Description: AARS, Inc. is an innovative substance abuse prevention, treatment, and research agency serving the greater SF Bay Area. This San Francisco County funded violence prevention program specifically targets aftercare/reentry youth involved with juvenile justice system in San Francisco. This prevention program consists of intensive case management and enrichment activities for youth to help support their positive lifestyle free of violence and alcohol and other drugs. Under the supervision of the Program Manager or his/her designee, incumbent will assist in planning, development, implementation and evaluation of the program work plan. The Case Manager is also responsible for the collection of data for program evaluation and administrative reports. This position requires an ability to maintain strict confidentiality and handle information with tact and diplomacy. Responsibilities (other duties may be assigned): Conducts comprehensive client needs assessments and monitors and facilitates client progress; Serves in a multi-disciplinary team and ensures right level of care is provided to the client; Carries a client caseload and provides intensive case management and enrichment activities for clients and their families; Assists clients in developing individualized plans of care with specific goals, objectives, and measurable outcomes to address their needs such as mental health, substance abuse, vocational, and educational issues; Makes referrals and provides linkages to relevant services and treatment sources; Provides advocacy for clients and their families; Provides crisis intervention as needed; Develops and maintains confidential client charts; maintains quality assurance standards; monitors security and performs regular audits for compliance and accuracy of data; Completes accurate and timely documentation and billing per City and County requirements; Assists in the preparation and timely submission of monthly, quarterly, year-end and any other reports; Develops and implements curricula as needed to meet the changing client needs in the program; Develops and maintains strong partnerships with public and private community organizations; Assists Program Manager in effectively implementing the violence prevention program; Attends meetings, training, conferences, etc. as required.

Qualifications: Bachelors degree in related field; At least 2 years experience in violence prevention or mental health and substance abuse prevention or treatment; Strong interest and ability to work well with at-risk and in-risk youth; Ability to work well in multi-disciplinary team; Working experience with Motivational Interviewing and Stages of Change; Familiarity and experience with group dynamics and as well as working with families; Working knowledge of / familiarity with cross-cultural issues; Bicultural / bilingual in Spanish preferred; Good conceptual and interpersonal skills; Ability to communicate effectively across all levels of the agency; Knowledgeable in community resources in San Francisco; Ability to work flexible hours; Knowledgeable and skilled in the use of Microsoft Office and the internet; Valid California drivers license (Class B a plus).
