Bloodborne Pathogen Exposure Requirements


This regulation only applies to employees who have the potential for occupational exposure to blood
borne pathogens
• Establish a written Exposure Control Plan designed to eliminate or minimize employee exposure.
• Periodically reevaluate this policy
• Train employees how to protect themselves from health hazards associated with blood borne
pathogens, training includes the following:
– An explanation of the modes of transmission of blood borne pathogens
– An explanation of the employer’s exposure control plan and the means by which the employee can obtain a
copy of the written plan
– An explanation of the appropriate methods for recognizing tasks and other activities that may involve
exposure to blood and other potentially infectious materials
– An explanation of the use and limitations of methods that will prevent or reduce exposure (i.e. engineering
controls, work practices, and personal protective equipment)
– Information on the types, proper use, location, removal, handling, decontamination and disposal of personal
protective equipment and the basis for PPE selection
– Information on the appropriate actions to take and persons to contact in an emergency involving blood or
other potentially infectious materials
– An explanation of the procedures to follow if an exposure incident occurs, including the method of reporting
the incident and follow-up that will be made available medical follow-up that will be made available
– Information on the hepatitis B vaccine, including information on its efficacy, safety, method of
administration, benefits of being vaccinated, and that the vaccine and vaccination will be offered free of
charge
– An explanation of the signs and labels and/or color coding
– An opportunity for interactive questions and answers with the person conducting the training session

Bloodborne Pathogen Exposure


Requirements (cont.)
• Maintain sharps injury log
• Maintain training records
• Affix warning labels to containers of regulated waste, refrigerators and freezers containing
blood or other potentially infectious material
• Access doors to the work area or containment module shall be self-closing.
• An autoclave for decontamination of regulated waste shall be available within or as near as
possible to the work area.
• All regulated waste shall either be incinerated or decontaminated by a method such as
autoclaving known to effectively destroy bloodborne pathogens.
• A ducted exhaust-air ventilation system shall be provided.
• Engineering controls shall be examined and maintained or replaced on a regular schedule to
ensure their effectiveness.
• Establish and maintain an accurate list of employees/job classifications with occupational
exposure
• Food and drink shall not be kept in refrigerators, freezers, shelves, cabinets or on countertops
or benchtops where blood or other potentially infectious materials are present.
• Provide, at no cost to the employee, appropriate PPE such as, but not limited to, gloves, gowns,
laboratory coats, face shields or masks and eye protection, and mouthpieces, resuscitation
bags, pocket masks, or other ventilation devices.

Hazardous Waste Emergency


Response Requirements
• Train employees how to respond to emergency situations prior to employers being called in real
emergencies (varying degrees of training based on response level)
• Develop and implement a written safety and health program for employees involved in
hazardous waste operations that shall be available for inspection by employees, their
representatives and OSHA personnel.
• The program shall be designed to include:
– Pre-emergency planning and coordination with outside parties.
– Personnel roles, lines of authority, training, and communication
– Emergency recognition and prevention.
– Safe distances and places of refuge.
– Site security and control
– Evacuation routes and procedures
– Decontamination procedures
– Emergency medical treatment and first aid.
– Emergency alerting and response procedures.
– Critique of response and follow-up.
– PPE and emergency equipment.
– and to address as appropriate site analysis, engineering controls, maximum exposure limits, hazardous
waste handling procedures and uses of new technologies

• The emergency response plan shall be rehearsed regularly as part of the overall training
program for site operations.

Hazard Communication Requirements


• Develop, implement, and maintain at each workplace, a written hazard communication program.
• Assess the hazards of chemicals which they produce or import, and all employers to provide information to
their employees about the hazardous chemicals to which they are exposed, by means of a hazard
communication program, labels and other forms of warning, material safety data sheets, and information and
training.
• Provide employees with effective information and training on hazardous chemicals in their work area at the
time of their initial assignment, and whenever a new physical or health hazard the employees have not
previously been trained about is introduced into their work area. Training must cover the following topics:
– How the hazard communication program is implemented in that workplace, how to read and interpret
information on labels and the MSDS, and how english/non-english-speaking employees can obtain and use
the available hazard information
– The hazards of the chemicals in the work area. (Hazards may be discussed by individual chemical or by
hazard categories such as flammability
– Engineering controls, work practices, and the use of PPE
– Methods and observation- i.e. visual appearance or smell- workers can use to detect the presence of a
hazardous chemical to which they may be exposed.
• Ensure that labels on incoming/outgoing containers of hazardous chemicals are not removed or defaced;
• Maintain any material safety data sheets that are received with incoming shipments of hazardous chemicals,
and ensure that they are readily accessible during each work shift to laboratory employees when they are in
their work areas;
• Maintain a list of the hazardous chemicals known to be present using an identity that is referenced on the
appropriate material safety data sheet.

Hazard Communication
Requirements (cont.)

• Identify/document in HazCom Program methods to be used to inform the other employer(s) of


any precautionary measures that need to be taken to protect employees during the workplace's
normal operating conditions and in foreseeable emergencies;
• Provide appropriate hazard warnings, or alternatively, words, pictures, symbols, or combination
thereof, which provide at least general information regarding the hazards of the chemicals,
and which, in conjunction with the other information immediately available to employees under
the hazard communication program, will provide employees with the specific information
regarding the physical and health hazards of the hazardous chemical.
• If the site becomes newly aware of any significant information regarding the hazards of a
chemical, the labels for the chemical shall be revised within three months of becoming aware
of the new information
• Identify/document in HazCom Program the method to be used to inform employees of the
hazards of non-routine tasks (for example, the cleaning of reactor vessels), and the hazards
associated with chemicals contained in unlabeled pipes in their work areas
• Ensure that laboratory employees (not applicable to Occupational Exposure to Chemicals in the
Laboratory) are provided information and training

Signs and Tags, Specifications for Accident


Prevention Requirements
• Instruct all employees that caution signs indicate a possible hazard against which proper
precaution should be taken.
• All signs shall be furnished with rounded or blunt corners and shall be free from sharp edges,
burrs, splinters, or other sharp projections. The ends or heads of bolts or other fastening
devices shall be located in such a way that they do not constitute a hazard.
• The wording of any sign should be easily read and concise. The sign should contain sufficient
information to be easily understood. The wording should make a positive, rather than negative
suggestion and should be accurate in fact.
• Signify the actual or potential presence of a biohazard and identify equipment, containers,
rooms, materials, experimental animals, or combinations thereof, which contain, or are
contaminated with, viable hazardous agents by using a biological hazard warning .
• Use tags as a means to prevent accidental injury or illness to employees who are exposed to
hazardous or potentially hazardous conditions, equipment or operations which are out of the
ordinary, unexpected or not readily apparent.
• Tags shall contain a signal word and a major message.

