Objetivos
• Al terminar el ejercicio el estudiante estará en condiciones de:
• Comprender superficialmente la naturaleza del cáncer
• Identificar la importancia del cáncer del pulmón
• Entender la historia natural, investigaciones diagnósticas básicas,
estadificación, formulación de plan, intención de tratamiento de los
pacientes con NSCLC
• Conocer las supervivencias medianas, tasa de respuestas, supervivencias
libres de progresión con las diferentes estrategias de tratamiento –
basadas en evidencia.
• Tener conocimiento de las estrategias de tratamiento con quimioterapia en
NSCLC avanzado en primera línea, en segunda línea, y más allá.
• Conocer íntimamente los estudios más importantes que establecen los
avances en terapia antiangiogénica en cáncer del pulmón avanzado
• Conocer íntimamente los estudios más importantes que establecen los
avances en terapia contra el EGFR en cáncer del pulmón avanzado
Capacitación de NSCLC
Objetivos
• Conocer los estudios más importantes que establecen los avances en
terapia con Pemetrexed en cáncer de pulmón avanzado
• Conocer los aportes de los estudios Fase IV con Bevacizumab en
NSCLC
• Conocer los méritos relativos de las diferentes estrategias de
tratamiento – específicamente la terapia de mantenimiento con Erlotinib
en NSCLC
• Poder hacer un paralelismo y contraste entre Erlotinib y Gefitinib
• Poder argumentar con eficacia a favor de la estrategia de mantemiento
con erlotinib sobre mantenimiento con pemetrexed
• Argumentar con eficacia a favor de quimioterapia de segunda línea con
erlotinib y no con gefitinib, ni pemetrexed, ni docetaxel.
• Reconocer el impacto de la histología en el manejo farmacológico del
NSCLC
• Reconocer el impacto de las mutaciones del EGFR en el manejo
farmacológico, y pronóstico del NSCLC

Resección (lobectomía o pneumonectomía) en NSCLC estadíos I y II con
Adecuada reserva pulmonar (FEV1 > 800 mL, FVC > 2 Litros) y
Sin comorbilidades que hagan la cirugía prohibitivamente riesgosa.
La mortalidad por lobectomía es 3%
La mortalidad por pneumonectomía es 7%.
La radioterapia adyuvante ha demostrado incrementar la mortalidad.
Se deben seleccionar cuidadosamente los candidatos para radioterapia

• Estadíos IIIA y IIIB:
– La adición de quimioterapia a la radioterapia (quimiorradiación)
incrementa la sobrevida en NSCLC estadío III.

• Náuseas y vómito: Severos y comunes. Usualmente inicia
1 hora y se extiende por varias horas o días. Se pueden
disminuir estos efectos con el uso de medicamentos
antieméticos como la dexametasona, ondansetron
(granisetron, tropisetron) y lorazepam.
• Alteraciones electrolíticas: Pueden ser severas e incluyen:
hiponatremia, hipokalemia, hipomagnesemia, hipocalcemia y
pueden ser observadas varios días después del tratamiento.
• Anafilaxis: Se puede observar después de varias administraciones.
• Neuropatías periféricas: Ocurren con frecuencia y pueden ser
muy incapacitantes. Usualmente ocurren con dosis por encima de
300 mg/m2.
• Otros efectos: Hiperuricemia y disfunción autonómica.

– Supervivencia mediana 2-4 meses sin tratamiento.
– Se incrementa a 8-10 meses con terapia actual

• Extent of exposure to bevacizumab and the incidence of serious
AEs were similar for patients >65 and ≤65 years old as well as for
the overall population
• Risk of experiencing AEs associated with anti-VEGF therapy did
not appear to be increased for patients

• Incidence of grade ≥ 3 hypertension was similar in patients >65
years versus the overall population
• Median overall survival was similar for patients >65 and ≤65 years
