What is DAFAC?
DISASTER ASSISTANCE FAMILY ACCESS CARD

(DAFAC) is a card in duplicate form filled in with

basic information of the affected family.
One copy is issued to each family before or
during disaster operation and the other copy is
with the LSWDO for reference every time there is
distribution of relief goods or whenever there are
grievances.
DAFAC is instituted to give access to disaster
victims to avail of the assistance provided by
national and local government, NGOs and other
service providers thru the LSWDO.

Objectives of using DAFAC

To provide disaster victims access to

assistance during disaster relief operations

and post disaster operations and
rehabilitation
To have a systematic and speedy distribution
of relief assistance
To account all victims of disaster and to be
able to come up with a validated and
disaggregated data by age and gender
To avoid duplication of assistance and
services provided to disaster victims

Utilization of DAFAC
Part the Social Protection Services

(MFO2) mandate is the use of DAFAC.

occupation, income
If applicable- whether 4Ps beneficiary and/or IPs
ethnicity
The form also asks for the highest educational
attainment, occupational skills and health status
of each family member and under what specific
sector he/she belongs along with house
ownership/status and the housing condition
