A BRIEF OVERVIEW OF SOME INCIDENTS IN OUR
COMPANY
GANGWAY INCIDENT
GANGWAY FALLS PARTED
CLASSIC CASE OF POOR MAINTENANCE &
INSPECTION.

THE ROPE WAS LESS THAN 2 YRS OLD.

ON INSPECTION ABOUT 25% OF STRANDS WERE


FOUND HACKED AND REST 75% WAS FOUND TO BE
ELONGATED.
WHICH MEANS THAT 25% OF WIRE STRANDS WERE
CORRODED AND PARTED, REST 75 % COULDNT TAKE
THE LOAD AND ELONGATED AND FINALLY BROKE
APART.

A DULL EDGE ON A BROKEN WIRE STRAND INDICATES


THAT THE WIRE HAS PARTED DUE TO POOR CONDITION
(PROBABLY DUE TO CORROSION) AT A LOAD LESSER THAN
ITS BREAKING STRENGTH ,WHEREAS A SHARP EDGE
INDICATES THAT THE WIRE HAS ELONGATED AND PARTED
WHEN ITS BREAKING LOAD HAD EXCEEDED.

LUCKILY NO ONE WAS PRESENT ON THE
GANGWAY.

THIS INCIDENT TELLS US HOW


IMPORTANT IT IS TO CLOSELY INSPECT
AND MAINTAIN ANY KIND OF WIRES
FIRE BY FUNNEL SPARKING
FUNNEL SPARKING
Usually after every EGE wash the Sparks are
seen coming out of the funnel. These sparks
generally extinguish before falling on the deck
but sometimes they do not.

The EGE wash was not informed to the Deck


department.
FIRE BY FUNNEL SPARKING cont
New Spares received were left on deck and
covered by Plastic to protect against rain.
This plastic caught fire by the falling sparks
and damaged the Spares.
This cost the company $15,000 and also delay
in maintenance.
Cause: LACK OF COMMUNICATION.
IMPORTANCE OF TOOL BOX MEETING.
Tool Box meetings are not possible to be held in
port. Any work planned for port MUST be
discussed & informed prior arrival port.
FIRE BY FUNNEL SPARKING cont
BURN INJURY TO OILER FROM
HOT FUEL OIL
Any pressure vessel must be opened only after
ensuring it has been completely depressurized,
without any doubt.
Opened main engine HFO filter on a running ship
without properly bypassing the filter. The pressure
inside was not released thru the vent cock.
The cock hole was chocked, as such he THOUGHT
pressure has been released.
Moreover, during Tool Box meeting the person was
specifically informed that this job To be done in port
ONLY after FWE.
BURN INJURY TO OILER FROM HOT FUEL
OIL

As hot oil was gushing out, the FO pump had
to be stopped momentarily.
The Person not only endangered himself but
also the vessel, as the vessel was in heavy
traffic zone (approaching Singapore), the main
engine power was lost momentarily.

Cause : Wanted to go ashore rather than work


in port
