What kind of occurrence to be
reported ?
• Detail of occurrence that may not be captured
by the mandatory reporting system (CASR 19)
• Other safety related information which is
perceived by the reporter as an actual or
potential hazard to aviation safety.

You wish for other to learn and benefit from
the occurrence or hazard report, but are
concerned about protecting your identity

• When conducting a VOR approach into SUB on RWY 28,
there is a real possibility of lining up on the southern
taxiway (SP2) instead of RWY 28. In hazy conditions, from
far you can mistake the taxiway for the runway as it
appears brighter than the runway. Also, the offset from the
VOR to the runway is only 3 degrees and couple with the
PAPI being unserviceable on RWY 28 will even mislead the
crew even more. Regarding possibility of mistaking taxiway
for runway, do not disengaged the autopilot too early
during approach until you have positively identified the
runway in use.

• I was the Approach Controller and cleared
Aircraft X for a visual approach to Runway 17
and switched them to the Tower frequency.
Aircraft X began to maneuver and appeared to
be attempting to land at [a nearby airport],
which lies 12 miles southeast of [the intended
destination]. I called the Tower and the Tower
had Aircraft X on frequency. The Tower issued
a heading to Aircraft X to land at the correct
airport. (hazy condition)

Mechanic
• I was the Lead Mechanic for a propeller build-up during
which #2 and #4 Blades were swapped. When the prop was
finally put on a plane a month later, the airplane
experienced excessive vibration. This is when the prop was
inspected and found [to have] blades…installed in the
wrong locations.
I think the blades were installed improperly because too
many people were involved in the build-up. Blades #1 and
#3 were installed first, so I think we just got confused as to
which side #2 went on. Inevitably after installing #2
incorrectly, then #4 would also be incorrect. I think we also
failed to double-check our work like we did when installing
#1 Blade.

Ground Handling
• I received two large suitcase at the gate. I sent
the bag down to the first floor on a
dumbwaiter. I left the gate and entered the
restricted zone, then took the first bag from
the dumbwaiter and put it behind me. When I
reached to get the second bag, the suitcase I
had put behind me began to roll toward an
area for vehicular traffic. An employee who
happened to be there noticed the suitcase
and stopped it, but it was a near miss.
