JOB DESCRIPTIONS OF NURSE MANAGERS

CHARGE NURSE JOB SUMMARY Provides leadership in assessing, planning, implementing, and evaluating patient care in specific unit/s assigned to him/ her during his/her shift. He/she is responsible in ensuring that agreed standards and all regulatory requirements are complied with. He/she manages all the assigned staff, equipment and material resources in the unit. Ensures that continuity of care with other shifts and teamwork are maintained and promoted all the time.

DUTIES AND RESPONSIBILITIES 1. Maintains attentiveness to customers need; initiates steps that will comply with their requirements to meet and exceed their expectation. 2. Assesses the number and level of personnel needed to provide quality patient care in the unit during the shift. 3. Prepares staff assignment for the shift. 4. Adjust staffing and assignments appropriately. 5. Assess the health status of patients per nursing standards of the unit. Ensures that: a. A comprehensive history and physical examination is completed in each patient. b. assessment data are organized, accurate, complete, accessible and confidential. c. reports and records are assessed and interpreted. d. pertinent information are communicated among the members of the health team such as: i. Accurate identification of patient ii. Complete consent form iii. Vital signs and time taken iv. Laboratory / diagnostic results

v. Nurses progress notes 6. Plans nursing care as follows: a. Directs / supervise Staff Nurses in the formulation of nursing care plan. b. Ensures that salient patients problem are identified. c. Desired outcomes specific to the problems are formulated. d. Nursing care plan is documented. 7. Demonstrates knowledge and skill in the following: a. All unit specific nursing procedures b. Care and use of all unit specific equipment/ supplies. 8. Plans ways to solve problems and make improvements in the delivery of patient care. 9. Participates in the orientation and evaluation of new employees and other inservice training programs. 10. Supervises personnel in the implementation of Nursing standards and procedures of the unit. 11. Ensures that doctors order are carried out and implemented as prescribed. 12. Keeps the Unit Manager informed of needs, problems, incidents and other reportable situations during the shift. 13. Assist in the investigation of problems, irregularities and non- conformances. 14. Demonstrates effective infection control practices conscientiously (ex. Waste disposal, correct hand washing, use of personal protective equipment). 15. Perform nursing audit and patients rounds. a. Makes nursing rounds to assess quality of patient care and to identify problems and needs. b. Performs random chart audit to assure documentation of care plan during the shift. complete and accurate

16. Endorses all completed and contemplated procedures and all pertinent data of the all patient to the incoming shift and whenever he/she goes out of the unit. 17. Responds to and provide appropriate nursing action for emergent situations.

18. Demonstrates understanding of role in hospital emergencies, fire and disaster. 19. Conducts self professionally at all times. 20. Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, practices and processes. 21. Actively participates and assist in quality improvement activities. 22. Performs other task as assigned.

QUALIFICATIONS: 1. Registered Nurse in the Philippines 2. 1 to 2 years staff nurse experience 3. Completed Preceptorship Seminar and Charge Nurse Seminar

UNIT MANAGER JOB SUMMARY Provides leadership in assessing, planning, implementing, and evaluating patient care in specific unit/s assigned to him/ her during his/her shift. He/she is responsible in ensuring that agreed standards and all regulatory requirements are complied with. He/she manages all the assigned staff, equipments and material resources in the unit. Ensures that continuity of care with other shifts and teamwork are maintained and promoted all the time.

DUTIES AND RESPONSIBILITIES 1. Maintains attentiveness to customers need; initiates steps that will comply with their requirements to meet and exceed their expectation. 2. Assesses the number and level of personnel needed to provide quality patient care in the unit during the shift. 3. Ensures that: a. The health status of all patients is assessed as outline in the Standards of Nursing Practice in their respective unit/s.

b. The assessment data are organized so that they are accurate, complete, accessible and confidential. c. Records and reports are assessed and interpreted. d. Pertinent information are communicated among health team members, such as: accurate identification of patient, complete consent form, vital signs and time taken, pre-anesthesia evaluation, confirmation of surgical site, pre operative medication and time given, laboratory/ diagnostic results, last oral intake, elimination, admissions, transfer and discharge, laboratory results, significant change in patients health status. 4. Prepares bi-weekly schedule of staff. 5. Directs staff in the implementation of the standards of nursing practice, policies, procedures, guidelines, work instruction and protocols. 6. Demonstrates knowledge and skill in the following: a. all unit-specific nursing procedures b. care and use of all unit-specific equipments/ supplies 7. Plans ways to solve problems and make improvements in the delivery of patient care. 8. Participates in the orientation, continuing education programs and other in-service training programs of the staff. 9. Develops in-service training for the unit. 10. Participates in related quality management system and quality assurance activities of the hospital. 11. Leads in the development of continuous quality improvement activities. 12. Demonstrates effective infection control practices conscientiously ( ex. Waste disposal, correct hand washing, use of personal protective equipments. 13. Conducts a thorough investigation of problems, irregularities and nonconformances. 14. Analyze incident reports, corrective action report, customer complaints and other related reports and take appropriate actions. 15. Monitor compliance to a. hand hygiene practices

