Main Menu

When logging onto the system you will be greeted by the main menu page, from this page you can Add, view and
delete Patients, Physiotherapists, treatments and Hospitals by using the buttons provided, or as another way of
doing the same thing you can click on the tabs at the top. From the menu page you can also view an electronic
version of the Treatment and Diagnosis codes that will be printed out and stuck around the Physiotherapists and
Receptionists offices.

Adding Patients

Once you have clicked on the Add Patient button on the main page it will take you to this form, which you
fill out and once that has been done you click the Create button in the right hand corner. This will then
add the patients details to the patient table. These details will then be stored so that they can be updated
and deleted at will. The red asterisks (*) next to the field names mean that the form can not be submitted
without these fields being filled in (Mandatory Fields).

Once you have clicked on the View Patient button on the main page it will take you to this page, while on
this page you can view what patients are already enrolled within the Physiotherapy Centre. You can also
print this if you click on the Print button at the bottom of the page, next to that button is a button called
Download as CSV which will allow you to open the enrolled patient list in Excel, from there you can edit
the list at will. If you want to update the patient table then you can select the create button on the top of
the page which will then take you to the add patient page where you can input the data.

Adding Physiotherapist

Once you have clicked on the Add Physiotherapist button on the main page it will take you to this form,
which you fill out and once that has been done you click the Create button in the right hand corner. This
will then add the Physiotherapists details to the Physiotherapist table. These details will then be stored so
that they can be updated and deleted at will. The red asterisks (*) next to the field names mean that the

Viewing Physiotherapist

Once you have clicked on the View Physiotherapist button on the main page it will take you to this page,
while on this page you can view what Physiotherapists work within the Physiotherapy Centre. You can also
print this if you click on the Print button at the bottom of the page, next to that button is a button called
Download as CSV which will allow you to open the Physiotherapist list in Excel, from there you can edit
the list at will.

Adding Treatment Sessions

Once you have clicked on the Add treatment session button on the main page it will take you to the Add
treatment session form, which you fill out and once that has been done you click the Create button in the
right hand corner. This will then add the Treatment Session details to the Treatment Session table. These
details will then be stored so that they can be updated and deleted at will.

Viewing Treatment Sessions

Once you have clicked on the View Treatment Session button on the main page it will take you to this
page, while on this page you can view what Treatment Sessions have been made within the Physiotherapy
Centre. You can also print this if you click on the Print button at the bottom of the page, next to that
button is a button called Output to CSV which will allow you to open the appointments list in Excel, from
there you can edit the list at will.

Adding Hospital

Once you have clicked on the Add Hospital button on the main page it will take you to this form, which
you fill out and once that has been done you click the Create button in the right hand corner. This will
then add the Hospitals details to the Hospital table. These details will then be stored so that they can be
updated and deleted at will. The red asterisks (*) next to the field names mean that the form can not be
submitted without these fields being filled in (Mandatory Fields).

Viewing Hospital

Once you have clicked on the View Hospitals button on the main page it will take you to this page, while
on this page you can view what Hospitals are referring to the Physiotherapy Centre. You can also print this
if you click on the Print button at the bottom of the page, next to that button is a button called Download
as CSV which will allow you to open the hospital list in Excel, from there you can edit the list at will.

Hospital chart

This is the hospital chart, to view the chart you click on the charts button at the top of the screen and then select the
hospital chart. It shows how many patients have been referred to the physio from each hospital. On the right hand
side of the page is the chart key, this tells you what each segment of the chart belongs to.

Gender Chart

This is the Gender chart that is on the system. The gender chart is situated next to the hospital chart. This shows the
comparison between how many patients are male and how many patients are female.

Diagnosis Chart

This is the diagnosis code chart which shows the most common diagnoses throughout the physiotherapy. On the
right hand side of the page is the chart key which tells you what the different segments of the chart refer to.

Treatment chart

This is the treatment chart. This shows the amount of times each treatment has been carried out. On the right hand
side of the screen is the chart key which indicates what each segment of the chart refers to.

Physio chart

This is the physio chart. This shows how many patients each physio has. On the right hand side of the screen is the
chart key which indicates what each segment of the chart refers to.

Each of our charts update automatically. This means that whenever you input new data in the system for example a
new physio or patient or if you amend any information on it then they charts will automatically change. The chart
key is linked from the tables on the system so that all of the information for the different charts in on the correct
page. This will ensure that you know exactly what each part of the chart belongs to.

Queries
The screen shots below show all the tables that have queries on. To use the query you must type the keyword
(specific codes, patient/physio names, hospital areas/name etc.) that you would like to find in the search bar at the
top of the page. This then searches through the table and finds all the matches for your search and displays them in
a table format.
