HOW TO USE

MICROSCOPE IN
VITREORETINAL
SURGERY
A. M. Ichsan
Operating
Microscope
□ For the retinal surgeon it
is important that several
functions are integrated
into the foot pedal, for
example, a switch for
choosing the microscope
light or an inverter, if
needed
A modern binocular surgical (stereo-) micro- scope
required :
 Co-axial illumination that should allow a
magnification of 10–30-fold.
 Motorized power zoom, power focusing and X–
Y-positioning via foot pedals.
 Fitted with the corresponding laser filters to
permit photocoagulation.
 A light-splitter is necessary for co-visualization
of the operating personnel and for the
integration of a video system.
Wide-angle
viewing systems
(WAVs)
 Contact : LANDERS lens system Need fixation
to eyeball, Need assistance

 Non-contact wide-angle systems (BIOM, EIBOS,

RESIGHT) are widely in use and can be
managed by the surgeon alone.
- They offer a greater depth of field and
better visualization through media
opacities.
- Lower incidence of postoperative
epithelial defects

Binocular Indirect
Ophthalmo-Microscope
(BIOM System)

□ Plano-concave contact lens which is

directly placed onto the cornea, or a
highly refractive lens (60D, 90D, 120D)
which is placed in front of the lens of
the surgical microscope comparable to
indirect ophthalmoscopes.
□ Results in an inverted image.
□ Upright image is created (e.g., using the
“stereo diagonal inverter (SDI)” of
Oculus’ new lightweight BIOM 5c Oculus)
features surgeon-controlled electronic
focusing and provides automatic image
inversion of the SDI 4c.
EIBOS (Erected
Image Binocular
Microscope) □ EIBOS system is extremely
robust and has the additional
advantage of an inbuilt
inverter that avoids the need
for manual inversion when
changing from the posterior
segment to the anterior
segment view during the
surgery
Field of View: 100.125 degrees
Lens: 90.132 D