Signs and Tags, Specifications for Accident


Prevention Requirements (cont.)
• The signal word shall be readable at a minimum distance of five feet (1.52 m) or such greater
distance as warranted by the hazard.
• The tag's major message shall be presented in either pictographs, written text or both.
• The signal word and the major message shall be understandable to all employees who may be
exposed to the identified hazard.

Occupational Exposure to Hazardous


Chemicals in Laboratories Requirements
• Measure the employee's exposure to any substance regulated by a standard which requires monitoring if there
is reason to believe that exposure levels for that substance routinely exceed the action level (or in the
absence of an action level, the PEL).
– If the initial monitoring prescribed by paragraph (d)(1) of this section discloses employee exposure over the action level (or in
the absence of an action level, the PEL), the employer shall immediately comply with the exposure monitoring provisions of the
relevant standard
• Where hazardous chemicals are used in the workplace, the employer shall develop and carry out the provisions
of a written Chemical Hygiene Plan which is:
– Capable of protecting employees from health hazards associated with hazardous chemicals in that laboratory and
– Capable of keeping exposures below specified limits
• The Chemical Hygiene Plan shall include each of the following elements and shall indicate specific measures
that the employer will take to ensure laboratory employee protection;
– SOPs relevant to safety and health considerations to be followed when laboratory work involves the use of hazardous
chemicals;
– Criteria used to determine and implement control measures to reduce employee exposure to hazardous chemicals including
engineering controls, the use of PPE and hygiene practices
– A requirement that fume hoods and other protective equipment are functioning properly and specific measures that shall be
taken to ensure proper and adequate performance of such equipment;
– Provisions for employee information and training
– The circumstances under which a particular laboratory operation, procedure or activity shall require prior approval from the
employer or the employer's designee before implementation;
– Provisions for medical consultation and medical examinations
– Designation of personnel responsible for implementation of the Chemical Hygiene Plan including the assignment of a Chemical
Hygiene Officer, and, if appropriate, establishment of a Chemical Hygiene Committee;
– Provisions for additional employee protection for work with particularly hazardous substances. These include "select
carcinogens," reproductive toxins and substances which have a high degree of acute toxicity.

Occupational Exposure to Hazardous Chemicals


in Laboratories Requirements (cont.)

• Employee training must include the following:


– Methods and observations that may be used to detect the presence or release of a hazardous chemical (such as monitoring
conducted by the employer, continuous monitoring devices, visual appearance or odor of hazardous chemicals when being
released, etc.);
– The physical and health hazards of chemicals in the work area; and
– The measures employees can take to protect themselves from these hazards, including specific procedures the employer has
implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency
procedures, and personal protective equipment to be used.
• Employees must be informed of the following at the time of an employee's initial assignment to a work area
where hazardous chemicals are present and prior to assignments involving new exposure situations:
– The contents of 29CFR 1910.1450 and its appendices which shall be made available to employees;
– The location and availability of the employer's Chemical Hygiene Plan;
– The permissible exposure limits for OSHA regulated substances or recommended exposure limits for other hazardous
chemicals where there is no applicable OSHA standard;
– Signs and symptoms associated with exposures to hazardous chemicals used in the laboratory; and
• The Chemical Hygiene Plan shall be readily available to employees, employee representatives and, upon request,
to the Assistant Secretary.
• Provide all employees who work with hazardous chemicals an opportunity to receive medical attention, including
any follow-up examinations which the examining physician determines to be necessary
• Establish and maintain for each employee an accurate record of any measurements taken to monitor employee
exposures and any medical consultation and examinations including tests or written opinions.
• Where the use of respirators is necessary to maintain exposure below permissible exposure limits, the
employer shall provide, at no cost to the employee, the proper respiratory equipment.

Lockout/Tagout Control Of Hazardous


Energy (LOTO) Requirements

Establish a program consisting of:


– Energy control procedures,
– Employee training and
– Periodic inspections
Procedures shall be developed, documented and utilized for the control of potentially hazardous energy.
The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to
be used for the control of hazardous energy, and the means to enforce compliance including, but not limited
to, the following:
– A specific statement of the intended use of the procedure;
– Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous
energy, including stored energy;
– Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility
for them; and
– Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices,
tagout devices, and other energy control measures.
Provide Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware for
isolating, securing or blocking of machines or equipment from energy sources.
Tagout devices shall warn against hazardous conditions if the machine or equipment is energized and shall
include a legend such as the following: Do Not Start. Do Not Open. Do Not Close. Do Not Energize. Do
Not Operate.

Lockout/Tagout Control Of Hazardous Energy


(LOTO) Requirements
Lockout/tagout devices shall not be used for other purposes; and shall meet the following requirements:
– Durable - capable of withstanding the work environment
– Standardized – only one color; shape; or size; and additionally, in the case of tagout devices, print and format
– Substantial - prevents removal without the use of excessive force or unusual techniques,
– Identifiable - indicate the identity of the employee applying the device(s).
Conduct a periodic inspection of the energy control procedure and shall meet the following requirements:
– Conduct at least annually to ensure that the procedure and the requirements of this standard are being followed.
– Inspection shall be performed by an authorized employee other than the ones(s) utilizing the energy control procedure being
inspected.
– Correct any deviations or inadequacies identified in the inspection.
– Where lockout/tagout is used for energy control, include a review, between the inspector and each authorized employee (for tagout
this includes affected employees also), of that employee's responsibilities under the energy control procedure being inspected.
Each authorized employee shall receive training in:
– The recognition of applicable hazardous energy sources,
– The type and magnitude of the energy available in the workplace, and
– The methods and means necessary for energy isolation and control.
Each affected employee shall be instructed in:
– The purpose and use of the energy control procedure.
All other employees whose work operations are or may be in an area where energy control procedures may be
utilized, shall be instructed about:
– The procedure, and
– The prohibition relating to attempts to restart or reenergize machines or equipment which are locked out or tagged out.

