TERMS AND CONDITIONS The applicant shall submit the following: a) one (1) unit of machine randomly selected from its stock, and b) Operators manual or brochure containing the technical specifications of the machine.

2. The machine shall be broken in and adjusted by the applicant prior to the test by AMTEC. Subsequently, breakdown of the machine during AMTEC test shall terminate the test. 3. The applicant shall send a representative to be present during the conduct of the test and an operator to run the machine during the laboratory and field tests. 4. A testing fee shown in Table 1 shall be charged the applicant. Fifty percent (50%) shall be paid to AMTEC in advance upon acceptance of the application. This advanced payment becomes non-refundable once the test commences. Table1. For out-of-town tests, the applicant shall shoulder the transportation expenses (fuel, van rental, or plane fare) of at most three (3) AMTEC personnel. AMTEC shall schedule the test of the machine on a first-come-first-serve basis. Under very meritorious situations, overtime testing may be scheduled subject to availability of test staff and facilities/equipment. In this case, the applicant shall also pay for overtime at Three Thousand Pesos (P 3,000.00) minimum per machine. The test report shall be completed within one week. AMTEC shall conduct test and evaluation of the machine based on Philippine National Standards or, if not available, on AMTEC Standard Procedures of Test and Inspection. AMTEC shall prepare a test report within two (2) weeks after the completion or termination of the test of the machine and provide the applicant one (1) original copy. The test report shall be released only upon full payment of the assessed testing fee. The applicant may request for another original copy of the report for Five Hundred Pesos (P 500.00) and/or authenticated duplicate copy of the test report for Two Hundred Pesos (P 200.00). AMTEC shall have right to publish the results of the test of a commercial model. It shall also have the option to release information regarding the test to individuals and groups who would have legitimate and constructive use for it. However, result of the test of a prototype model shall be confidential and cannot be used for any commercial purpose.

10. The applicant shall pullout its machine within two (2) weeks after completion, suspension, or termination of the test. After the two-week period, AMTEC shall charge a storage fee of one (1) percent of the cost of the machine per day. 11. The test conducted on the machine as per its specification shall be valid only for a period of five (5) years. A retest or revalidation of the performance of the machine shall be required after this period.
