SEVEN BASIC CHARACTERISTICS OF GOOD REPORT PERTINENT ACCURATE CLEARLY PHRASED UNBIASED COMPLETE CONCISE TIMELY THE DIFFERENCE BETWEEN REPORTING AND INTERPRETATION Objective/factual reporting Subjective/interpretive reporting To report well, the competent and honest observer generally need not concern Himself interpreting what he observes. Difference between fact and interpretation. NOTE TAKING, Use one you find workable Develop own system of abbreviations as in dates, etc. Jot down facts only- get only the bones of the skeleton. Omit articles, connectives- write key words of the information. Write later the details when they are still fresh in your mind. THE ORGANIZATIONS AND EXPRESSION OF A REPORT. HEADING COLLECTION AND SOURCE DATA THE INFORMATION PART THE INTERPRETATIVE COMMENTS. THE TEST OF A GOOD REPORT. All of the who, what, when, where, why and how have been included in the collection data and information section of the report. It gives a clear account of how the information was obtained It contains no unsupported generalizations. It contains no inexactness of expression. It contains no inaccuracies of observation. It is repetitive. It contains only information of significance of the assignment. THE FOUR (4) “C” TEST OF A GOOD REPORT. 1. CLEAR Is the report clear? Will the reader understand what the situation is or what happened? (who? What? Where? When? Why? How?) Is the report limited to one main subject? Is the report written in terms the reader knows? Is the information of interest to the reader? Are the important ideas emphasized? Do the points “hang together”? Are the recommendations and conclusions, if any, clear stated? Is the report written in “talking language”? CONCISE. Is the report as brief as possible? Can the report be read rapidly? Are meaningless phrases left out? Are unnecessary words eliminated? Are the sentences short and simple? Does the get to the point quickly? Are over- worked expression avoided? A good report is like a girl’s bathing suit. It must not be too brief to be scandalously cheap and not too long to be uninteresting. CORRECT Is the grammar correct? All words spelled correctly? Are the facts and figures correct? Correct punctuations used? Are all words used correctly? Are contractions and abbreviations avoided? COMPLETE Is the report dated/signed/classified? Is it clear to whom the report is to be sent? Does the report present all the information which the reader needs? Is it clear whether a reply is expected? Does you report have a human touch? Information report format is divided into four (4) principal parts: the heading, body, the comments portion and the signature. HEADING
The heading contains the reporting
unit, date of report, the subject, and date of information. The date acquired, the place acquired, the report number, the reference, the evaluation, the attachments (s) and the source (s) 1.Reporting unit-indicate here the branch/Group or Station submitting the standard abbreviations for the designated office may be used here. 2. Date of report- refers to the date when the information report is accomplished by the originating personnel. 3. Subject- contains as succinctly as possible the name or description of the matter (s) reported upon. 4. Date of information- give her the date (s) when each particular information was obtained by the reporting officer. 5. Date acquired- place here the date (s) when each item of information was obtained by the reporting officer. 6. Place acquired- give here the place (s) where each item of information was obtained by the reporting officer. 7. Report number- inserts here the number of the report. These reports well be numbered from one and so on which will be preceded by the project symbol and the letter I (demoting information); e.g. CA/I/C42 will indicate information report number 42 of project “Cobra” (Code Symbol CA). 8. References (s)- state here any pertinent reference, whether to previously relate information reports and or reference material and/or operational reports from which the information was taken. 9. evaluation- state here the degree of reliability of the information reported, both as to source and as to contents by use of evaluation codes (letters and numbers). 10. Attachment (s) - enumerate here the different annexes appended and pertinent to the information report. 11. Source (s) - describe the source to assist the reader in arriving at an evaluation of the report, while at the same time, for security reason, protecting the source’s identity. Body
This will contain a narrative
discussion of the subject (s) of the information report. This should cover the important points of the report proper. Always try to make the report specific (concise), objective, complete, clear and accurate. No inclusions, opinions, analysis and the like should be included here. COMMENTS
This should include the
objective views of the reporting officer and /or P/A and/or fieled sources as to the significant of the information submitted. SIGNATURE

Subject was observed using phone booths near Old Tiongsan Bazaar along Magsaysay Avenue at about 1810H and a note suspected to have been placed by subject on top of the phone booth was recovered. The note contained a written message which says: Attend meeting April 4. Agenda Strategy Campaign Autonomy Act. Further info indicate that subject met a male companion at Dainty restaurant whom the talked for about an hour then parted ways. COMMENT (S): 1. First message maybe presumed to be referring to the second message mentioning about a meeting on April 4 about strategy to be adopted for the Autonomy Net Information Campaign.
