NURSING CARE PLAN


ASSESSMENT SUBJECTIVE DATA: The patients father, which is the primary care giver, complained that his child is difficult to be comforted and be stopped when crying. Di ko kasi alam kung paano patahanin ito e, lagging mama niya kasi kasama niya. Lagi kasi akong gabi na umuwi gawa ng trabaho ko kaya di ko na sila nakakahalubilo. RATIONALE For the care giver to understand what is his care receiver is going through and develop sympathy. To see if care giver is ready for the responsibility or is he needing for more knowledge about care giving To at least ease the discomfort of the patient and at the same time first step of teaching the care giver in care giving. EVALUATION

After more than

30 minutes, the care giver was able to give comfort to the child and was able to stop the child from crying.

Thorough assessment of care givers readiness and knowledge through interview and observation

After explanation and teachings, father or the care giver is starting to perform techniques in attending his childs needs. Father has shown positive feedback about what he had learned in the health teacing.

The caregiver

should provide an effective comfort for the patient in at least a short period.

The care provider must be helped by anyone in providing comfort for at least short period of time. He must also be

the patients father. OBJECTIVE DATA: As of my observation, baby is persistently crying even if comforted by his father. The caregiver should acknowledge and perform techniques in care giving.

Advise the father regarding time management. Advise him to have more time or maximize his time for his family despite of his working schedule.
