The purpose of Patient Safety and Quality Care is essentially to

improve health care quality by ensuring patient safety and
improving the quality of interventions and strategies.3
  Patient Safety as defined by the IOM is “the
prevention of harm” to the patent.1
 This includes the need to “promote health and
prevent illness.”1
Definition of
Patient Safety and  Quality Improvement was defined by the IOM as
Quality Care “the degree to which health services for

likelihood of desired health outcomes and are
consistent with current professional knowledge.”2
  Recommendations to improve Patient Safety and
Quality Care:4
 1. Safe – which essentially means to avoid
harm.
 2. Effective – base services on scientific
knowledge that are beneficial.
 3. Patient-Centered – or providing care that is

 4. Timely – reducing delays for care.

 6. Equitable – or providing care that doesn’t

  Latent failure – errors that relate to the
policies and resources rather than the
health care professional.
 Active failures – are errors that have to do
with the contact with the patient
Types of Safety  Organizational system failure – involves

“improve the quality of health care through patient safety
interventions and strategies.”3

PSQCM to the the things can lead to errors.
Student  It also helps students understand that
they can learn from errors and improve
their practice to prevent further errors
from occurring.
 Improvement to safety and quality care is important in
improving outcomes in health care.
IOM recommendations are to create safe, effective,
patient-centered, timely, efficient and equitable care.4
Conclusion A culture of safety is important to both nurses and student
nurses.
