I. TRAQUEOSTOMA

DEFINICIN: Procedimiento quirrgico que crea una abertura en la

trquea a travs de una incisin en el cuello con la insercin de un tubo
o cnula para facilitar el paso del aire a los pulmones.

INDICACIONES
En las insuficiencias respiratorias agudas altas por:
Cuerpos extraos larngeos
Edemas de la laringe
Edemas de la base de la lengua
Epiglotitis
Estenosis larngea o subgltica
Malformaciones congnitas
Neoplasias larngeas
Parlisis de cuerdas vocales
Traumatismos larngeos
Difteria larngea y otras infecciones agudas
Traumatismos craneoenceflicos

TCNICA QUIRRGICA

Paciente en decbito dorsal y cuello

2cm above the
suprasternal notch
and
below
the
cricoid cartilage.

dejando indemne al menos 1 anillo traqueal por
debajo del cartlago cricoides.

Heavy scissors are used to create the tracheotomy if the tracheal rings are
thickly calcified

#15 blade scapel is used to create longitudinal tracheostomy. The laterally

placed traction sutures provide enough distraction to allow for
tracheostomy tube placement without excessive dilation. After confirming
tube placement the traction sutures are removed.

In the obese or a kyphotic neck, the distance from trachea to skin will
be increased. In this case a cricoid hook can be used to pull the trachea
anteriorly. Use of a longer tube is often necessary and fiberoptic bronchoscopy to
confirm proper endoluminal placement is recommended

Step 1: Immobilize the larynx and palpate the cricothyroid membrane - Stand at the
patient's right side if you are right-handed, or at the patient's left side if you
are left-handed. Immobilize the larynx with the nondominant hand and perform the
procedure with the dominant hand.
