We Need a Hero to Fight Cavities

Use your imagination to create a hero that is brave enough to lead our Cavity Patrol in fighting cavities.
The school with the most entries will receive a $500 cash donation!
This contest will include children in grades K-5. One winning entry will be chosen from each category, K-lst grade, 2nd-3rd grade, 4th-5th grade. The overall winner from each category will receive a 3D Nintendo DS! Also, a grand prize winner will be chosen overall and will receive an IPAD 2.
All contest entries should be returned to Johnson Pediatric Dentistry, 772 N. Dixie Dr., #101. We will be accepting entries from February lst-22nd. Upon return, each participant will receive a toothbrush and
certificate for ice cream.

Early orthodontic treatment will minimize more complex treatment later on in life.
