learning needs of patient and family
Assessing
1.Assess the patient,s learning
needs,his physical,emotional,and
experiential readiness for health
education.

behavior
b.what psychosocial adaptation is he
making
c.Is he ready to learn?
1.is he able to learn these behavior?
2.what are his expectation ?
3.what additional information is
needed about him
2.Assess the patients need for
education and preparation related to
the self care activities for which
he will be responsible after discharge.

to fasilitate data collection.

agencies for reinforcement of
learning after discharge.