First Aid Requirements


• Ensure the ready availability of the following:
– Medical personnel for advice and consultation on matters of plant health.
– In the absence of an infirmary, clinic, or hospital in near proximity to the workplace which is used
for the treatment of all injured employees, a person or persons shall be adequately trained to
render first aid.
– Adequate first aid supplies
– Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable
facilities for quick drenching or flushing of the eyes (eyewash stations) and body (emergency
showers) shall be provided within the work area for immediate emergency use.

– Legal issues of applying first aid (Good Samaritan Laws).
– Basic anatomy.
– Patient assessment and first aid for the following:
• Respiratory arrest.

• Musculoskeletal injuries.
• Shock.
• Eye injuries.
• Burns.
• Loss of consciousness.

Fire Brigade Requirements


• Prepare and maintain a statement or written policy which establishes the following:
– Existence of a fire brigade;
– Basic organizational structure; (shall be available for inspection by the Assistant Secretary and by
employees or their designated representatives.)
– Type, amount, and frequency of training to be provided to fire brigade members;
– Expected number of members in the fire brigade; and
– Functions that the fire brigade is to perform at the workplace. The organizational statement assure that
employees who are expected to do interior structural fire fighting are physically capable of performing
duties which may be assigned to them during emergencies.
• Provide training and education (prior to members performing fire brigade emergency activities)
on the following topics:
– Duties and functions that fire brigade members are expected to perform. Fire brigade leaders and
training instructors training and education must be more comprehensive than that provided to the general
membership of the fire brigade.
– Special hazards such as storage and use of flammable liquids and gases, toxic chemicals, radioactive
sources, and water reactive substances, to which they may be exposed during fire and other
emergencies.
– Any changes that occur in relation to the special hazards.
– Additional requirements apply relative to PPE specifications/training for interior fire fighting
• All fire brigade members shall be provided with training at least annually (enough to ensure
that members can perform assigned duties/functions satisfactorily/in a safe manner so as not
to endanger others). In addition, fire brigade members who are expected to perform interior
structural fire fighting shall be provided with an education session or training at least
quarterly.

Fire Brigade Requirements


• Develop and make available for inspection by fire brigade members, written procedures that
describe the actions to be taken in situations involving the special hazards and shall include
these in the training and education program.
• Maintain and inspect, at least annually, fire fighting equipment to assure the safe operational
condition of the equipment. Portable fire extinguishers and respirators shall be inspected at
least monthly.
• The quality of the training and education program for fire brigade members shall be similar to
those conducted by such fire training schools as the Maryland Fire and Rescue Institute; Iowa
Fire Service Extension;

Portable Fire Extinguishers Requirements


• Provide/mount/locate/and identify portable fire extinguishers so that they are readily
accessible to employees without subjecting the employees to possible injury
• Only approved portable fire extinguishers shall be used .
• Shall not provide or make available in the workplace portable fire extinguishers using carbon
tetrachloride or chlorobromomethane extinguishing agents.
• Remove from service all soldered or riveted shell self-generating soda acid or self-generating
foam or gas cartridge water type portable fire extinguishers which are operated by inverting
the extinguisher to rupture the cartridge or to initiate an uncontrollable pressure generating
chemical reaction to expel the agent.
• Provide/select/distribute portable fire extinguishers for employee use based on the classes of
anticipated workplace fires and on the size and degree of hazard which would affect their use.
• CV uses ABC fire extinguishers, therefore they should be distributed every 50 feet.
• Distribute portable fire extinguishers or other containers of Class D extinguishing agent for
use by employees so that the travel distance from the combustible metal working area to any
extinguishing agent is 75 feet (22.9 m) or less. Portable fire extinguishers for Class D hazards
are required in those combustible metal working areas where combustible metal powders,
flakes, shavings, or similarly sized products are generated at least once every two weeks.
• Portable extinguishers or hose shall be visually inspected monthly.
• Ensure portable fire extinguishers are subjected to an annual maintenance check. Stored
pressure extinguishers do not require an internal examination. The employer shall record the
annual maintenance date and retain this record for one year after the last entry or the life of
the shell, whichever is less.

Portable Fire Extinguishers


Requirements (cont.)
• Assure that:
– Stored pressure dry chemical extinguishers that require a 12-year hydrostatic test are emptied and
subjected to applicable maintenance procedures every 6 years. Dry chemical extinguishers having non-
refillable disposable containers are exempt from this requirement.
– Portable fire extinguishers are maintained in a fully charged and operable condition and
kept in their designated places at all times except during use (this is completed by an
outside contractor – Al who and how frequently??).
– Alternate equivalent protection is provided when portable fire extinguishers are removed from service
for maintenance and recharging (Please refer to 29 CFR 1910.157 (f) for specifics on hydrostatic testing
and maintenance intervals).
– Hydrostatic testing is performed by trained persons with suitable testing equipment and facilities.
• Where the employer has provided portable fire extinguishers for employee use in the
workplace, the employer shall also provide an educational program to familiarize employees with
the general principles of fire extinguisher use and the hazards involved with incipient stage
fire fighting.
• This training shall be provided upon initial employment at least annually thereafter.

Emergency Alarm Systems Requirements


• The employee alarm shall be:
– Distinctive and recognizable as a signal to evacuate the work area or to perform actions designated under the emergency
action plan.
– Provide warning for necessary emergency action as called for in the emergency action plan, or for reaction time for safe
escape of employees from the workplace or the immediate work area, or both.
– Capable of being perceived above ambient noise or light levels by all employees in the affected portions of the workplace.
Tactile devices may be used to alert those employees who would not otherwise be able to recognize the audible or visual alarm.
• Explain to each employee the preferred means of reporting emergencies, such as
– manual pull box alarms,
– public address systems, radio or telephones.
• The employer shall post emergency telephone numbers near telephones, or employee notice boards, and other
conspicuous locations when telephones serve as a means of reporting emergencies. Where a communication
system also serves as the employee alarm system, all emergency messages shall have priority over all non-
emergency messages.
• Establish procedures for sounding emergency alarms in the workplace.
• Assure that
– All employee alarm systems are restored to normal operating condition as promptly as possible after each test or alarm.
– All employee alarm systems are maintained in operating condition except when undergoing repairs or maintenance.
– A test of the reliability and adequacy of non-supervised employee alarm systems is made every two months.
– Manually operated actuation devices for use in conjunction with employee alarms are unobstructed, conspicuous and readily
accessible.
• Back-up means of alarm, such as employee runners or telephones, shall be provided when systems are out of
service.