b. Physical examination schedules of all staff. 16. Performs nursing audit and patient rounds. a. Makes nursing rounds to assess quality of patient care and to identify problems and needs. b. Performs chart audit of all patients on a daily basis to ensure complete and accurate documentation of care plan. 17. Conducts regular staff meeting. 18. Ensures that the following are collated and updated a. PRC licenses of all times b. Professional Tax receipt c. Annual Membership of specific organization d. Training certificates. 19. Responds to and provide appropriate nursing action to emergent situations. 20. Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, practices and processes. 21. Actively participates and assist in quality improvement activities of the department. 22. Conducts regular performances appraisal of all nursing personnel. Recommends personnel for recognition/ promotion. 23. Accomplishes progress reports monthly reports as needed. 24. Acts as a member of the Quality Leaders of the hospital. 25. Assumes other function as delegated by the Department Manager or as maybe required of the job.

QUALIFICATIONS: 1. Registered Nurse with at least 9 units of Master Degree in Nursing 2. Have at least 5 years experiences in clinical nursing, inclusive of 1-2 years Nursing Administration Experience.

SHIFT MANAGER JOB SUMMARY: Responsible for overseeing all nursing activities during an assigned shift to ensure compliance to hospital standards and standards of nursing practice directed towards of safe and quality health care.

DUTIES AND RESPONSIBILITIES 1. Maintains attentiveness to customers needs; initiates steps that will comply with their requirements to meet and exceed their expectation. 2. Assesses the number and level of personnel needed to provide quality patient care in the unit during the shift 3. Oversees patient care activities in all nursing unit. 3.1. Makes nursing rounds to assess quality of patient care and to identify problems and needs. 3.2. Reports to appropriate authority observations/complaint. 3.3. Completely documents all relevant concerns (complaints, observation for further investigation and analysis). 3.4. Provides supervision as necessary in matters related to the following: 3.4.1. Execution of responsibilities of the Charge Nurse 3.4.2 Admission, Transfer and discharge. 3.4.3. Performance of complex nursing procedures. 3.4.4. Drug Administration 3.4.5. Intravenous Therapy Procedures. 3.4.6. Utilization of equipment 3.4.7 Emergency Code Maxicart 4. Serves as a role model of excellent customer service and patient care by: 4.1 Responds constructively to the needs of all those he/she interacts with on a daily basis.

4.2 Assesses and monitors customer sat5isfaction and responding promptly to voiced and identified concern. 4.3. Acts as a clinical resource/expert to all staff. 4.4 Acts to investigate and remedy patient complaints received within the staff, physicians, patient relations and members of the other departments or healthcare team. 4.5 assist in monitoring customer satisfaction. 4.6 Acts as a consultant to unit personnel, external customer, and/or individuals from other disciplines. 4.7 Serves as a resource to staff to improve/develop/expand knowledge base for patient care decisions. 5. Recommends and facilitates changes in personnel assignments as approved by the respective Department Managers. 5.3 Directs Charge Nurse on duty in the implementation of recommended actions. 6. Performs random chart audit to assure complete and accurate documentation of care plan. 6.1. Reviews all chart of patients schedule to undergo any surgical or invasive procedures. 7. Supports ongoing clinical practice. 7.1. Participates in research activities to explore, validate and direct delivery of care for specific patient populations. 7.2. Facilitates implementation of new clinical practice techniques among the nursing staff . 8. Collaborates with other departments as deemed necessary utilizing appropriate communication techniques and proper channel of communication. 8.1. Implements policies and procedures on admission and discharge of patient. 8.2 Discusses relevant matters regarding with the Admitting personnel.

9. In case of fire/ earthquake / disaster; 9.1 Maintains close liaison with other unit brigade managers. 9.2 Liaise with fire department service upon arrival. 9.3. If necessary; decide on transport and relocation of patients in close coordination with the floor brigade managers. 9.4Coordinates activities in the evacuation and communicate situation to the ED Officer. 10. Actively participates in divisional activities such as in-service training programs, supervisory meetings, timely submission of statistics. 11. Demonstrates effective infection control practices conscientiously ( ex. Waste disposal, correct hand washing, use of personal protective equipments). 12. Responds to and provide appropriate nursing action for emergent situations. 13. Demonstrates understanding of role in hospital emergencies, fire and disaster. 14. QUALIFICATION: 1. Registered Nurse with at least 9 units in Masters Degree in Clinical or Nursing Administration. 2. Have at least ten (10) years experience in administrative and clinical nursing.
