Classification Mucoltytic
Mechanism of Action Mucolytic that reduces the viscosity of pulmonary secretions by splitting disulfide linkagesbetween mucoprotein molecular complexes. Also, restores liver restores of gluthathione to treat acetaminophen toxicity.
Indication Adjunct therapy for abnormal viscid or thickened mucuous secretions in patient with pneumonia, tuberculosis
Side Effects/ Adverse Effects Chest tightness, flushing, hypertension, hypotension, tachycardia
Classification Glucocorticoids
Mechanism of Action Significant glucocorticoid effects caused by drugs high affinity for glucocorticoid receptors.

Nursing Responsibilities Tell patient to notify prescriber if she develops chicken pox or measles
Side Effects/ Adverse Effects There are no known side-effects to the use of the vitamins and trace elements contained in CLUSIVOL Syrup at the recommended dosage levels. However, certain vitamins and trace elements do exert toxicity in massive doses .
Nursing Responsibilities Tell patient to seek medical attention right away if any of these SEVERE side effects occur when using Multivitamins with Minerals: Severe allergic reactions (rash; hives; itching; difficulty breathing; tightness in the chest; swelling of the mouth, face, lips, or tongue).

Advise patient not to treat diarrhea without consulting health care professional. May occur up to several weeks after discontinuation of medication
Name of Drug Brand Name: Procoralan, 75mg, Oral, OD Generic Name: Ivabradine
Mechanism of Action Ivabradine is a heart rate lowering agent that works through selective and specific inhibition of the cardiac pacemaker If current. If current controls the spontaneous diastolic depolarisation in the sinus node and regulate heart rate.
Indication
Treatment of chronic stable angina pectoris in people with a normal heart rhythm.
Side Effects/ Adverse Effects Luminous visual phenomena, Blurred vision, Slow heart rate, Headache Dizziness. y
Nursing Responsibilities Tell client to notify prescriber if she experience any other problems with her vision while taking this medicine.. Monitor clients heart rate if persistently below 50 beats per minute while taking this medicine, or she experiences symptoms of a slow heart rate such as dizziness, fatigue or low blood pressure, and report to prescriber.
Name of Drug Brand Name: Digoxin 0.25mg, tab, OD Generic Name: Lanoxin
Mechanism of Action Increases the force of myocardial contraction Prolongs refractory period of the AV node Decreases conduction through the SA and AV nodes
Side Effects/ Adverse Effects Fatigue, headache, weakness, blurred vision, yellow or green vision, Arrhytmias, bradycardia, ECG changes, AV block, SA block, Anorexia, nausea, vomiting, diarrhea, thrombocytopenia.
Nursing Responsibilities Monitor apical pulse for 1 full min before administering. Withhold dose and notify physician if pulse rate is <60 bpm in an adult, <70 bpm in a child, or <90 bpm in an infant. Also notify health care professional promptly of any significant changes in rate, rhythm, or quality of pulse Monitor blood pressure periodically in patients receiving IV digoxin
Name of Drug Brand Name: Omeprazole 40mg, Oral, OD Generic Name: Prilosec
Mechanism of Action Binds to an enzyme on gastric parietal cells in the presence of acidic gastric pH, preventing the final transport of hydrogen ions into the gastric lumen
Side Effects/ Adverse Effects Dizziness, drowsiness, fatigue, headache, weakness, chest pain, abdominal pain, acid regurgitation, constipation, diarrhea, flatulence, nausea, vomiting, itching, rash, allergic reactions
Nursing Responsibilities Assess patient routinely for epigastric or abdominal pain and frank or occult blood in the stool, emesis, or gastric aspirate
Name of Drug Brand Name: Innohep 3500 units, SC, OD Generic Name: Tinzaparin
Classification Anticoagulant
Mechanism of Action Tinzaparin binds to the plasma protein antithrombin III, forming a complex with then accelerates the inhibition of factor Xa. Its affinity for factor Xa is 2-4 times greater than that of ATIII. The inactivation of factor Xa in turn will exponentially generation of thrombin (factor IIa) molecules, which is needed to activate fibrinogen to fibrin. The coagulation cascade is inhibited because fibrin cannot be formed in the presence of tinzaparin. Like all LMWH, it cannot be given intramuscularly due to increased risk of hematoma.
Indication Prevention of postoperative venous thromboembolism in patients undergoing orthopedic surgery and in patients undergoing general surgery who are at high risk of developing postoperative venous thromboembolism. It is also used for the treatment of deep vein thrombosis and/or pulmonary embolism. It is indicated for use in preventing clot formation in indwelling intravenous lines for hemodialysis.
Side Effects/ Adverse Effects Mild headache, dizziness; back pain; fever; pain, irritation, swelling, or bruising of the skin where the medicine was injected; nausea, vomiting, stomach pain, gas; diarrhea, constipation; mild skin rash; or sleep problems (insomnia).
Nursing Responsibilities Explain patient importance of laboratory monitoring to ensure effectiveness of drug while maintaining safety Teach patient warning signs of bleeding and instruct her to report these signs immediately.
Classification Anti-asthmatic
Indication Management of reversible bronchospasm associated w/ obstructive airway diseases eg bronchial asthma, COPD.
Side Effects/ Adverse Effects Headache, pain, influenza, chest pain; nausea. Bronchitis, dyspnea, coughing, pneumonia, bronchospasm, pharyngitis, sinusitis, rhinitis.
Nursing Responsibilities Position high back rest Do back tapping after nebulization Do not give food immediately it can cause vomiting
Mechanism of Action
Indication Peptide based diet for patients w/ impaired GI function. Appropriate also for those w/ fat malabsorption requiring a low fat diet; provides total nutrition support via tube-feeding or oral administration; 1st choice after trauma, surgery at the ICU.
Nursing Responsibilities Place on high back rest to prevent aspiration Monitor for diarrhea Do not suction after feeding to prevent aspiration