Powered Industrial Truck Operations


Requirements
• Training Program must address:
– Operating instructions, warnings, and precautions for the types of truck the operator will be authorized
to operate
– Differences between the truck and the automobile
– Truck controls and instrumentation: where they are located, what they do, and how they work
– Engine or motor operation
– Steering and maneuvering
– Visibility (including restrictions due to loading)
– Fork and attachment adaptation, operation, and use limitations
– Vehicle capacity
– Vehicle stability
– Any vehicle inspection and maintenance that the operator will be required to perform
– Refueling and/or charging and recharging of batteries
– Operating limitations
– Any other operating instructions, warnings, or precautions listed in the operator's manual for the types
of vehicle that the employee is being trained to operate
– Surface conditions where the vehicle will be operated
– Composition of loads to be carried and load stability
– Load manipulation, stacking, and unstacking
– Pedestrian traffic in areas where the vehicle will be operated
– Narrow aisles and other restricted places where the vehicle will be operated
– Hazardous (classified) locations where the vehicle will be operated
– Ramps and other sloped surfaces that could affect the vehicle's stability
– Closed environments and other areas where insufficient ventilation or poor vehicle maintenance could
cause a buildup of carbon monoxide or diesel exhaust
– Other unique or potentially hazardous environmental conditions

Powered Industrial Truck Operations


Requirements
• Only certified operators, per specified PID, may operate equipment.
• Written training programs must include formal and practical instruction, and operator evaluation in the
workplace.
• All trainers must have the knowledge and ability to teach and evaluate operators.
• Training must occur every 3 years and mandatory re-training when unsafe operations, accident or changes in
conditions or different vehicles occur.
• Ensure that all approved trucks bear a label or some other identifying mark indicating approval by the testing
laboratory.
• Modifications and additions which affect capacity and safe operation shall not be performed by the customer
or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates,
tags, or decals shall be changed accordingly.
• If the truck is equipped with front-end attachments other than factory installed attachments, the truck shall
be marked to identify the attachments and show the approximate weight of the truck and attachment
combination at maximum elevation with load laterally centered.
• The user shall see that all nameplates and markings are in place and are maintained in a legible condition.
• If the type of load presents a hazard, the user shall equip fork trucks with a vertical load backrest extension
manufactured in accordance with paragraph (a) (2) of this section.
• Battery charging installations shall be located in areas designated for that purpose.
• Facilities shall be provided for flushing and neutralizing spilled electrolyte, for fire protection, for protecting
charging apparatus from damage by trucks, and for adequate ventilation for dispersal of fumes from gassing
batteries.
• Operators must verify trailer chocks, supports, and dock plates prior to loading/unloading.
• Certain industrial trucks are not allowed to be used in various locations based on atmospheric conditions, Please
refer to 29 CFR 1910.178 Table N-1 for a summary of appropriate use of industrial trucks in various locations.

Welding Requirements
• Fire watchers-
– Are required for locations including but not limited to the following :
• Where other than a minor fire might develop
• Combustible materials closer that 35 ft to point of operation
• Combustibles that are 35 ft or more away but are easily ignited
• Wall or floor openings within 35 ft radius expose combustibles
– Must be trained in the use of fire extinguishing equipment (which shall be readily available) and be familiar with the facilities
for sounding an alarm in the event of a fire.
– Be maintained at least a half hour after the welding or cutting operation was completed.
• Before cutting or welding is permitted the area shall be inspected by position responsible for inspection and
granting authorized welding and and cutting operations. Precautions that are to be taken shall be in the form of
a written permit.
• Any welding, cutting or burning of lead base metals, zinc, cadmium, mercury, beryllium or exotic metals or
paints not listed here shall have proper ventilation or respiratory protection.
• Ensure First aid equipment available at all times
• Cutters, welders and their supervisors must be suitably trained in the safe operations of their equipment and
the safe use of the process.
• If the object to be welded or cut cannot readily be moved, all movable fire hazards should be removed.
• If the object to be welded or cut cannot be moved and if all the fire hazards cannot be removed, then guards
shall be used to confine the heat sparks and slag and to protect the immovable fire hazards.
• If the requirements stated in paragraphs (a)(1)(i) and(a)(1)(i) of this section cannot be followed the welding
and cutting should not be performed.
• Workers in charge of oxygen or fuel-gas equipment (including distribution piping systems and generators) must
be instructed and judged competent for such work

• Oxygen cylinders shall be stored in an upright secured position 20 feet from any flammable
gases or petroleum products
• Workman assigned to operate arc welding equipment must be properly instructed and qualified
to operate such equipment.
• Workman assigned must be familiar with this section (1919.254, and 1910.252(a, b, and c) If
gas shielded arc welding is done, they must be familiar with the American Society Standard
A6-1-1966.
• Operators of equipment should report any equipment defect or safety hazards and discontinue
use of equipment until its safety has been assured. Repairs shall be made only by qualified
personnel.

Electrical Safety (general) Requirements


• Electrical equipment shall be free from recognized hazards that are likely to cause death or
serious physical harm to employees.
• Safety of equipment shall be determined using the following considerations:
– Suitability for installation and use - listing or labeling for that identified purpose and shall be used or
installed in accordance with any instructions included in the listing or labeling.
– Mechanical strength and durability, including, for parts designed to enclose and protect other equipment,
the adequacy of the protection thus provided.
– Electrical insulation.
– Heating effects under conditions of use
– Arcing effects - classification by type, size, voltage, current capacity, specific use.
– Conductors shall be spliced or joined with splicing devices suitable for the use or by brazing, welding, or
soldering with a fusible metal or alloy.
– Parts of electric equipment which in ordinary operation produce arcs, sparks, flames, or molten metal
shall be enclosed or separated and isolated from all combustible material.
– Electrical equipment may not be used unless the following are identified on the equipment:
• Manufacturer's name, trademark, or other descriptive marking by which the organization responsible for the
product may be identified is placed on the equipment.
• Voltage, current, wattage, or other ratings as necessary.
• The marking shall be of sufficient durability to withstand the environment involved.
– Disconnecting means and circuits shall be properly identified this includes service, feeder, and branch
circuit, at its disconnecting means or overcurrent device,
– Sufficient access and working space shall be provided and maintained about all electric equipment to
permit ready and safe operation and maintenance of such equipment.
– Please refer to 29 CFR 1910.303 (g)(1)(i) Table S-1 for working clearances
– Working space required by this subpart may not be used for storage. When normally enclosed live parts
are exposed for inspection or servicing, the working space, if in a passageway or general open space, shall
be suitably guarded.

