SCENARIO 2
Click

A graduate student in the Faculty of Dentistry last year came to the dentist
with complains of sound and pain when opening the jaw. Extra oral
examination of the right cheek looks bigger than the left cheek. It is difficult
to open your mouth and when you close your jaw, click on the left jaw joint.
An-intraoral examination was seen of the posterior upper right jaw of the
extrusion, and the missing posterior teeth were missing. Radiographic
examination shows abnormalities in TMJ. The dentist explains that this occurs
due to tooth loss so the occlusion is not normal when closing the jaw or jaw
in a resting state. The doctor and educated the students than to find out
the TMJ abnormalities that occurred, the patient was referred to the
radiology department to do an X-ray examination on his TMJ.

LEARNING OBJECT
1. Students are able to understand and explain the patient's condition in the

scenario

2. Students are able to understand and explain the etiology of the case in the

scenario

3. Students are able to understand and explain the mechanism of clicking

4. Students are able to understand and explain how to communicate, inform

and educate patients about TMJ disorders

5. Students are able to understand and explain the radiographic examination of

TMJ

6. Students are able to understand and explain the various abnormalities in TMJ

7. Students are able to understand and explain the diagnosis on TMD

8. Students are able to understand and explain the TMD treatment plan

9. Students are able to understand and explain TMD examinations

1. Students are able to understand and
explain the patient's condition in the
scenario

 the patient's right cheek looks bigger than the

left cheek
 then the right upper jaw posterior extrusion
tooth.
 the posterior mandibular missing teeth can
cause the patient's face to look asymmetrical
(looks not the same on each side).
2. Students are able to understand and
explain the etiology of the case in the
scenario

 Macro trauma
 Micro trauma
 Occlusion conditions
 Emotional stress
 Deep pain input
3. Students are able to understand and
explain the mechanism of clicking
 This sound is heard when the condyle moves from
the intermediate zone of the disc to the posterior
border.
4. Students are able to understand and
explain how to communicate, inform and
educate patients about TMJ disorders
5. Students are able to understand and
explain the radiographic examination
of TMJ
To get a radiographic picture can be done in
several shooting techniques, namely:
 transcranial
 transfaringeal
 panoramic
 tomography
 computed tomography (CT)
6. Students are able to understand and
explain the various abnormalities in TMJ
 joint sound
 feeling tired / tired of the muscle wasting
 limitations in opening the mouth
 tooth wear
 facial pain
 headache
 and even disorders of the ear
7. Students are able to understand and
explain the diagnosis on TMD

 Diagnosis of TMJ cases is often stated simply as a

disorder of the jaw joint which is characterized by
joint sounds or mild crepitus.

 More specific diagnoses are expressed in the form

of muscle pain, open-mouth muscle spasms,
arthritis, shifts or disc damage.
8. Students are able to understand and
explain the TMD treatment plan

a. Examination and Diagnosis

b. Active care : Treatment of symptoms,
calm the patient, resting the jaw,
provision of medicines, exercise, heat
therapy
c. Temporomandibular Joint Surgery
9. Students are able to understand and
explain TMD examinations

 Inspection (Bilateral)

 Palpation (Bilateral)

 TMJ Sounds.

 Range of Motion of Mandible.
