones survival is from the cold. Remember in cold water during the winter ones ability to assist in his/her rescue will be greatly diminished after 10 minutes. 3) In rough conditions, individuals should turn their back to the waves to keep their mouth and noise clear of spray. 4) Tighten up wrist, ankle and neck fastenings of protective clothing to reduce heat loss and the onset of hypothermia. Do not attempt to swim back to the boat for the same reasons.

Recovery 1) Getting the person aboard can be difficult. If there is a bathing platform or boarding ladder and the person in the water is able to help themselves, use it if it is safe to do so. If they are unconscious or exhausted, a form of lifting gear will need to be improvised. 2) A short strap used in conjunction with a block and tackle rigged on the end of a halyard (sailing vessel) or attached to a suitable strong securing point on the wheel house (powerboat) would make it easier for a heavy casualty to be brought on board. 3) A par-buckle can be improvised by using ropes, nets or a small sail, and then rolling the person out of the water. 4) A dinghy provides another option for recovery, perhaps by partially deflating one section of the Sponson tube to make it easier to get them on board. (7) Prevention is better than cure. Ensure that all the actions and safety precautions to prevent a person overboard have been taken and practice drills regularly in all weathers and sea conditions - You could save someone's life.

Laboratory (1) (2) The laboratory supervisor has responsibility for implementation of the Safety Plan in his laboratory. The laboratory supervisor shall ensure that information and training are provided at the time of an employees initial assignment to a hazardous work area. Refresher information and training shall be conducted. Employees must obtain prior approval to proceed with a laboratory task from the laboratory supervisor. Revetment Construction Reclamation 3.1.2 Rockfill (0-400mm) shall be carted using tipper trucks and dumped at a safe distance away from the water line. The material shall be spread into the sea using bulldozers. All rockfill in the water is deemed to be auto-compacted. Rockfill layers above the water level shall be compacted as per the earthworks method statement. Reclamation shall continue until the levels and slopes on the drawings are met. Surveying shall be done using conventional survey equipment with the aid of experienced divers.
