OBJECTIVE

INSTITUTION: QUEZON MEDICAL CENTER (QMC) SHIFT: 2-10 p.m. AREA: Pediatric Ward ENTRY COMPETENCY: Basic knowledge in Anatomy and Physiology, communication skills, basic nursing procedures, PHC I and II, NCM 100 and 101. TERMINAL COMPETENCY: Utilize and apply technically; psychologically the proper techniques and basic nursing procedures in rendering nursing care for pediatric clients. GENERAL OBJECTIVE: At the end of the clinical experience, the team members will be able to develop knowledge, skills, and attitude in the care of pediatric clients. CORE COMPETENCIES SPECIFIC OBJECTIVE Given the opportunity in the area, the team members will be able to: a. Attend the orientation and acquaint with the ward personnel, policies, physical setup, patients, and necessary routine Conduct orientation regarding: personnel policies and regulations physical set-up Tour the team members in the area Attend area orientation Attend the ward tour acquaint with personnel, patients, physical set-up TEAM LEADERS ACTIVITIES TEAM MEMBERS ACTIVITIES

activities and equipment in the ward. b. Demonstrate professional behavior in the clinical area

SAFE AND QUALITY NURSING CARE

a. Use nursing assessment skills to determine the health status of assigned patient through the use of inspection palpation percussion auscultation b. Formulate individualized list of nursing diagnoses in the development of nursing care plans

Closely monitor team members in performing different assessment techniques Supervise team members in actual performance of IPPA

c. Prepare a plan of care to meet the individualized needs of assigned patients

Explore with the members useful techniques in formulating appropriate nursing diagnoses. Instruct members to prioritize their patients needs Encourage team members to verbalize questions regarding NCPs conduct initial conferences hold mini case analyses Act as facilitator in discussions

Demonstrate adequate preparation for clinical experience Wear proper uniform and decorum Independently assume responsibility completing the care of assigned patients Perform appropriate assessment techniques (IPPA) Identify signs and symptoms of alterations in the health condition of assigned patient Identify abnormal physical assessment findings and proper recording of these findings. Document alterations in patients health status utilize assessment data as basis for problem identification correctly state problems to formulate nursing diagnoses state the rationale for prioritizing stated nursing diagnoses verbalize an appropriate plan of action for each assigned patient identify common alterations in the health status of assigned patient which interfere with basic needs

develop an individualized plan of care based on an understanding of growth and development and cultural diversity implement planned nursing interventions with guidance of the CI or team leader modify nursing care using the patients physical condition as a guide assess vital signs verbalize pertinent knowledge about drugs prior to administering medications seek opportunities to perform nursing skills correctly state outcome criteria for each patient goal report the response of patient to nursing care rendered identify ways to modify care attend the orientation and preconference identify the health needs of clients attend and participate in case discussions identify simple learning needs of the patient and the family offer sample explanations to clients recognize behaviors that indicate need for further teaching

d. Demonstrate competency in the performance of nursing interventions when caring for patients

work closely and supervise in the actual performance of nursing interventions require team members to submit or recite drug study supervise in medication preparation and administration provide learning opportunities for team members

e. Evaluate the effectiveness of the NCP.
