Download the registration form onto an A4 size paper (portrait) only. Please keep a copy of filled form for your record. Registration does not guarantee admission. Please specify the correct e-mail ID in the form as confirmation of form submission will directly go to your e-mail id. Applying under Free Seats belonging to (i) Weaker Section and (ii) Disadvantaged Group in the neighbourhood - The form is to be collected from the school office or downloaded from the Directorate of Education website www.edudel.nic.in.

The registration number of your application is PS/12/2525. PROCEDURE FOR THE SUBMISSION OF SUPPORTING DOCUMENTS FOR REGISTRATION 1. The Registration Form (with valid Registration Number), Acknowledgement Slip and Undertaking Form to be printed onto an A4 size paper (portrait) only. 2. Recent stamp size photographs of the child and of both the parents to be affixed. 3. Self-attested photocopies of childs Birth Certificate, Residence Proof and other required documents to be attached to the form. 4. The form to be signed by both the parents.

The completed form with required documents and a Registration Fee of Rs.25/- (in cash) to be submitted to the school office on any working days between 02-Jan-2012 to 16-Jan-2012 (9 AM to 11.30 AM). School office remains closed on Second Saturdays and all Sundays. The Acknowledgement Slip will be issued by the school office. 6. Any form without the requisite Registration Fee & supporting documents will not be accepted. Documents to be submitted (Self Attested) 1. Photocopy of birth certificate. 2.