• At least one entrance of sufficient area shall be provided to give access to the working space
about electric equipment.
• Where there are live parts normally exposed on the front of switchboards or motor control
centers, the working space in front of such equipment may not be less than 3 feet.
• Illumination shall be provided for all working spaces about service equipment, switchboards,
panelboards, and motor control centers installed indoors.
• The minimum headroom of working spaces about service equipment, switchboards, panel-
boards, or motor control centers shall be 6 feet 3 inches.
• Except as required or permitted elsewhere in this subpart, live parts of electric equipment
operating at 50 volts or more shall be guarded against accidental contact by approved cabinets
or other forms of approved enclosures, or by any of the following means:
– By location in a room, vault, or similar enclosure that is accessible only to qualified persons.
– By suitable permanent, substantial partitions or screens so arranged that only qualified persons will have
access to the space within reach of the live parts.
– By location on a suitable balcony, gallery, or platform so elevated and arranged as to exclude unqualified
persons.
– By elevation of 8 feet or more above the floor or other working surface.
– In locations where electric equipment would be exposed to physical damage, enclosures or guards shall be
so arranged and of such strength as to prevent such damage.
– Entrances to rooms and other guarded locations containing exposed live parts shall be marked with
conspicuous warning signs forbidding unqualified persons to enter.
• For circuits that exceed 600V, additional requirements apply.

qualified person will be exposed.

Waste Contingency and Preparedness Requirements
• Develop a plan which shall cover those designated actions employers and employees must take
to ensure employee safety from fire and other emergencies.
• The following elements, at a minimum, shall be included in the plan:
– Emergency escape procedures and emergency escape route assignments;
– Procedures to be followed by employees who remain to operate critical plant operations before
they evacuate;
– Procedures to account for all employees after emergency evacuation has been completed;
– Rescue and medical duties for those employees who are to perform them;
– The preferred means of reporting fires and other emergencies; and
– Names or regular job titles of persons or departments who can be contacted for further
information or explanation of duties under the plan.
– The employer shall establish an employee alarm system which complies with 1910.165.
– A list of the major workplace fire hazards and their proper handling and storage procedures,
potential ignition sources (such as welding, smoking and others) and their control procedures, and
the type of fire protection equipment or systems which can control a fire involving them;
– Names or regular job titles of those personnel responsible for maintenance of equipment and
systems installed to prevent or control ignitions or fires; and
– Names or regular job titles of those personnel responsible for control of fuel source hazards.
– Maintenance procedures performed on equipment and systems installed on heat producing
equipment to prevent accidental ignition of combustible materials.

Plan Hazardous Waste Contingency and
Preparedness Requirements

• Control accumulations of flammable and combustible waste materials and residues so that they
do not contribute to a fire emergency. The housekeeping procedures shall be included in the
written fire prevention plan.
• If the employee alarm system is used for alerting fire brigade members, or for other purposes,
a distinctive signal for each purpose shall be used.
• Designate and train a sufficient number of persons to assist in the safe and orderly emergency
evacuation of employees, before implementing the emergency action plan.
• Review the plan with each employee covered by the plan at the following times:
– Initially when the plan is developed,
– Whenever the employee's responsibilities or designated actions under the plan change,
and
– Whenever the plan is changed.

Occupational Noise Exposure Requirements


• Train all employees required to wear hearing protection annually on its use and monitor them for potential hearing
loss (shall be updated as changes occur in PPE and work processes).
• Ensure that each employee is informed of the following:
– The effects of noise on hearing;
– The purpose of hearing protectors, the advantages, disadvantages, and attenuation of various types, and instructions on selection,
fitting, use, and care; and
– The purpose of audiometric testing, and an explanation of the test procedures.
– Program is provided at no cost to the employee.
• Implement a monitoring program to identify employees to be included in the hearing conservation program, when
information indicates that employee exposure may equal/exceed the 8 hr TWA of 85 dba.
• Establish a baseline audiogram within 6 months of employment, and prior to a total of 14 hours work exposure, for
employees who are exposed to sound levels greater than 85 dba on an 8-hr Time-Weighted Average (TWA) average
basis.
• Obtain new audiogram for all affected employees annually, and compare to prior audiograms to determine if a there
has been a shift in the standard threshold (decrease in hearing), if one has occurred
– The employee shall be notified within 21 days.
– Use of hearing protection shall be re-evaluated and/or refitted and if necessary a medical evaluation may be required.
• Maintain accurate record of all employee exposure measurements and that all records are maintained as required
by this regulation.
• Evaluate hearing protection for the specific noise environmental in which the protector will be used.

• Operator should be trained in safe work standards including the use of fire extinguishers
• Complete monthly inspections and keep a record of all hoist chains (including end connections).
Inspections should address the following:
– Excessive wear

– Distorted links interfering with proper function, and
– Stretch beyond manufacturer's recommendation.
• Complete monthly inspections and keep a record of all hooks with deformation or cracks.
• The certification records for inspections must include;
– Date of inspection
– Signature of person performing inspection
– The serial number or other identifier of hook inspected.
• Establish preventive maintenance program based upon the crane manufacturer’s
recommendations.
• Placement Warning or “Out of Order” signs on the crane.
• Keep/Maintain certification record(s) which include the date(s) of inspection and the signature
or person(s) who performed inspection. The same records must be kept on inspections of all
other ropes.

