HEALTH ORGANISATION

The most important idea behind the success of any project is best
organization. So to fight against diseases proper system of health
organization is required.

Health organization is a system of methodical approach to carry out desired


functions constructed after thoroughly evaluations of the needs, resources &
capacity.

Aims of these organization is to meet the health needs of entire population &
cover full range of preventive & rehabilitation services.

AT INTERNATIONAL LEVEL:
 WHO (World Health organization): best &
highest level of organization for health. It
works for all countries who are member of
it/not. It does specific research on major
diseases of world, Collect data; make guide
lines for diagnosis, treatment, prevention &
cure of all diseases. It has now provided
many literatures for hypertension also for
public education.

 American heart association: It is institution based on America but


provides education on many cardiovascular diseases (including
hypertension) through out the world by means of internet, magazines
etc.

 Blood pressure association: It is institution based on London & conducts


programmes with the names of “know your numbers” & organized camps
for measurement for blood pressure measurements & screen population
of U.K. but it also provide guide line to their online members with the
help of internet.

AT INDIA LEVEL:
In India due to lack of resources & other major health problems than
hypertension, there is no specific attention or institution working specially
on hypertension.
So from diagnosis of case to education for prevention & control is provided
by only regular health care system which is only way now for fighting
against hypertension.

In India there are three levels of health care:

1. At primary level: Patient has 1st contact with health care provider.
Here care is close to patient & many health problems are resolved here.
It is provided by PHC (primary health centre), health workers, village
health guides, trained dais or private practitioners (in cities).
 Health workers, village health guides can provide first aids &
treatment of simple diseases. In addition they can provide
education for specific diseases of course one of them is
hypertension.
 At private practitioners only diagnosis & treatment of
hypertension is possible. Any complication relate with
hypertension is not treated by him.
 Dais are female who are trained to deliver baby they are not
much useful for hypertension management because their low
educational level & old ages.
 PHCs are most helpful set ups for management of hypertension at
village level. They educate people & advice and supply proper food
for prevention of hypertension. They also collect statistics
showing true picture of hypertension of villages. If they can de
provided, they can perform some basic investigation and correctly
diagnose & treat hypertension and other condition. It is main
training centre for health workers.
2. At secondary care level: It is the next higher level of care & at this
level more complex problems are dealt with. It is provided by CHC
(Community health centre), District hospital
 CHC covers 80,000-1,20,000 population with 30 bed facilities. It
act as referral centre for PHC & higher facilities are provided here
so complicated cases/severe cases are dealt here.
 In district hospitals some more investigating facilities are there &
severely affected people & atypical cases for better evaluation are
sent here. It is best place to collect data pertaining to related
district.
3. At tertiary care level: It is a specialized level of health care & requires
specific facilities & attention of highly specialized health workers. It is
provided by medical collage hospitals, regional hospitals, specialized
hospitals, All India Institutes etc. They are autonomous structure so
they have own resources so with major health problems they can
concentrate other diseases also like hypertension. They can perform
epidemiological research in specific directions also.

In Vadodara District:
 S.S.G. hospital is working as secondary level.
 In Pipariya village there is Dhiraj General Hospital Which is associated
with medical collage so it can provide better facilities in villages with
the help of mobile hospital, ambulance, education to villagers by
students.
 Baroda heart institute & Banker’s heart institute are specific
organization working on cardiovascular diseases & its complications.

Our patient is taking treatment in Dhiraj General Hospital because he is


working at there.

RECOMMENDATION FOR BETTER PERFORMANCE:

AT PHC LEVEL:-
 Better infrastructure for transport, communication, is needed.
 Lab facilities required little more advancement.
 Proper selection of staff & better salary to them can improve quality of
service.
 Organization of blood pressure checking camp periodically can beneficial
to people for screening. Or health worker can be trained to measure
blood pressure of village people periodically.

AT URBAN LEVEL:-
 In urban area there is more prevalence so dietary education & promotion
to exercise is best tool.
 Education to people by their camp in their working institute can make
awareness.
 Organization of blood pressure checking camp is also required.
