Registration Information
Only one form per registrant; duplicate as necessary. Mastercard, Visa, and checks made payable to the Kansas Dental Association are accepted for payment. An entire office can send their forms together and make one payment. A $40 late fee will be charged for EACH registrant after May 28, 2012 EXCEPT for dental spouses/guests and dental students. A $50 processing fee will be charged for each on-site registration EXCEPT for dental spouses/guests and dental students.

Hotel Reservations
Room reservations for the meeting will be made by individual attendees directly with the Hotel at Oldtown reservations at (877) 265-3869. All reservations must be guaranteed with a major credit card. Reservations must be received on or before 5:00 PM, Tuesday, May 15, 2012. After this time, reservations will be based on a space-and-rate-available basis. The KDA does not reserve or put a hold on extra rooms. If any meeting attendee books a room outside the group block through a travel agency or discount site, please let a KDA Staff person know so we can have it credited towards the KDAs block.

All refund and cancellation requests must be made in writing. A $50 handling fee, PER REGISTRANT, will be charged for all refunds prior to May 28, 2012. No refunds will be made after May 28, 2012 for any reason. Registrations may be transferred at any time without penalty.

Robert Marbach, BA, CDT is a Past Board of Directors at The Pankey Institute. He is Founder of Authentic Dental Lab in San Antonio, TX. Mr. Marbach is a Board Certified Dental Technician currently working on his Masters in dental technology and has over 25 years of dental experience. In 1999 Bob received his Technical Certificate of Completion from the Pankey Institute. Mr. Marbach is a past member of the Advisory Board for The Canadian Journal of Dental Technology. He is also a member of the prestigious TEREC Group, a technical research consortium. Bob has lectured internationally for many years and his lecture has received approval from the Academy of General Dentistry. He has also received his Fellowship in the Academy of Dental Facial Esthetics and received the National Association of Dental Laboratories Inventors Award for developing and patenting the Shade Wand, a shade-taking light used by dentists worldwide. And, in 2011 he was inducted into the American Society For Advanced Dental Esthetics. Corky Willhite, DDS has achieved Fellowship by the Academy of General Dentistry and the American College of Dentists. He is one of 50 Fellows of over 6,000 American Academy of Cosmetic Dentistry members worldwide and has served on their Board of Governors (currently American Board of Cosmetic Dentistry) and spent years as an Examiner for Accreditation. He is on the faculty of the Center for Esthetic Excellence in Chicago and post-graduate esthetic programs at Eastman Dental Center and the University of Minnesota. His private practice in suburban New Orleans, The Smile Design Center, is limited to Cosmetic Dentistry.

One child per classroom may experience a traumatic dental injury. The primary care dentist needs to act quickly and skillfully. Triage treatment determines the prognosis. The program is designed to improve the participants ability to treat a traumatic dental injury and is designed for all attendees. Each participant will receive: Pre-packaged supply list Listing of the necessary radiographs Full understanding of the healing process Step-by step evaluation approach Logical evaluation form that reviews: Extra and intra-oral soft tissues, Facial and dental bones, Periodontia and tooth structures Listing of the necessary clinical tests Specialist referral list Timetable for follow-up treatment

Identify the basics of occlusion and how it relates to the success of anterior restorations and full mouth rehabilitation. Facing the esthetic challenges in your practice to decide today what type of restoration is best for your patient. View clinical secrets on preps, temps, impressions, cords, cements and why is shade selection difficult and how to get it right the first time.