Hazardous Material Employee Requirements


• All employees do not need to be trained to the degree specified if arrangements have been made in advance for an
outside fully-trained emergency response team to respond in a reasonable period and all employees, who may come to
the incident first, have sufficient awareness training to recognize that an emergency response situation exists and
they have been instructed to call the designated outside fully-trained emergency response team for assistance.
• Develop and implement a written safety and health program for employees involved in hazardous waste operations
that shall be available for inspection by employees, their representatives and OSHA personnel.
• The program must include:
– Policy statements of the line of authority and accountability for implementing the program, the objectives of
the program and the role of the site safety and health officer or manager and staff;
– Means or methods for the development of procedures for identifying and controlling workplace hazards at the
site;
– Means or methods for the development and communication to employees of the various plans, work rules,
standard operating procedures and practices that pertain to individual employees and supervisors;
– Means for the training of supervisors and employees to develop the needed skills and knowledge to perform
their work in a safe and healthful manner;
– Means to anticipate and prepare for emergency situations and;
– Means for obtaining information feedback to aid in evaluating the program and for improving the effectiveness
of the program.
– Site analysis, engineering controls, maximum exposure limits, hazardous waste handling procedures and uses of
new technologies.

• The employer shall develop an emergency response plan for emergencies which shall address, as a
minimum, the following areas to the extent that they are not addressed in any specific program
required in this paragraph:

– Emergency recognition and prevention.
– Safe distances and places of refuge.
– Site security and control.

• Training for emergency response employees shall be completed before they are called upon to
perform in real emergencies. Such training shall include the elements of the emergency response
plan, standard operating procedures the employer has established for the job, the personal
protective equipment to be worn and procedures for handling emergency incidents.

Hazardous Waste Operations Requirements


• A person trained in hazardous waste management procedures, must instruct facility personnel
on hazardous waste management procedures (including contingency plan implementation)
relevant to the positions in which they are employed.
• Mark containers either with the words "Hazardous Waste" or with other words that identify
the contents of the containers.
• Waste must be placed in one of the following:
– Containers which comply with the applicable requirements of subparts I, AA, BB, and CC of 40 CFR Part
265; &/or
– Tanks which comply with the applicable requirements of subparts J, AA, BB, and CC of 40 CFR Part 265
except 265.197(c) and 265.200; and/or
– On drip pads which comply with subpart W or 40 CFR Part 265
• Prepare a manifest to transport hazardous waste for offsite treatment/storage/or disposal by.
– Classing/describing the hazardous material in accordance with Parts 172 and 173 of this subchapter, and
– Determining that the packaging or container is an authorized packaging, including Part 173 requirements,
and that it has been manufactured, assembled, and marked
• Package the waste in accordance with the applicable Department of Transportation regulations
on packaging under 49 CFR Parts 173, 178, and 179 before transporting hazardous waste or
offering hazardous waste for transportation off-site, a generator must.
• Label each package in accordance with the applicable Department of Transportation regulations
on hazardous materials under 49 CFR Part 172 before transporting or offering hazardous waste
for transportation off-site.

Universal Waste Requirements

• A small quantity handler of universal waste (batteries, fluorescent lamps, used oil) must:
– Manage universal waste in a way that prevents releases of any universal waste or component of a universal
waste to the environment,
– Contain any universal waste that shows evidence of leakage, spillage, or damage that could cause leakage
under reasonably foreseeable conditions in a container. The container must be closed, structurally sound,
compatible with the contents of the battery, and must lack evidence of leakage, spillage, or damage that
could cause leakage under reasonably foreseeable conditions.
– Inform all employees who handle or have responsibility for managing universal waste on proper handling
and emergency procedures appropriate to the type(s) of universal waste handled at the facility.
– Immediately contain all releases of universal wastes and other residues from universal wastes.
– Clearly label or mark each lamp or a container or package in which such lamps are contained with one of the
following phrases: "Universal Waste -- Lamp(s)," or "Waste Lamp(s)," or "Used Lamp(s).“
– Clearly label or mark Universal waste batteries (i.e., each battery), or a container in which the batteries
are contained, with any one of the following phrases: "Universal Waste -- Battery(ies), or "Waste
Battery(ies)," or "Used Battery(ies);“
• A small quantity handler of universal waste may accumulate universal waste for no longer than
one year from the date the universal waste is generated, or received from another handler,
unless the requirements of paragraph (b) of this section are met.

Abrasive Wheel Machinery Requirements


• Use Abrasive Wheel only on machines provided with safety guards as defined in the
following paragraphs of this section, except:
– Wheels used for internal work while within the work being ground
– Mounted wheels, used in portable operations, 2 inches and smaller in diameter; and
– Types 16, 17, 18, 18R, and 19 cones, plugs, and threaded hole pot balls where the work offers
protection.
• The safety guard shall cover the spindle end, nut, and flange projections.
• All abrasive wheels shall be mounted between flanges which shall not be less than one-
third the diameter of the wheel unless they are listed exception in section (c)(1)(i)
• On offhand grinding machines, work rests shall be used to support the work.
• Natural sandstone wheels and metal, wooden, cloth, or paper discs, having a layer of
abrasive on the surface are not covered by this section.
• Safety guards as specified in paragraphs (b) (1) through (10) of this section;
• Immediately before mounting, all wheels shall be closely inspected and sounded by the
user (ring test) to make sure they have not been damaged in transit, storage, or
otherwise.
• Abrasive wheel machinery guards shall meet the design specifications of the American
National Standard Safety Code for the Use, Care, and Protection of Abrasive Wheels,
ANSI B7.1-1970, which is incorporated by reference as specified in Sec. 1910.6.

Laser Requirements
• Only qualified and trained employees shall be assigned to install, adjust, and operate laser
equipment.
• Proof of qualification of the laser equipment operator shall be available and in possession
of the operator at all times.
• Employees, when working in areas in which a potential exposure to direct or reflected
laser light greater than 0.005 watts (5 milliwatts) exists, shall be provided with anti-
laser eye protection devices as specified in Subpart E of this part.
• Areas in which lasers are used shall be posted with standard laser warning placards.
• Beam shutters or caps shall be utilized, or the laser turned off, when laser transmission
is not actually required. When the laser is left unattended for a substantial period of
time, such as during lunch hour, overnight, or at change of shifts, the laser shall be
turned off.
• Only mechanical or electronic means shall be used as a detector for guiding the internal
alignment of the laser.
• The laser beam shall not be directed at employees.
• Laser equipment shall bear a label to indicate maximum output.
• Employees shall not be exposed to microwave power densities in excess of 10 milliwatts
per square centimeter.
• Employees shall not be exposed to light intensities above:
– Direct staring: 1 micro-watt per square centimeter;
– Incidental observing: 1 milliwatt per square centimeter;
– Diffused reflected light: 2 1/2 watts per square centimeter.
• Laser unit in operation should be set up above the heads of the employees, when possible.

