Evidence: Use of Computer

Based Clinical Decision
Support System for Health
Professionals
The main protocols on which the functioning of the Internet is
dependent are listed below:
Transmission Control Protocol (TCP) allows computers
to connect to a network and exchange data; carries the task
of breaking messages into small packets (datagrams)

Internet Protocol (IP) considered a lower level protocol

which is responsible for making decision on the packets and
routing them.

Other protocols used on the Internet are the following:

Hypertext Transmission Protocol (HTTP) which
supports the World Wide Web (WWW)
File Transmission Protocol (FTP) permit users to send
all types of electronic files over the Internet
Internet
The Internet is aglobal system of interconnected
computer networks that use the standard Internet
protocol suite (TCP/IP) to serve several billion users
worldwide.

Worldwide network of interconnected computer

networks that operates using a standard set of
communications

The Internet carries an extensive range of

information resources and services, such as the
inter-linked hypertext documents of the World Wide
Web (WWW) and the infrastructure to support
email.
Electronic mail (e-mail)
1972- The first e-mail software appeared, that allowed users
to list, selectively read file, forward and respond to messages.
One of the most popular users for internet remains the ability
to send and receive e-mail. An e-mail address has two parts
separated by @ sign.

Use of Emoticons and Abbreviations

Emoticons or Smileys are small icons used to denote mood
using characters on a standard keyboard to form a picture.

File attachments
To send a file created by a word processor or other
application programs.

Newsgroups
a sort of worldwide bulletin board system that is accessed
using a software called newsreader.
Web addresses are also referred to as URLs. It can identify a
specific machine, directory and file of that machine
Example: https://www.facebook.com/
Domain Name identifies the computer name, web server
name, web name or host name.
The Internet Domain Name System (DNS) permit users to give
globally unique names to networks.
Top level domains:
.edu educational site (for a university or college)
.com commercial business site/computer
.gov government/non military site
.mil military sites or agencies
Modem (modulator-demodulator)
is a device that modulates an analog carrier signal to encode digital information, and
also demodulates such a carrier signal to decode the transmitted information.

Wi-Fi (Wireless Fidelity)

uses radio waves, allows access without wire connection.

Broadband
refers to the wide bandwidth characteristics of a transmission medium and its ability to
transport multiple signals and traffic types simultaneously. The medium can be coax,
optical fiber, twisted pair or wireless. In contrast, baseband describes a communication
system in which information is transported across a single channel.

Digital Subscriber Lines(DSL)

use sophisticated technology to pack data onto copper wires.

WiMAX(Worldwide Interoperability for Microwave Access)

is a WAN (wide area network)standard that is designed to operate in a cell-phone type
of network but provide relatively high-speed data access. Like WiFi, it is a networking
standard (actually several) thats designed to allow various devices to interoperate,
however unlikeWiFi its designed to operate over large distances outdoors to support a
service provider using a WiMAX network to serve many individual customers.

LTE (long-term evolution)

, marketed as 4G LTE
, is a standard for wireless communication of high-speed data for mobile phones and
data terminals
The Internet is a fast and efficient way of accessing
evidence to support nursing practice.
Diagnosis and Direct Care
If a patient has a problem or disease process with
which the nurse is unfamiliar it is just a quick trip to
the computer. Between in-house resources and the
internet any RN can have a quick overview of what
the problem is, where it comes from, what
symptoms it exhibits and what are the standard
forms of care.

Patient safety can also be attributed to information

accessible on the internet. If the nurse is doesn't
recognize an ordered drug prescription or feels
there is a problem with the dosage she can look it
up immediately. Well established websites like
WebMD.com provide current information about
clinical management and treatment of disease.
Nurses and the Internet
The ability to use the internet is an essential skill
for both nursing students and nurses, both to
support their studies, especially as nursing
students spend a lot of their time in placement
away from the university campus and to support
their development of skills in using what is
becoming an increasingly essential tools for
professionals.
Incorporating Evidence: Use of Computer Based Clinical
Decision Support System for Health Professionals

Decision support system (DSS) automated tools designed to

support decision-making activities and improve the decision-
making process and decision outcome
Clinical decision support system (CDSS) designed to support
healthcare providers in making decisions about the delivery
and management of patient care
Nursing decision support systems (NDSS) tools that help
nurses improve their effectiveness, identify appropriate
interventions, determine areas in need of policy or protocol
development, and support patient safety initiatives and
quality improvement activities.
Goals: patient safety and improved outcomes for specific
patient populations as well as compliance with clinical
guidelines, standards of practice, and regulatory requirements
-The computer can become a patient health medium with
reference databases, library access for healthcare
information, drug and disease management information, self-
help programs, and advice about prevention available.
Nursing needs to better understand the relationship between
patient autonomy and well-being. Dual purpose of
documentation

- Every nurse must understand the dual purpose of

documenting and capturing information in a automated
manner, first that of improving care for the individual patient
and second that of improving care for future populations of
patient via aggregated information used for clinical decision-
making.

- Current evidence indicates that CDSS can improve patient

care quality, reduce medication errors, minimize variances in
care, improve guideline compliance, and promote cost
savings.
