Importancia de infeccion por CMV
En ausencia de profilaxis la enfermedad por CMV ocurre en 20-
30% de los receptores de TR, principalmente alrededor de los 90
dias post trasplante.

Control inmunologico
La respuesta inmune adaptativa de los linfocitos T y B juegan un
papel critico en el control de replicacion de cmv.
Celulas B: producen ac que neutralizan la glicoproteina b,
glicoproteina h.
Celulas T cd4+ y cd8+, celulas t reguladoras. Se dirigen contra
antigenos como pp65, pp50, ie-1, gb. Expresión de citocinas
como ifn-y, tnf-a, il-2, controlan la replicación de cmv.

En inmunocompetentes la enfermedad
generalmente es asintomatica.
En receptores de SOT se puede requerir el
ajuste de dosis de inmunosupresion
El syndrome viral agudo puede remitir sin la
administracion de antivirales
• Ventajas:
– Eficacia comprobada
– Reduce los efectos indirectos de infeccion por CMV
– Facil de coordinar
• Desventajas:
– Mayor costo y riesgo de toxicidad
– Resistencia
– Enfermedad por cmv de inicio tardio
– SE RELACIONA CON ESTATUS D+/R-, PERIODOS DE PROFILAXIS MAS
CORTOS (36%vs16% EN 1ER AÑO POST TR EN PROFILAXIS 100 dias vs 200
dias).

• Ventajas:
– Minimiza exposicion al farmaco
– Reduce costo y toxicidad
– Menor riesgo de Resistencia
– Mejor incidencia de enfermedad tardia por CMV
• Desventajas:
– Mayor dificultad logistica
– progresion rapida de la enfermedad en estatus d+/r-
– no previene los efectos indirectos de infeccion por cmv
incluyendo los que afectan el injerto y supervivencia del
paciente
SE HA DEMOSTRADO QUE LA INCIDENCIA DE ENFERMEDAD POR CMV SE REDUCE
DE MANERA SIGNIFICATIVA (HASTA 70%) CON AMBOS METODOS.
