Brief instructions for completing the questionnaire

Identification
1. Name of the department/ unit/ section/ centre: the official name of the department or section or Centre which is responsible for collecting, compiling, processing and producing the educational statistics in the agency. Eg: Planning Section, Statistics Department, etc In this document, we will refer to this office as department. 2. Name of the parent department/ institute/ agency: the official name of the department, institute or agency to which your department belongs. Eg: Office of Basic Education, National Institute for Education Administration and Planning, Centre for Education Management. 4. Type of institute or agency: Please check appropriate box. Government: Fully controlled by government. Semi-government: Autonomous organization supported by government.

Personnel
7. Number of staff by duty and responsibility: One staff member can be assigned more than one responsibility. 7.1 Administration personnel: staff members responsible for undertaking administrative tasks such as: bookkeeping, disbursement, dispatch for your department. 7.2. Learning achievement: includes information such as repetition, completion, graduation and student performance by subject. 1.2.10.Education finance: includes information regarding school income and expenditure. 1.2.11.Community participation: includes information such as existence of Parent-Teacher Association, community involvement. 1.2.12.In-service training: includes information such as number of teachers who have received training, type of training, etc.
