Identifying Data The bay is 1 year and I month old now, shes a baby girl. Shes in good health that can be identified by her body. Quite fat. She already has 8 teeth. The baby has a fair, smooth and soft skin and her hair is kind of curly type which is same with his father. The way the baby smile and the eyes are all inherited from her father. The baby has birth marks in her butt area and at between her legs.

II.

Medical History The bay was delivered at San Carlos Doctors Hospital, February 2, 2011 about 4:30 in the morning. She was delivered by caesarean section because e the mother has having difficulty with normal labour. The bay weighs 2.9 kilos when delivered and was brought home February 4, 2011.

Educational History (if child is already in school) The child is not yet studying because she is still 1 year old and 1 month.

Interview Proper 1. Discuss the mothers reactions when she discovered shes pregnant. When the mother discovered that she was pregnant, she was shocked because she didnt expect it. It was an unwanted pregnancy. The first she did was to really abort the child, fearing that her mother would really get angry and mad. 2. Discuss also the fathers reaction. The case was an unexpected pregnancy and the father did not take responsibility with it or did not even recognize it, so I was not able to interview the father. 3. What were the problems encountered by the mother from the start of pregnancy to birth of baby? The mother encountered a lot of problems when shes having her pregnancy, emotionally and psychologically.

Describe/Discuss First step The first step of the baby was when she was 11 months old. First word uttered/speech The first word uttered by the baby was papa.

First food intake The first food eaten by the baby was Gerber banana flavour.

First health problem The first health problem encountered by the baby was cough and colds.

First medical check up/first immunization The first medical check up of the baby was at San Carlos Doctors Hospital when she was 1 month old, one month after her birth. The paediatrician that checked the baby was Grace Uy. The first immunization was taken at Canlaon City Health Center, same month when she had her first check up.
