Electrical & Grounding audit SEP 2017
AREA OF IMPROVEMENT
Following Point need to be address ASAP
1. Implement SECMC Checklist of Daily ELCB Checklist properly for all Electrical appliances.
2. Complaint register should be provided by admin or camp management with signage out site for immediate repair of any damaged or
faulty things.
3. Emergency rout map must be displayed at public places and in the mess hall specially including muster point location.
4. Implement of Monthly Equipment Check List and Equipment inspection Tag system should be adopted for all Equipments ,
Electrical Connection Boards, Electrical Panels or JBs, refrigerator ,Record should be maintain by camp management & contractor.
5. All main holes, gutters must be covered & drained properly not chocked at all.
6. Ensure Earthling resistance reading as per standard of static charge Pit
7. Ensure replacement of smoke silencer of main Diesel generator
8. All damaged light need to be replace and remove the naked wiring.
9. Tagging and identification mark of all JBs Breaker should be install & volt , ampere meters must be operational.
10. All contractor tools , remain material & scrape must be removed by contractor on the completion of job, he is responsible for
ensuring area safe to residence of work.
11. Need to be replace Two core wiring and cables with three core standard cables & must be buried all cables as per company
standard with rout markers.
12. Ensure proper housekeeping on daily basis of whole area specially Kitchen , surrounding of residential area, also bush cutting on
priority basis periodically when required.
13. Make a log book checklist of generator out put load.
14. Oil storage area should be away from with secondary containment & Fire Extinguisher must be kept there with all Equipment and
oil or diesel storage area.
15. Police canteen area must be provided proper toilet & washing facility .
16. All roatary parts of machinary must be guarded and dont removed safe guard from Grinders in any case or condition.
17. Always standard copper rod and copper earthing cable should be used for proper earthing of machinery , equipments and
containers or porta cabins for offices or residence.
Audit observations on 20 Sep 2017
1) Cover of refrigerator in TSF 2 mess was removed due to damaged
2) A door closer was found in damaged condition of room # 3) Two Lights cover were found in damage condition.
3)No Emergency Exit signages were provided in mess Hall and TV Hall at all
4) Un necessary combustible thinner & material was stored in TV hall & mess hall, which may cause serious fire in TV hall.
5) Smoke Detectors of all A series room and0 B Series rooms were found inactivated condition Which were activated on spot with
the help of camp Boss of
TSF 2, Also smoke detectors of TV room & Mess hall one smoke detector was found inactivated which were activated later.
6) Smoking shelter was provided but one desk was found in damaged condition & smoking tray was missing
7) Main Electrical DP for TSF2 residential area of SECMC was found unlocked, without passing voltages & without any shield in
circuit for preventing direct contact accidently.
8) All available 13 residential porta cabins was provided proper earthing but we cant seen any Earthing pit in whole area and no one
knows about the earthing pit, also earth test readings come more then required limits earth resistance.
9) It is observed In all rooms observed that cable TV device stand was not provided it may cause of proper damage, means this device
may damaged the LED TV
10) The Pipe hoses and Electrical cables were observed in the access of Masjid may trip some someone & it may cause of fire and
electrocution due to damaged the un buried electrical cables, also poor house keeping was observed in this area.
11) In second snap remain bricks and construction wastage material was dumped in north side of TSF 2 beside tower room
12) Workers of police canteen was preparation the vegetable for lunch time. While the were not wearing mandatory and required
PPEs, in dirty area in second snap worker can observed in chapel & half dress its very hygienic condition for Police health.
13) Gutter cover was missing during audit in the front of TSF2 residence of SECMC & poor house keeping of area.
14) SEMS construction team was left remain bricks and their construction wastage beside new constructed access of vehicles also
leveling required of whole area.
15) South wall of TSF2 some tents and SEMS tools and scrape was scattered in whole area, sems workers were resting in night time in
these tents w/o any approval , w/o illumination of area while too much bushes were found beside, may cause snake bite cases in this
condition
16) They dont have first aid box & Snake catcher with them in this area. Used cover alls helmets, shoes & other scrape was found in
this area.
17) Poor house keeping was observed in surrounding of police canteen area, some damaged cots wooden scrape broken furniture was
found during audit of this area. While they were cutting and making food for police worker in this area.
18) Available Electrical board was found w/o ELCB and two core cable was used for connections w/o double insulation while as per
company procedure double insulated three core cables should be used for all electrical boards with standard ELCB Earth leakage
circuit breaker for avoiding electric shock to user.
20) Very unhygienic condition of Dishes washing area un covered cooking pots in this area

