los glucocorticoides son hormonas naturales que previenen o suprimen las respuestas inmunes e inflamatorias cuando se administran en dosis farmacolgicas.
Acta como un agonista del cortisol.

Algunas de las respuestas de los glucocorticoides son la reduccin del edema y una supresin general de la respuesta inmunolgica. Los glucocorticoides inhalados disminuyen la sntesis de la IgE, aumentan el numero de receptores b adrenrgicos en los leucocitos y disminuyen la sntesis del cido araquidnico. En consecuencia, son eficaces en el tratamiento del asma bronquial crnico y las reacciones alrgicas.

Mecanismo de Accin

El receptor de glucocorticoides tiene ubicacin citoplasmtica y se encuentra en diversos tejidos. Es capaz de atravesar la membrana plasmtica por su alta liposolubilidad, para poder llegar a interactuar con el receptor. penetra en el ncleo donde ha de regular la expresin de los genes que responden especficamente a los glucocorticoides.

Estas respuestas son la inhibicin de la infiltracin leucocitaria en el lugar de la inflamacin, la interferencia con los mediadores de la inflamacin y la supresin de las respuestas inmunolgicas.

Farmacocintica

Administracin oral y parenteral. Rpida absorcin por el tracto gastrointestinal. Efectos pico a 1-2h de su administracin VO. Atraviesa la placenta y se distribuye por la leche materna. Metabolizacin heptica, excrecin urinaria. Biodisponibilidad: 90%, semivida plasmtica: 300 minutos, semivida biolgica: 36-54 hs, volumen de distribucin: 70 litros, unin a protenas: 66-77%.

Dosis individualizada con respuesta variable.

VO. 0.75-9 mg/d dividida en 2-4 dosis. Mantenimiento depende de la respuesta obtenida. IM y EV. 0.5-9 mg/d como dosis inicial.

Efectos txicos

Efectos txicos: A grandes dosis puede producir excitacin del SNC y hemorragia digestiva. En casos de intoxicacin grave est indicada la administracin de fenobarbital que disminuye la vida media de dexametasona en un 44%.

Conclusin

El MTZ frmaco muy efectivo como antibitico y antiparasitario, aunque como era de esperarse, la resistencia microbiana al frmaco - sobre todo en H Pylori, es cada vez mayor
