Letter of Inquiry

Dr. Maria Gomez-Salinas Director of the Diabetes Clinic St. David's Hospital 1000 Greenberg Lane Wichita, KS 66780 Dear Dr. Gomez-Salinas: I am writing you in hopes of finding out more about how the new Glucoscan II blood glucose monitoring system, which a representative at Lifescan informed me that your clinic is currently using. Originally, I saw Lifescan's advertisement of this new device in the January 19XX issue of Diabetes Forecast and became very interested in it. I wrote the company and got much useful information, but was recommended to write several current users of the system as well. For a technical report that I am writing for a technical writing class at Johnson County Junior College, I need some help with the following questions: 1. How often does the Glucoscan II need to be calibrated in practical, everyday use conditions? 2. How accurate is the Glucoscan II compared to other similar systems that your patients have used? 3. What problems do your patients experience with this new device? The Lifescan representative indicated that your clinic is one the leaders in implementing new technology for diabetics, and therefore I am eager to hear from you. In the report I will acknowledge your contributions, and I will send you a copy of the completed report if you wish. Thank you for your time, and I hope to hear from you soon. Sincerely, Anita Teller Student, Medical Technology Johnson County Junior College

Mr. Rodrigo Iwamoto ABS Enterprise Cubao, Quezon City Dear Mr. Iwamoto: A complete check-up was made this morning when we learned from your letter of August 12 that you were unable to locate the 3 dozen E-143 towels on invoice 582. As our records clearly indicate that these towels were included in the shipment, we are asking your cooperation in making a further search to see if they were overlooked when the shipment was checked. Since the first of the year we have been numbering each item on the invoice and putting the same number on the package. In your order the E143 towels were marked No. 19 on the invoice, but it occurs to us that we have failed to mark the package and that perhaps it was set aside in your receiving room and not associated with the number on the invoice. Will you please make a further investigation and let us know your findings?

Sincerely,

Jenny Cruz

Letter of Recommendation
August 5, 2008 To Whom It May Concern: As Principal of George Mason High School I am delighted to recommend to you a stellar member of our teaching staff, Mr. Samuel Cohen, and to write this letter on his behalf. Sam has taught American History at the school for three years. He is one of our most popular and effective instructors and has already won several teaching awards. He came to us directly upon completion of his bachelors degree, but immediately settled in as if an old pro. Sam is a master of all of the ingredients of successful teaching, especially the ability to inspire. This gentleman is highly professional in both dress and demeanor. To my knowledge Sam has never come to work without both a suit and tie, more times than not a bow tie. The majority of others at the school, both teachers and students, dress differently and lean to more informality. Never once, however, has Sams manner of dress elicited the slightest comment. Why? It is simply because Sams attire seems entirely natural given his dignified comportment. That comportment in turn is driven by Sams love and respect for American History. Sam views himself as the messenger for something serious and important, and this is reflected in the way he teaches and in the way his students respond. Reaction to Sams classes has been overwhelmingly positive. So much so that more than once it has been reported to me that study hall students have been spotted lingering outside the door to Sams classroom listening to his lessons. Sams impact has even been felt outside the school with the parents themselves. His section on the importance of voting has inspired students to cajole their parents to register to vote. Several have mentioned this to me at parent-teacher meetings. One even called the school to have me pass on their thanks to Mr. Cohen. The only thing that we in the school administration have ever worried about in regard to Sam is the prospect of funding his bottled water. Sam is such a passionate and prolific speaker that he needs a steady supply to maintain his voice. So far we have gotten a pass on this, as students have bought it for him, including what must have been a years supply on his last birthday. It will be a sad day when we announce that Sam will be pursuing his graduate degree on a fulltime basis. We can only hope that he keeps his promise and returns to us as soon as the ink is dry on his new diploma. Sincerely, Harold W. Brown Principal George Mason High School

These assistants, under Sharon's management, were responsible for many of the office's basic administrative and clerical functions. Sharon effectively scheduled and managed several assistants to maintain efficient office operations. Sharon was always willing to offer her assistance and had an excellent rapport with the many constituents served by our office including clients, employers, and other professional organizations. She would be an asset to any employer and I recommend her for any endeavor she chooses to pursue.

Resignation Letter
August 13, 2010 Dear Sir Valdemar, Before anything else, allow me to express my sincerest apology for all the disappointments that I had brought to you. I am very sorry for being absent without permission and delaying all the pending assignments. It was a momentary lapse of judgment on my part and I regret doing so. For the past two years, I have been so happy and thankful to be a part of the team. My experience working for the company is wonderful. You have welcomed me with open arms. Despite my shortcomings as an employee, you have remained patient and supportive in building me professionally and personally. For these, I am forever grateful to you and the company. I have always considered it an honor and privilege to be a part of the family. Unfortunately, there have been some realizations on my part lately. The goals I want to pursue no longer coincide with the company goals. As you may recall I cannot finish the tasks that you have assigned to me. It took me so many days to finish a specific job that was supposed to be done in a day or two. Quite frankly, I dont want to resign but I deemed it necessary. I was supposed to file for an indefinite leave but this is too much to ask. Hence, I decided to resign. It was a difficult decision to make but in the end I had to be honest to myself and also put the companys interest in mind. I am not applying or accepting any job offers from other companies as of the moment. I plan to do some freelance work while sorting out what I would really like to pursue in the future. Ill be back in the office to get my stuff and turn over my work this week or next week. I would be glad to extend my assistance to facilitate the transition of the work to the next employee if needed. Once again, thank you for all the invaluable lessons you have taught me and making me a part of the company. I will be missing you and the whole team as well. I love you all, really. Best Regards, Randy C. Santos