21) Cover of refrigerator in TSF 2 mess was removed due to damaged
22) Highly unhygienic condition was observed of police canteen tea kitchen area. Gas cylinder was placed very closed to the stove which
is the unsafe & may results serious consequences in the shape of fire incident or explosion in area.
23) Gutter water was accumulated in between the residential rooms containers may cause of generation of mosquitoes and different
disease to residents of TSF2 and surroundings
24) Knives were left on the cooking table while these should be stored properly in knives stand
25) Dirty cloths and empty cartons were found in SEMS kitchen of TSF2 while these should be disposed off timely
26) Water filter was found disconnected in bellow picture while water was being used by kitchen staff for cooking purpose.
27) Uncovered waste bins was found in TSF2 SEMS kitchen Its very unhygienic condition & water bottles were used as table for
cooking pots.
28) Fire Extinguisher was not provided for fuel gas cylinders beside the Kitchen, Later they provided when we highlights this point. No
smoking signage already provided.
29) Plants empty pots, contractor boxes, wheel barrows, other scrape was scattered beside the south wall & the back of containers with
west wall this garbage may increasing the snake bite cases in site office area & snakes can hide easily in this scrape & bushes.
30) Poor housekeeping was observed in construction area of SEMS at TSF2, tile fixer was used baby grinder w/o safe guard
immediately stop him
31) DTI training teams training material was scattered in area w/o any barrication or ownership signage
32) Due to blockage of Sewerage pipe produce hygienic condition behind the police residence & KT office , Poor housekeeping on the
water tank of RO station at site office some tripping hazards was created due to poor house keeping

Some Definitions of Electricity
Definitions:
A physical agency caused by the motion of electrons, protons, and other charged
particles, manifesting itself as an attraction, repulsion, magnetic, luminous, and
heating effects, etc.

CONDUCTOR: contains many free electrons --- gold, copper, silver, aluminum

INSULATOR: contains few free electrons-Usually non-metallic such as wood, rubber,


glass, etc

VOLTAGE : unit of measurement of electromotive force (EMF)

CURRENT : Continuous movement of electrons past a given point. (measured in


amperes)

RESISTANCE: Opposition to movement of electrons. Makes it possible to generate


heat, control current flow, & supply correct voltage to devices

Some most common hazards of Electricity
Hazards:-
SHOCK:-
Electric shock occurs when the human body becomes part of the path through which current
flows. The direct result can be electrocution. The indirect result can be injury resulting from a
fall or movement into machinery because of a shock
BURNS :-
Burns can result when a person touches electrical wiring or equipment that is energized.
ARC-BLAST :-
Arc-blasts occur from high- amperage currents arcing through the air. This can be caused by
accidental contact with energized components or equipment failure.
FIRES :-
Electricity is one of the most common causes of fires both in the home and in the workplace.
Defective or misused electrical equipment is a major cause.

oMore than 10 mA- Muscle contraction No Let Go danger

Electrical equipment shall be free from recognized hazards that are likely to
cause death or serious physical harm to employees

Three core cables must be used at all.


All cables must buried proper as per company standard after testing & inspection
from company approved competent person
All cables must be provided standard PVC conduit or flexible conduit for protection of
weather or damages.

Grounding
Definition:-
The path to ground from circuits, equipment, and enclosures shall be permanent and continuous
Types of Grounding:-
There are two kinds of grounding:
1. Electrical circuit or system grounding
One conductor of the circuit is intentionally grounded to earth
Protects circuit from lightning, or other high voltage contact
2. Electrical equipment grounding
All metal frames & enclosures of equipment are grounded by a permanent connection
or bond
The equipment grounding conductor provides a path for dangerous fault current
to return to the system ground at the supply source should a fault occur
Exposed non-current carrying metal parts of cord and plug connected
equipment which may become energized shall be grounded
If in a hazardous location
If operated at over 150 volts
Earth Resistance Value
Ideally a ground should be of zero ohms resistance. There is not one standard ground
resistance threshold that is recognized by all agencies. However, the NFPA and IEEE have
recommended a ground resistance value of 5.0 ohms or less.

1) Cover of refrigerator in TSF 2 mess was removed due to damaged

Live parts of electric equipment operating at 50 volts or more guarded against accidental
contact by approved cabinets

1) A mixture machine was left in this TSF2 area w/o barrication while
there work was finished couple of week before.
