Check list of items that will be required for Lane Architecture +Design, P.C. to perform a review of a proposed renovation to a dwelling in a multifamily building: Trigger points that will cause one to need to file with the NYC DOB are anything that does not constitute minor alterations or ordinary repairs as written in the New York City Building Code 27-126: For the purposes of this code, minor alterations or ordinary repairs shall not include the cutting away of any wall, floor, or roof construction, or any portion thereof; or the removal, cutting, or modification of any beams or structural supports; or the removal, change, or closing of any required means of egress; or the rearrangement or relocation of any parts of the building affecting loading or exit requirements, or light, heat, ventilation, or elevator requirements; nor shall minor alterations or ordinary repairs include additions to, alterations of or rearrangement, relocation, replacement, repair or removal of any portion of a standpipe or sprinkler system, water distribution system, house sewer, private sewer, or drainage, system, including leaders, or any soil, waste or vent pipe, or any gas distribution system, or any other work affecting health or the fire or structural safety of the building. 28-105.4.2 of the New Code states: Work Not Constituting Minor Alterations or Ordinary Repairs. Minor alterations or ordinary repairs shall not include: 1. The cutting away of any load bearing or required fire rated wall, floor, or roof construction, or any portion thereof; 2. The removal, cutting, or modification of any beams or structural supports; 3. The removal, change, or closing of any required exit; 4. The addition, rearrangement, relocation, removal or replacement of any parts of the building affecting loading or exit requirements, or

light, heat, ventilation, or elevator requirements or accessibility requirements or any fire suppression system; Additions to alterations of, or rearrangement, relocation, replacement, repair or removal of any portion of a standpipe or sprinkler system, water distribution system, house sewer, private sewer, or drainage system, including leaders, or any soil waste or vent pipe, or any gas distribution system; Any plumbing work other than the repair or replacement of plumbing fixtures, piping or faucets from the exposed stop valve to the inlet side of a trap; The alteration or repair of a sign for which a permit is required; or Any other work affecting health or the fire or structural safety of the building or the safe use and operation of the serviced equipment therein.

Drawings of general information required (when filing for permit): Title Block in a NYC B-Scan format Architects information Plot plan locating unit boundary showing lot, block, zone, street names, north arrow, distance to nearest corner, hatch area of proposed work on the building plan. Indicate Key Plan with the elevator(s) and egress stairs located. This can be separate or included in the plot plan. Drawing List Door schedule with dimensions and fire ratings. Tenant Safety Notes Smoke/Carbon Monoxide Detector Notes (also refer to Misc. below)

List of controlled Inspections For Alt. 2, Dir. 14 (no egress change) provide the note: No Change of use, occupancy, egress or zoning this application. Include in the general notes the buildings rules (perhaps found in the Alteration Agreement) that explains for the contractor to read: 1. the permissible hours of construction 2. the permissible days of construction 3. the method of disposing of debris 3. Show existing conditions: Plan with all partitions indicating where work is proposed and the adjacent rooms identified. Indication of all appliances, plumbing fixtures, riser locations (plumbing, electrical, gas, heat). Indication of all demolition work. 4. Show proposed scope of work: Plans, elevations, sections (to extent required so that proposed work is fully comprehensible). o Indicate demising walls and tenant separation. o Indicate room names and usage, including dimensions. o Show any egress stairs and convenience stairs near or in proposed areas of renovation. o Indicate corridor or hallway and stair widths. o Indicate handicap accessible areas and paths. o Locate smoke detectors (within 10 of bedroom doors, outside rooms). o Note gas fired equipment on plans. o Indicate any plumbing fixture that is removed, relocated or is new to the floor. Plumbing riser diagram with sizes of waste lines and vents (for plumbing changes). Details of unusual conditions, including any new proposed penetrations through exterior surfaces. Confirmation that all electrical work will be performed by a licensed electrician.

Confirmation that all plumbing work will be performed by a licensed plumber. 5. Wall and Ceiling Construction: 6. Partition types (new, existing and demolished and type of construction for rated partitions). Non-combustible materials; specific anchorage for ceiling conditions. Fire stopping to be inspected on a TR-1 form by the applicants architect. Removal of the existing ceiling is discouraged and only approved on a case by case basis by the building board and management. All fire separations must be maintained at all times. 7. Flooring: Installation details including waterproof membrane if wet space is modified or expanded (if permitted). This includes spaces such as bathrooms, laundry areas, and kitchens. Installation details including sound attenuating material if proposing stone or tile over a quiet space. 8. Kitchen: All appliances clearly identified. Plumbers letter confirming existing gas meter and piping is adequate if a new large gas range is proposed. Indication of proposed range hood type; details of any proposed venting to exterior (if permitted). Indication that the range is at a proper distance from any operative window for ventilation if so desired. Electrician or engineers letter confirming existing service adequate for the out requirements of a new electric wall oven (if applicable). 9. Bathrooms: If the toilet is to be replaced, then it is to be a 1.6 gallon low-flow toilet. Drop in tub detail including a waterproofing membrane. If shower is new or replaced, show details at curb, drain, and wall.