Bulletin
September 24, 2010 Negros Dengue Cases Now Reach 5,900 with 30 Deaths By Danny B. Dangcalan The cumulative number of dengue fever cases in Negros Occidental this year has reached 5,912 cases as of September 11, or an increase of 538 percent from last years 926 cases in the same period, with 30 deaths, provincial health officer Ernell Tumimbang said. This does not include Bacolod City, which has 2,099 dengue cases as of September 13, with five deaths, records of City Health Office revealed. The dengue record this year exceeded the 5,000 cases in 2007 when the provincialgovernment declared an outbreak level. The Provincial Health Office (PHO) report showed that of the 30 deaths this year, five were from Himamaylan City, four each from Cadiz City and Silay City, three in Talisay City, two each in Sagay City and Murcia, and one each in Kabankalan City, La Carlota City, Bago City, EB Magalona, San Carlos City, Sipalay City, Isabela, Calatrava, San Enrique and Toboso. There were only 13 deaths in the same period last year, the PHO report showed. Tumimbang said current records show that dengue cases decreased by 15 percent in Morbidity Week 36 (September 5 to 11) from Morbidity Week 35 (August 29 to September 4). The dengue cases have started decreasing since Week 34 (August 22 to 28), the PHO report showed. Tumimbang earlier said the number will naturally increase since this is a cumulative report from January to September, but if we look at it per week, we will see that the cases are actually decreasing since last week of August. He said the situation is manageable as the attack rate and the case fatality rate, at 0.5 percent, is still considerably low considering that Negros Occidental is a big province. Board Member Melvin Ibaez, chairman of the Anti-Dengue Task Force, earlier said there is no need to declare a state of calamity as the provincial government has adequate resources to use to combat dengue, without having to use the provincial governments calamity fund.

Petitions
The President The White House 1600 Pennsylvania Ave Washington, D.C. 20500 Dear President Bush: For two decades, a deadly civil war has destroyed millions of lives in northern Uganda. Since 1986, 250,000 people have died, and the war has forced more than 2 million people to live in squalid makeshift camps where over 1,000 people die each week from starvation and disease. Each night over 40,000 children flee their homes to avoid abduction by a notorious rebel group that has abducted over 25,000 children, whom they force to become sex slaves or child soldiers. Those who resist are killed or mutilated, (WHO). United Nations Under-Secretary General for Humanitarian Affairs (OCHA) has described northern Uganda as the worlds worst forgotten crisis; and former UN Under-Secretary of Children in Armed Conflicts, Olara Otunnu describes it as slow-motion genocide. Yet, despite the magnitude of the problem, the our Government has not taken decisive action to alleviate suffering of the people of northern Uganda. The Ugandan Government continues to utilize an aggressive military approach to the conflict. However, this strategy has failed to put an end to the violence and exposes the communities to even greater dangers. [As one of your constituents], I am writing to ask you to raise this matter in Congress to: Call for an urgent congressional hearing on northern Uganda. Declare northern Uganda a special humanitarian emergency. Facilitate the dismantling of the internally displaced persons (IDP) camps, and aid in the resettlement of these over 2 million displaced persons. Ensure that Uganda abides by the United Nations Convention on the Rights of the Child Child Soldier Treaty to end the practice of recruiting children into war. Provide shelter, food and protection to the children who flee their homes every night to avoid abduction. For more information about the humanitarian catastrophe in northern Uganda please contact UNight: For the Children of Uganda a not-for-profit organization dedicated to building an effective constituency in the United States to end the 20-year war on the children of Uganda. Email: info@unight.org Thank you for your attention, I look forward to hearing from you. Truly yours, Mrs. Erlinda M. Lizada

Personal History Statement
Capiz State University Poblacion Mambusao Campus is one of my unforgettable schools because of the knowledge that I gained from smart, nice and supportive instructors and the whole administration. Staying at CAPSU Poblacion for 3 years is also one of my very unforgettable and challenging experiences because of those precious friends and different kind of bonding in different Departments especially to the BSOA Department that I treat as my younger sisters and brothers. I can also say that it is challenging because of problems that Ive encountered and different task that was given me. I am Michelle Alayon Lariosa a graduating student of Bachelor of Science in Office Administration, 24 years of age and a resident of Marcelo Street Mambusao, Capiz. My parents is Marianito Calante Lariosa and Esel Alayon Lariosa, I have one sister named Diana Marie A. Lariosa, she is 23 years old and a professional and licensured nurse. I finished my vocational course Computer System Design and Programming at AMA Computer Learning Center in Roxas City. In outside school organization I was elected as 3rd SK Kagawad of Brgy. Poblacion Proper Mambusao, Capiz and served the community for 6 years (2001 2007), I am also one of the elders of Youth for Christ (YFC) Mambusao Chapter and Ive been in the religious youth organization for 7 years. I was an active member of Kalingawan Performing Arts Group and CAPSU Choir for 2 years of this institution. At present I am the elected President of BSOA Department. As graduating student I will surely reminisce the time that I was spent in this institution. For all the knowledge which the instructors shared with us will be surely apply to our future and through their motivation we can also be a good employee someday, and I thank them. For all my classmates thank you also for the great memories and different kind of bonding and relationship that we shared because we treat each one another as true brothers and sisters.