Fire Protection Requirements


• All automatic sprinkler designs used to comply with this standard shall provide the necessary
discharge patterns, densities, and water flow characteristics for complete coverage in a
particular workplace or zoned subdivision of the workplace.
• Assure that only approved equipment and devices are used in the design and installation of
automatic sprinkler system.
• Properly maintain automatic sprinkler system installed including:
– Main drain flow test to be performed on each system annually.
– Opening the inspector's test valve at least every two years to assure that the sprinkler system operates
properly.
• Conducting proper acceptance tests on sprinkler systems and record the dates of such tests.
Proper acceptance tests include the following:
– Flushing of underground connections;
– Hydrostatic tests of piping in system;
– Air tests in dry-pipe systems;
– Dry-pipe valve operation; and
– Test of drainage facilities.
• Assure that every automatic sprinkler system is provided with at least one automatic water
supply capable of providing design water flow for at least 30 minutes. An auxiliary water supply
or equivalent protection shall be provided when the automatic water supply is out of service,
except for systems of 20 or fewer sprinklers.
• Assure that automatic sprinkler system piping is protected against freezing and exterior surface
corrosion.

Equipment Requirements

• Ensure safe condition of tools and equipment used by employees, including tools and equipment
which may be furnished by employees.

personal protective equipment.

Compressed gases (general) Requirements


• Inspect compressed gas cylinders to ensure that they are in a safe condition to the extent
that this can be determined by visual inspection. Visual and other inspections shall be
conducted as prescribed in the Hazardous Materials Regulations of the Department of
Transportation (49 CFR parts 171-179 and 14 CFR part 103).
• In-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks,
rail tankcars, or motor vehicle cargo tanks shall be in accordance with Compressed Gas
Association Pamphlet P-1-1965, which is incorporated by reference as specified in Sec. 1910.6.
• Compressed gas cylinders, portable tanks, and cargo tanks shall have pressure relief devices
installed and maintained in accordance with Compressed Gas Association Pamphlets S-1.1-1963
and 1965 addenda and S-1.2-1963, which is incorporated by reference as specified in Sec.
1910.6.

• Keep all places of employment, passageways, storerooms, and service rooms shall be kept clean
and orderly and in a sanitary condition.

or other dry standing places should be provided where practicable.

Confined Spaces Requirements


• Evaluate workplace to determine if any spaces are permit-required confined spaces
• If the workplace contains permit spaces, inform exposed employees by posting danger signs or
by equally effective means, of the existence and location of and the danger posed by the
permit spaces
• CV policy is to not allow CV employees to enter permit spaces, therefore management shall take
effective measures to prevent employees from entering the permit spaces
• When there are changes in the use or configuration of a non-permit confined space that might
increase the hazards to entrants, management shall reevaluate that space and, if necessary,
reclassify it as a permit-required confined space.
• When CV arranges to have a contractor perform work that involves permit space entry, CV
shall:
– Inform the contractor that the workplace contains permit spaces and that permit space intry is allowed
only through compliance with a permit space program
– Apprise the contractor of the elements, including the hazards identified and CV experience with the
space, that make the space in question a permit space;
– Inform the contractor of any precautions/procedures that CV has implemented for the protection of
employees in/near permit spaces where contractor personnel will be working
– Coordinate entry operations with the contractor, when both CV and contractor personnel will be working
in or near permit spaces
– Debrief the contractor at the conclusion of the entry operations regarding the permit space program
followed and regarding any hazards confronted or created in permit spaces during entry operations.

• Effectively manage any physical or chemical incident, product emergency or other critical situation which could

• To achieve this objective Novartis Emergency Management (NEM) has to ensure that:
– Adequate measures to minimize and master the consequences of an incident are taken;
– Necessary cooperation with authorities and public services is guaranteed;
– Authorities, personnel, media, public, shareholders and other stakeholders are rapidly informed in a comprehensive
and accurate manner;
– Necessary measures are taken to rectify any damage sustained.
• Management at all levels must ensure that the required structures and procedures are put in place, that team
members are nominated and trained, that documentation and facilities required are available, in order to
activate the system when necessary.
• An incident or product-related problem or any other problem associated with Novartis is to be reported
(following ANITA, or ISAN dependent on emergency), if it fulfills one or more of the following criteria:

– Considerations on the type and scale of handling of the biological agents, and
– The suitability of installations and equipment.
• Design facilities in which biological agents of different risk groups are handled to the level appropriate for the
highest risk group.
• Notify the local Biosafety Officer and train the employees accordingly prior to a change of the containment level (i.e.
temporary downgrading or planned upgrading).
• Clearly label Biological hazard zones. BL2 and higher at the entrance using the international biohazard warning sign
and indicate the type of containment (BL2, BL3 or BL4) , potential special hazards, and emergency contact persons
responsible for biosafety in the work area.
• Attach the biohazard warning sign if containers with hazardous or potentially hazardous biological agents/materials
are removed from biohazard areas (i.e., for autoclaving).
• Work practices and PPE must be in line with the biological material (risk group, exposure routes and means of
transmission) and the processes used.
• If hazardous biological agents (risk group 2 and above) are handled, detailed operating instructions (code of practice)
must be made available in the work areas.
– These operating instructions must include information on the potential effect of the biological agents and safety precautions in an
emergency.
– The operating instructions must be clear and written in a language understood by the workers. The instructions should be, wherever
appropriate, complemented by oral instructions.

• Establish a medical surveillance program for all sites where there is an unknown human risk potential

according to an established program.
• Maintenance and cleaning work in areas of category BL2 or higher may be carried out only after it has
been ensured that the maintenance or cleaning staff are not exposed to biohazardous material.
– The work may be carried out only under the supervision of a qualified person or by individuals
who have undergone suitable safety training.
• Biological material which might pose a risk to the environment must be inactivated by disinfection,
sterilization or incineration prior to final discharge into the environment.
• The ability to differentiate between material before and after inactivation must be assured, e.g. by
autoclave sensitive tape or other labeling.
• Methods used for disinfection and sterilization must be periodically validated (in-house or contractor
) to confirm their efficiency.
• Equipment for disinfection and sterilization must conform to the relevant standards and should be
certified by an appropriate institution.
• Written emergency procedures in case of spills or accidental release of hazardous biological material
must be established (first aid, prevention of spreading, decontamination and safe disposal of
potentially contaminated material). Provisions for proper documentation and internal and external
communication strategies must be included as part of the contingency plan.

