Purpose
The lockout/tag out system will help to
prevent you and any one working on
equipment, is not electrocuted, hit,
crushed
otherwise
hurt
during
machinery service and repair.

Lockout/ Tag out are tools of your safety


The Lockout/ Tag out requires
to follow a specific set of steps
while working, servicing or
repairing equipment. If you
work on or around equipment
you required to be trained in
Lockout/ Tag out procedures.

Energy can be dangerous!


Any kind of energy can be a serious hazard when it comes
or released unexpected or suddenly.
Energy can be:
Electrical: when current runs through wire or cable.
Hydraulic: When liquid moves through pipes or
hoses
Pneumatic: In form of compressed gas or
pressurized steam and gas
Mechanical: Stored or built up energy in spring.

What is Lockout ?
Lockout means putting a lock on the part of machine that controls energy.
e. g. Switch, Circuit Breaker, Valve, Block, etc.

It involves:
Shutting off and locking out electricity
Releasing and locking out energy
Draining and locking out material

Locks need to be safe and strong


A lockout lock can be a key or combination type. But it can not be used
other than lock out purpose.

A Lock also has to be:


Durable: enough for the heat, cold,
humidity or corrosiveness in the area
where it is being used.
Standardized: by color, shape, size, etc.
through out the facilities.
Strong: so that it can not be removed
by heavy force or tools.
Identified: by the name of employee who
installs and removes it.

Who will perform the lockout ?


Management will select and trained the employees who will be authorized
to perform the lockout.

They will also learn:


How to recognize the types of energy
used by machine and equipment.
How to keep that energy controlled

Affected Employee :
Employees who work with machine and equipment
They need to understand:
Why lockout/tag out is important
How the procedure works
The importance of not attempting
to repair a machine without
lockout/tag out

Other Employee :
Employee who worked in a area where
lockout / tag out may be used

Steps of Safe Lockout


1. Prepare for shutdown: Know the types of energy, find out switches,
valve and other types of parts that control the energy and needs to be
locked
2. Notify affected employees: about Your
locking or tapping of equipment and why
3. Turn off the machine / equipment
4. Locate and Isolate all energy sources:
get rid on stored energy like, spring,
hydraulic, etc.
5. Lock out the switches and other energy
sources attached locks that hold
them in off or safe position

Steps of Safe Lockout


6. Test the operating control : Pull all the power in On position to
make sure that energy dose not go on.
7. Put operating control back on off or Safe position
8. Test the electrical part to sure that
equipment is de-energized.
9. Perform necessary services or
maintenance.

Remember: Check with your supervisor if


you are not clear.

What is tag out ?


Tag out means that warn of the danger of starting of the machine.
A tag should have printed warning about
what could happen if machine starts up.
Such as:
Dont start - worker could be injured
or
Dont not operate Serious injury could
occur

Remember: Tag alone will not prevent

Tag Requirements
Tag should be:
Durable
Strong
Standardized
Identity for authorized employee
Same print and format through out the facility
Easy read and understand
Tough enough not to removed easily

To attach a tag use nylon cable that:


Cant be reused
Can be attached by hand
Is self locking
Cant be released with less than 50 pond load

Use a team for big lockout


For big lockout use a team or group but few things to be
followed:
One member of the group shall be primarily
responsible
Make sure that all are safe during lockout/ tag
out
Each authorized group shall put his own lock
during the group lockout

Lock /Tag Removal


When maintenance or servicing is over, the lock/tag will be removed by
the same authorized employee who put it.
In absence of the same employee (during night shift, shift change, etc.),
Follow the company set systems.

The Lock/ tag removal procedure:


Make all employees are at safe distance from
equipment
Remove tools from machine
Re-install guards
Remove lockout devices
Turn on energy
Notify other employee that machine is working

Inspection of Lockout / tag out system


The periodic inspection of lockout / tag out system to be carried
pout to ensure that the system is working effectively.
During inspection an authorized
employee to check that all employee is
following lockout/tag out procedure.
