This Risk Assessment form is a generic form and may not cover all the Risks associated with your project. 5 Classifications of Hazards: 1. Risk Assessment
In general there are four ways of dealing with hazards: 1. Elimination, 2. 2 Change equipment or materials materials, 3. Change work methods, and 4. Use personnel protective equipment (PPE). These four ways are based on the hierarchy of controls provided for by the OHS Regulations. The controls adopted should always be the highest level that can be reasonably attained.

Before commencing work on live equipment: The Testing Enegrised Circuits Authorisation must be completed, complete the necessary OH&S requirements to work in the Disciplines labs, i.e. Lab Induction, General Access Quiz and if necessary Machines Lab Access Quiz, obtain permission for your supervising academic to work live with Hazardous potentials, enlist a 'buddy' that will be present with you whilst working with live Hazardous potentials potentials, inform a suitably trained (CPR) technical staff member each time you are working with live hazardous potentials.

Risk Analysis
There are 5 steps to performing a successful Risk Assessment: 1. Identifying the Hazards, 2. Assess the Ri k 2 A th Risk, 3. Documenting, 4. Controlling, and 5. Ongoing monitoring. There are many tools available to help with this process the one most tools process, generally used is WorkCovers Hazpak.

Risk Assessment is a very judgmental procedure. What you consider safe may be considered unsafe by others General rules of OH&S: 1. dont assume anything. y g 2. There is no such thing as common sense.

Please: 1. complete Risk Assessment form & Hazpak, p p , 2. if you will be working at an Industrial Sponsors premises you MUST also supply a Risk Assessment of the work space you will be working in 3. 3 if you have ANY about your Industrial Sponsors safety then please contact Tharaka Dissanayake immediately. 4. arrange for a meeting with your project supervisor(s) to discuss results 5. if you are both happy with documents submit in Assignment Box foyer of EA Building Unsupervised Access, Equipment & Benches will not be issued/allocated until Risk Assessment documentation has been submitted.

Remember: Your academic supervisor is the person who the Uni deems to be responsible for your safety whilst doing your Final Year Project. If you are unhappy with any of the conditions or equipment you are working with: if this is an issue with University conditions/equipment please see me or your academic supervisor, y p , if this is an issue with conditions/equipment provided by an Industry Sponsor please see Zhiyong Chen. If you think something or a situation is too dangerous you dont have to do it you can say NO ....

Failure to comply with these 3 OH&S requirements will result in you not being allowed to progress from PartA to PartB OR present your FYP work on demonstration day.