Contained Use of Microorganisms, Cells, Recombinant Nucleic Acids, and Other Biological
Materials of Defined and Undefined Biohazard
• Principal investigators (competent persons who direct biological activities) are responsible for the
safe handling of microorganisms, cells, nucleic acids, and other potentially biohazardous material
including:
– Evaluation of risks,
• Identifying possible hazards.
• Assigning biological agents to one of the four risk groups.
• Assessing the suitability of the containment measures of the respective BL or BL-LS to limit damage to
human health or the environment.This is determined by the potential for exposure via 1) penetration
through the skin (via punctures, cuts, contamination of broken skin, etc.), 2) contact with mucous
membranes, 3) inhalation of aerosols, or 4) ingestion.
• Judging possible consequences for human health and the environment in the event of equipment failure
or human error.
– Adoption of measures,
• Implement appropriate measures to minimize the risks for personnel, the community and the
environment under the intended conditions and in case of conceivable incidents.
• Only well established methods of disinfection and sterilization may be applied.
• All materials containing biohazardous microorganisms, cells, recombinant nucleic acids, or other
potentially biohazardous materials must be inactivated by disinfection, sterilization, or incineration
prior to discharge into the environment (i.e., solid waste, wastewater, air).
• For transport within a given site of biohazardous See Guidance Note 4.5
• Emergency plans must be available in all areas where biohazardous microorganisms, cells, recombinant
nucleic acids, or other potentially biohazardous materials are handled. All staff of these areas must be
trained according to these plans.
– Allocation of adequate protective devices and equipment,
– Instruction and training of their associates (which must be completed/documented at least
once/year) and
– Reporting projects of potential risk to the local BSO.

Transportation of Biological Waste
• Observe internationally agreed regulations as well as local regulations regarding packaging,
labeling, procedures and documentation.
• Obtain clearance prior to import, export or interstate shipment for all regulated agents.
• Each site, company or sector must have its own written procedures for the transportation of
biological material, which must incorporate all local and international regulatory requirements.
Including:
– Packaging, labeling, receiving and shipping/receipt of biohazardous material
– Emergency situations during shipment and receipt of damaged packages with known or suspected
biohazard material.
– Keeping records of all training and shipping activities.
• Keep all shipping records a minimum of one year or as required by local laws and regulations.
• Investigate contractors to be used for the transportation and shipment of biological material
prior to placing a contract with them to insure that they hold official permits and observe all
regulations.
• Properly train persons shipping biological materials and waste. It is the employer’s
responsibility to ensure shippers are trained at least biannually. The training should include:
– Training for employees who identify and classify biological materials for shipment, pack, mark, and label
packages, and prepare documentation for local and international shipping.
– Function specific training, which should provide detailed training in the requirements applicable to the
function for which that person is responsible; and
– Safety training, which should cover the hazards presented by dangerous goods, safe handling and
emergency response procedures.

Warehousing
• Limit the quantity of goods stored in a compartment taking into consideration the:
– Hazardous properties of the stored goods
– Total fire load
– Fire protection of the compartment
– Business risks
• Segregate warehousing activities from storage areas, depending on the activities’ ignition risk and the fire
hazard of the stored goods.
• Proper layout of stock and good housekeeping are essential for the safe operation of a warehouse. Aisles must
be kept free and the access to emergency exits, fire extinguishers and hydrants must not be obstructed
• Packaging used for stored goods must allow safe storage.
• Depending on the risk, early detection of spills shall be ensured by
– regular surveillance
– gas detectors
– leak detection systems.
• In warehouses and open-air storage areas, smoking and open flames are forbidden. A hot work permit system
must be implemented.
• Warehouses must be adequately protected against lightning strikes.
• Shrink wrapping using open flames is not permitted.
• Sufficient detection devices, alarm procedures, and adequate fire extinguishing systems must be ensured in
conformity with the protection concept.

Hazardous Waste
• Classify wastes according to the local legislation.
• Follow Local laws and regulations.
• Develop a coordinated waste management concept for an entire area of responsibility.
• Carry out transfers of wastes only after the full line of responsibilities has been established. They have to be
tracked with a waste manifest.
• Do not transfer pollutants from one environmental medium to another without a net reduction of the
environmental burden.
• Do not start new processes and activities until a safe and environmentally sound waste management is
guaranteed.
• Allocate costs for waste management, whenever feasible, to the products, processes or activities causing
them.
• Implement procedures for segregation/collection/storage of waste, in order to ensure optimal recycling,
treatment and disposal.
• Where technically/economically feasible, waste is to be recycled/treated on site, in order to minimize risks
for future liabilities.
• When designing new products and processes or activities, waste is to be minimized as far as technically and
economically feasible.
• Do not mix or dilute waste with inert materials for the sole purpose of compliance with legal limits for final
disposal. If you do mix wastes evaluate to ensure chemical compatibility.
• Ensure the suitability of containers, which are used for the transportation and storage of wastes.
• Appropriately label wastes to ensure safe handling.
• Transport wastes according to local and/or international legislation as well as specific sectorial guidance, i.e..
for the transportation of dangerous goods.

B If you discover a fire

1 Raise the Alarm

Upon detecting any fire or smoke, break the glass of the


nearest fire alarm call point or call the extension "201", "202",
Security "265", atau TIS "233".
2 Notify your supervisor immediately

3 If possible, attack the fire

Use the nearest fire extinguisher of fire hose reel to put out the
fire. However, do not put yourself at risk.
4 Evacuate
Should the fire get out of control, close all door behind you and
leave the building immediately via the exit staircase.

C If you hear the fire alarm

1 Fire alarm activated

If you hear the fire alarm, remain alert and prepare yourself to
evacuate. If directed to evacuate via the public address system,

4 Remember that the few people are burned to death in fires.


Most people die from smoke, poisonous gases and panic.
Panic is usually the result of not knowing what to do.

5 If you have an escape plan and adapt it to the emergency, you


can greatly increase your chances of survival.