If steam shower is proposed, show additional protection requirements only if permitted by Board). 10.Clothes Washer/Dryer (if permitted by building): Watertight pan recommended; if dryer venting to exterior detail and sections of proposed new vent. Indicate if the dryer is vent-less or is vented to the outside. 11.Through-the-wall Air Conditioners and exterior vents from dryers (if permitted by building): Indication of location(s). Sleeve installation detail and details at the exterior louver. Exterior elevation, showing adjacent windows. Indication of louver style and color. 12.Central Air Conditioner systems and split systems (if permitted by building): Indication of the locations of all equipment; specifications and cut sheets for all proposed equipment. Installation detail of condenser units with sections. Installation details of air handling units. Details of vibration and sound proofing. Indication of drip pans, water sensors, alarms and automatic shut-off switches at air handling units. 13.Windows: Details of any installation that varies from the buildings approved windows include head, jamb, sill details; exterior elevation; materials; color; and window specifications. 14.Electrical: Electrical plan(s). Show location of proposed changes to the locations of electrical outlets, telephone wall jacks, cable wall jacks. This information might be included on the architectural drawings.

Show location of ground fault interrupting electrical outlets (if in wet or moist environments). Licensed electricians or engineers letter if upgrade requested. Confirmation that the electrical panel will remain accessible. Licensed electricians letter stating if the existing electrical supply to the unit is sufficient for a new steam shower unit (if permitted by the Board). 15.Handicapped accessibility (refer to Local Law 58/87 for a complete checklist): Provide ANSI standard cartoons and toilet room symbols. See below for examples: New swinging doors to be minimally 2-10 wide. New pocket doors to be minimally 2-8 wide. New halls to be minimally 36 to 42 wide (depending upon turning area beyond). 5-0 minimal diameter turning circle as a maneuvering space. New kitchen sink in new counter top that can be lowered in the future to allow one that is handicapped to use it. Provide the correct clear width at the new kitchen sink. Proper maneuvering clearance at doorways. Wall mounted devices are between 15 and 48 above the finished floor (except at special locations such as behind appliances). This can be a general note. Proper location of new toilet fixtures from the walls. This can be a standard diagram. Proper clearances around new lavatories. This can be a standard diagram. Proper dimensions of new showers. This can be a standard diagram. Proper dimensions of new bathtubs. This can be a standard diagram. Proper locations of blocking in walls for future grab bars. This can be a standard diagram. 16.Radiators: Any modification to existing heating system.

17.Intercom: Any modification to existing (if permitted by building). 18.NYC Energy Conservation Code (NYCECC) Compliance: Professional Statement Energy Analysis if project is not exempt. 19.Filing Documents (if filing for a permit at the municipalities): Tenant to submit to the building management the following forms for review (if applicable): PW-1 TR-1 TR-8 Form and indicate areas that apply to this project that would be one of the following:

Schedule C ACP-5 Landmark Commission application (if applicable) Other Include in the architects general notes that the work will be inspected as a Directive 14, with the inspection tasks of (state the tasks such as Fire Stops, Energy Code Compliance Inspections, and Final Inspection).

20.If additional square feet is proposed to be built upon the building: Submit the zoning calculations (showing information that includes the conformance to the set back requirements, FAR, existing buildings floor area and the proposed buildings area, bulk compliance, height compliance,) Include reference numbers from the zoning code for cross referencing. If this is in a Landmark District, then the NYC Landmark Commission will need to review the proposal to determine if it complies with their requirements (such as style, visibility from the street, height). This can be reviewed after the COOP approves the application. Any objection by the Landmark Commission can override any previous approval that the applicant received from the COOP or any other municipality. Show foundation plans and details (signed by an architect or a structural engineer) if applicable. Show wall and roof structural plans and details (signed by an architect or a structural engineer) if applicable. Submit ResCheck calculations to show that the new construction complies with the energy and environmental requirements. Coop to determine the redistribution of shares when the square feet of the overall building is altered. 21.Miscellaneous: Signed Alteration Agreement. Show locations of any proposed chasing of ceilings, floor slabs, or demising walls (will there be any trenching for new wiring or piping in the ceilings, floors, or demising walls if permitted by the building management). Offset of risers. Installation of garbage disposals. Signed Form that describes the Contractor Procedures. Certificates of insurance from the applicants contractors dates that have not expired and the amounts to conform to the buildings requirements and proper listing of those covered by the policy before any construction con proceed (even if the building management approves this application first if that is their policy). Include the building management, the applicant, and the applicants building (perhaps as required in the Alteration Agreement) as an additionally insured on all of the contractors insurance forms that pertain to this project. Show locations of the new and existing carbon monoxide and smoke detectors as required by code. After construction, applicant to provide to the board a written statement that the carbon monoxide and smoke detectors were installed and activated properly and conforms to the requirements of the NYC code.

Check payable of $850 to Lane Architecture +Design for our office to review an applicants submittal. This fee is for our service of reviewing the application once and a second time if there are items missing during the first submittal. (This is usually the only fee charged. Any additional submittals will be charged on an hourly basis which is $185 per hour. If the building management requires our office to visit the site, there will be additional service fees. Payments in full to be received by Lane Architecture + Design prior to services rendered. Fees subject to change after June 2012.
