9. Please share your thoughts about
how Student Conduct can make the
General Incident Report form more user
friendly?
Text Response
When looking up the information for students, it should automatically input. At the least,
it would be nice if you didn't have to start the form over when looking up this info.
The date formatting should be rearranged.
Definition page for offenses
Be able to have the system automatically log student information after look up rather
then having to manually enter in all of the information.
Inputing student information even with the student look up option is more time
consuming than it should be. If I put in my AID info or ZID info there should be an auto
fill function with my personal information when submitting the report.
Clarity on natures of incidents for front line staff. (examples in the drop down menu or
ability to go and look up best nature without leaving the page).
The violated policies can have better descriptions. It can be hard to tell if it was
"disruptive behavior" or something else.
Some of the language is jargon-y (most likely to the field) and makes it difficult to know
what exactly I should be sharing, specifically in regard to identifying parties in the
incident.

13. Please share your thoughts about
how Student Conduct can make the
Sexual Misconduct/Title IX Incident
Report more user friendly

Text Response
The date section should be more uniform.
Detail the process
Defintion page
Perhaps offer more clarity regarding what constitutes sexual misconduct and/or a title IX
violation.
Further clarity of where it goes and what happens, especially when reported by faculty
and students.
Same as above, just think it could be less field-related jargon to make understanding
what information needs to go where a little easier.
Summary of Improvements to IR Forms

students and staff so people do not
have to look up all of this data only
to retype it.
Use less Student Conduct language
to make it more understandable and
relatable to everyone
Have descriptions for each type of
incident to help with classification
