Physical four tier architecture can be further be bifurcated in Simple and High availability Architectures, applications and resources are installed on separate individual boxes. Simple Arch: Simple Architecture comprises of one to one mapping of resources and will not have any redundancy for failover or clustered environment for 24/7 support.
High availability Arch. : High Architecture comprises of redundancy in mapping of resources and will have failover or clustered environment for 24/7 support.
Physical four-tier with combined components means that one or more component of the arch are combined on one physical machines which exist in the architecture. Combined components tier architecture is designed to utilize the resources effectively by combining one or more components on single high end box. This design is good to go for the implementation having less number of users accessing the application with medium work load on the servers which shares resources on the same hardware. For example:

Production Environment Server landscaping and architecture for production environment proposed is High Availability, we have separate server which will host the entire database required for hosting the production arch as represented diagrammatically. Below Architecture is HA and does require redundancy at servers and storage level to avoid risk of any hardware failures.
