A letter giving permission from the person named on the certificate or if deceased, the appropriate authorised person (as stated in the Registrys Certificate Access Policy). Identification for applicant from Lists 1, 2 or 3; (see over for table); and Identification of the person from Lists 1, 2 or 3 who is entitled to the document (see over for table) (as stated in the Registrys Certificate Access Policy) (The WA Registry of Births, Deaths & Marriages Certificate Access Policy and fees can be viewed at www.bdm.dotag.wa.gov.au)
