Coronary Heart Disease: What is it?

A condition in which there is a build up of plaque inside the coronary arteries

Pathophysiology

Causes

CHD begins when certain factors damage the inside layers of the coronary
arteries, these factors include:

Smoking

High levels of fats and cholesterol in blood

High blood pressure

High levels of sugar in blood due to diabetes or insulin resistance

Blood vessel inflammation

Old age ( men at 45; women at 55)

What happens in Coronary Heart

Disease?

Plaque builds up in the coronary artery, especially with damaged inner layers.

Over time, plaque may:

Harden; or
Rupture

What happens in Coronary Heart

Disease?

Hardened Plaque

Narrows the lumen

Decreases blood and oxygen supply to the distal areas of the plaque

Ischemic part causes chest pain or discomfort (angina)

What happens in Coronary Heart

Disease?

Ruptured Plaque

The protective layer of the plaque ruptures releasing its contents

Platelets clump at the site of injury

Blood clot further blocks blood flow to the heart

Worsens angina and may lead to a heart attack when completely blocked

Signs and Symptoms

Chest pain radiating to shoulders, arms, neck, jaw, or back (angina)

Shortness of breath

Diagnosis

ECG

Stress Test

Chest X-ray

Blood tests

Coronary angiography

Assessment focus and management

Assessment focus

Pain related to decreased myocardial blood flow

Activity intolerance related to lack of oxygen supply

Anxiety related to threat to or change in health status
